Anopheles

A mosquito genus in the subfamily Anophelinae. Only certain species transmit human malaria and filariasis.... anopheles

Hybrid

This is produced by a cross-fertilization between two species. This happens a lot more often than botanists would like, since a species is presumed to have distinct genetic characteristics and shouldn’t do this hybridizing thing as often as it does. Most of the dozen or so species of Silk Tassel are really genetically the same, and the three hundred species of Aconite worldwide are all capable of hybridizing as well.... hybrid

Malaria

A protozoan disease of humans caused by blood parasites of the species, Plasmodium falciparum, P. vivax, P. ovale or P. malariae and transmitted by anopheline mosquitoes. P. falciparum is most likely to cause death, if untreated,. and can also be a great mimicker in its presentation. Malaria should be suspected in anyone with a fever or who is otherwise unwell and has returned from a malarious area.... malaria

Alstonia

Importance: Alstonia is a large shrub with straight bole and growing upto about 6m height. The roots are useful in skin diseases, erysipelas, leprosy, cobra bite and other venomous bites, epilepsy, fatigue, fever and otalgia. The fruits are useful in syphilis, insanity and epilepsy. The plant is believed to repel snakes.

Distribution: The plant is distributed throughout India in deciduous forests in areas up to 1800m elevation.

Botany: Alstonia venenata R.Br., belonging to the family Apocynaceae, is a large shrub to small tree up to 6m in height with greyish brown bark and bright yellow hard and woody root. Leaves are simple, arranged in whorls of 3-6, membranous, lanceolate, margins wavy, finely acuminate, main nerves numerous, close, parallel and united by inter marginal nerve. Flowers are white, arranged in terminal sub umbellate cymes or in racemes. Fruits are fusiform with stalked and beaked follicles, tapering at both ends. Seeds are many flattened with a tuft of hair at each end (Warrier et al, 1993). Other important species belonging to the genus Alstonia are the following.

1. A. scholaris R. Br.

This tree is common throughout India. The bark is valuable in debility and after effects of fever, chronic diarrhoea, dysentery and catarrhal fever. The milky juice is applied to ulcers and rheumatic pains, mixed with oil and dropped into ear to relieve earache. Ditanin is the active principle of the bark, possessing powerful febrifuge properties. The bark is astringent, tonic and febrifuge (Nadkarni, 1998).

2. A. spectabilis R. Br.

It is a large evergreen tree seen in tropical forests of Andamans. The bark contains alkaloids such as alstonamine, ditamine, echitamine and echitenine (Chopra et al,1980)

Agrotechnology: The plant is propagated mainly by seeds. Seeds are to be sown on seedbeds and germinated ones are to be transferred to polybags. About three months old seedlings are used for transplanting. If seeds are not available, thin stem cuttings can be planted in polybags and rooted cuttings used. Pits of size 60cm cube are to be taken at 3m spacing, filled with dried cowdung, sand and topsoil and made into a mound. To this mounds seedlings from polybags are to be transplanted. Irrigation is essential during early stages of growth. Application of organic manure every year is beneficial. Regular weeding is to be done. The plant is not attacked by any serious pests or diseases. Flowers are formed in the first year itself. It can be used for medicinal purposes after seven years of growth. Fruits and roots are the economical parts (Prasad et al,1997).

Properties and activity: The plant is a rich source of indole alkaloids. Alkaloids are present in various parts. Stem bark and root contain venenatine, alstovenine, 3-dehydroalstovenine and reserpine. Stem bark contains venoxidine (venenatine Nb -oxide), anhydroalstonatine, kopsinine, venalstonine, venalstonidine(venalstonine-6,7-epoxide), echitovenine and veneserpine. Fruits contain echitovenidine, (+)minovincinine, echitoserpidine, echitoserpine, echitoveniline, 11-methoxy echitovonidine, 11-methoxy (-) minovinicinine, echitoserpiline, (-)vincadifformine, 11-methoxy(-)vincadifformine and venoterpine. Leaves contain echitovenaldine, echitoveniline, alstolenine, deacetylakuammiline, polynuridine, dihydropolynuridine and raucaffrininoline. The yellow tint in bark is because of the presence of 3-alstovenine. A number of indole alkaloids have been further isolated from the plant. In addition to alkaloids fruits contain -amyrin acetate and lupeol ester of -hydroxy acid (Husain et al,1992).

The root is bitter, astringent, thermogenic, depurative, antitoxic, febrifuge and anodyne. The alkaloid alstovenine in lower doses exhibited monoamine oxidase inhibitor activity, while in higher doses it showed marked central stimulant effect. Veninatine exhibited reserpine like activity. Alcoholic extract of the fruits showed initial activation effect on acetylcholine esterase, followed alternately by inhibition and activation of the enzyme.... alstonia

Asparagus

Description: The spring growth of this plant resembles a cluster of green fingers. The mature plant has fernlike, wispy foliage and red berries. Its flowers are small and greenish in color. Several species have sharp, thornlike structures.

Habitat and Distribution: Asparagus is found worldwide in temperate areas. Look for it in fields, old homesites, and fencerows.

Edible Parts: Eat the young stems before leaves form. Steam or boil them for 10 to 15 minutes before eating. Raw asparagus may cause nausea or diarrhea. The fleshy roots are a good source of starch.... asparagus

Bacteriophage

A VIRUS which invades a bacterium (see BACTERIA). Containing either single-stranded or double-stranded DNA or RNA, a particular phage generally may infect one or a limited number of bacterial strains or species. After infection, once phage nucleic acid has entered the host cell, a cycle may result whereby the bacteria are programmed to produce viral components, which are assembled into virus particles and released on bacterial lysis (disintegration). Other (temperate) phages induce a non-lytic, or lysogenic, state, in which phage nucleic acid integrates stably into and replicates with the bacterial chromosome. The relationship can revert to a lytic cycle and production of new phages. In the process the phage may carry small amounts of donor bacterial DNA to a new host: the production of diphtheria toxin by Corynebacterium diphtheriae and of erythrogenic toxin by Streptococcus pyogenes are well-known examples of this e?ect.... bacteriophage

Bamboo

Description: Bamboos are woody grasses that grow up to 15 meters tall. The leaves are grasslike and the stems are the familiar bamboo used in furniture and fishing poles.

Habitat and Distribution: Look for bamboo in warm, moist regions in open or jungle country, in lowland, or on mountains. Bamboos are native to the Far East (Temperate and Tropical zones) but have bean widely planted around the world.

Edible Parts: The young shoots of almost all species are edible raw or cooked. Raw shoots have a slightly bitter taste that is removed by boiling. To prepare, remove the tough protective sheath that is coated with tawny or red hairs. The seed grain of the flowering bamboo is also edible. Boil the seeds like rice or pulverize them, mix with water, and make into cakes.

Other Uses: Use the mature bamboo to build structures or to make containers, ladles, spoons, and various other cooking utensils. Also use bamboo to make tools and weapons. You can make a strong bow by splitting the bamboo and putting several pieces together.

Green bamboo may explode in a fire. Green bamboo has an internal membrane you must remove before using it as a food or water container.... bamboo

Cryptococcus

A capsulate yeast which can infect humans. Can give rise to a cry ptococcoma in the lung and maylead to cryptococcal meningitis. Campylobacter

A species of bacterium found in farm and pet animals, from which it can be transmitted to humans, in whom it is a major cause of bacterial FOOD POISONING: outbreaks of infection have followed drinking unpasteurised milk from infected cows and eating undercooked meat and poultry. It causes diarrhoea.

In the United Kingdom, the number of cases of food poisoning (by all types of infection) has risen from 102.9 to 162.9 per 100,000 population over the last 15 years. In 2003, more then 70,000 cases of food poisoning were noti?ed. The use of preventive methods throughout the food production process, marketing and consumption of food is most important in controlling infection, as is taking hygienic precautions, such as hand-washing, after handling animals – including domestic pets.

Mild cases can be treated at home with no solid food but plenty of liquids and some salt. Serious cases require hospital care.... campylobacter

The genus Ficus constitutes an important group of trees with immense medicinal value. It is a sacred tree of Hindus and Buddhists. Among the varied number of species, the most important ones are the four trees that constitute the group “Nalpamaram”, namely, F. racemosa, F. microcarpa, F. benghalensis and F. religiosa (Athi, Ithi, Peral and Arayal respectively).

Gular fig, Cluster fig or Country fig, which is considered sacred, has golden coloured exudate and black bark. It is distributed all over India. Its roots are useful in treating dysentery. The bark is useful as a wash for wounds, highly efficacious in threatened abortions and recommended in uropathy. Powdered leaves mixed with honey are given in vitiated condition of pitta. A decoction of the leaves is a good wash for wounds and ulcers. Tender fruits (figs) are used in vitiated conditions of pitta, diarrhoea, dyspepsia and haemorrhages. The latex is administered in haemorrhoids and diarrhoea (Warrier et al, 1995). The ripe fruits are sweet, cooling and are used in haemoptysis, thirst and vomiting (Nadkarni, 1954; Aiyer et al, 1957; Moos, 1976). Nalpamaradi coconut oil, Candanasava, Valiya Arimedastaila, Dinesavalyadi Kuzhambu, Abhrabhasma, Valiya candanaditaila, etc. are some important preparations using the drug (Sivarajan et al, 1994).

It is a moderate to large-sized spreading laticiferous, deciduous tree without many prominent aerial roots. Leaves are dark green and ovate or elliptic. Fruit receptacles are 2-5cm in diameter, sub- globose or pyriform arranged in large clusters on short leafless branches arising from main trunk or large branches. Figs are smooth or rarely covered with minute soft hairs. When ripe, they are orange, dull reddish or dark crimson. They have a pleasant smell resembling that of cedar apples. The bark is rusty brown with a fairly smooth and soft surface, the thickness varying from 0.5-2cm according to the age of the trunk or bark. Surface is with minute separating flakes of white tissue. Texture is homogeneously leathery (Warrier et al, 1995).

Stem-bark gives gluanol acetate, -sitosterol, leucocyanidin-3-O- -D-glucopyrancoside, leucopelargonidin-3-O- -D-glucopyranoside, leucopelargonidin -3-O- -L-rhamnopyranoside, lupeol, ceryl behenate, lupeol acetate and -amyrin acetate. Stem- bark is hypoglycaemic and anti-protozoal. Gall is CVS active. Bark is tonic and used in rinder pest diseases of cattle. Root is antidysenteric and antidiabetic. Leaf is antibilious. Latex is antidiarrhoeal and used in piles. Bark and syconium is astringent and used in menorrhagia (Husain et al, 1992).

Plaksah is the Ficus species with few branches and many adventitious roots growing downward. It is widely distributed throughout India and in Sri Lanka, S. China, Ryuku Isles and Britain. Plakasah is one of the five ingredients of the group panchvalkala i.e, five barks, the decoction of which is extensively used to clear ulcers and a douche in leucorrhoea in children. This decoction is administered externally and internally with satisfactory results. Plaksah is acclaimed as cooling, astringent, and curative of raktapitta doshas, ulcers, skin diseases, burning sensation, inflammation and oedema. It is found to have good healing property and is used in preparation of oils and ointments for external application in the treatment of ulcers (Aiyer and Kolammal, 1957). The stem-bark is used to prepare Usirasava, Gandhataila, Nalpamaradi taila, Valiya marmagulika, etc. (Sivarajan et al, 1994). The bark and leaves are used in wounds, ulcers, bruises, flatulent colic, hepatopathy, diarrhoea, dysentery, diabetes, hyperdipsia, burning sensation, haemaorrhages, erysipelas, dropsy, ulcerative stomatitis, haemoptysis, psychopathy, leucorrhoea and coporrhagia (Warrier et al,1995) F. microcarpa is a large glabrous evergreen tree with few aerial roots. Leaves are short- petioled, 5-10cm long, 2-6cm wide and apex shortly and bluntly apiculate or slightly emarginate. Main lateral nerves are not very prominent and stipules are lanceolate. Fruit receptacles are sessile and globose occurring in axillary pairs. It is yellowish when ripe without any characteristic smell. Bark is dark grey or brown with a smooth surface except for the lenticels. Outer bark is corky and crustaceous thin and firmly adherent to inner tissue. Inner bark is light and flesh coloured with firbrous texture (Warrier et al, 1995). It is also equated with many other species of the genus. viz. F. Singh and Chunekar, 1972; Kapoor and Mitra, 1979; Sharma, 1983).

The bark contains tannin, wax and saponin. Bark is antibilious. Powdered leaves and bark is found very good in rheumatic headache. The bark and leaves are astringent, refrigerant, acrid and stomachic.

3. Ficus benghalensis Linn.

Banyan tree is a laticiferous tree with reddish fruits, which is wound round by aerial adventitious roots that look like many legs. It is found in the Sub-Himalayan tract and Peninsular India. It is also grawn throughout India. It is widely used in treatment of skin diseases with pitta and rakta predominance. Stem-bark, root -bark, aerial roots, leaves, vegetative buds and milky exudate are used in medicine. It improves complexion, cures erysepelas, burning sensation and vaginal disorders, while an infusion of the bark cures dysentery, diarrhoea, leucorrhoea, menorrhagia, nervous disorders and reduces blood sugar in diabetes. A decoction of the vegetative buds in milk is beneficial in haemorrhages. A paste of the leaves is applied externally to abcesses and wounds to promote suppuration, while that of young aerial roots cure pimples. Young twigs when used as a tooth brush strengthen gum and teeth (Nadkarni, 1954; Aiyer and Kolammal, 1957; Mooss,1976). The drug forms an important constituent of formulations like Nalpamaradi Coconut oil, Saribadyasava, Kumkumadi taila, Khadi ra gulika, Valiyacandanadi taila, Candanasava, etc. (Sivarajan et al, 1994). The aerial roots are useful in obstinate vomiting and leucorrhoea and are used in osteomalacia of the limbs. The buds are useful in diarrhoea and dysentery. The latex is useful in neuralgia, rheumatism, lumbago, bruises, nasitis, ulorrhagia, ulitis, odontopathy, haemorrhoids, gonorrhoea, inflammations, cracks of the sole and skin diseases (Warrier et al, 1995).

It is a very large tree up to 30m in height with widely spreading branches bearing many aerial roots functioning as prop roots. Bark is greenish white. Leaves are simple, alternate, arranged often in clusters at the ends of branches. They are stipulate, 10-20cm long and 5-12.5cm broad, broadly elliptic to ovate, entire, coriaceous, strongly 3-7 ribbed from the base. The fruit receptacles are axillary, sessile, seen in pairs globose, brick red when ripe and enclosing male, female and gall flowers. Fruits are small, crustaceous, achenes, enclosed in the common fleshy receptacles. The young bark is somewhat smooth with longitudinal and transverse row of lenticels. In older bark, the lenticels are numerous and closely spaced; outer bark easily flakes off. The fresh cut surface is pink or flesh coloured and exudes plenty of latex. The inner most part of the bark adjoining the wood is nearly white and fibrous (Warrier et al, 1995).

The bark yields flavanoid compounds A, B and C; A and C are identified as different forms of a leucoanthocyanidin and compound B a leucoanthocyanin. All the 3 were effective as hypoglycaemic agents. Leaves give friedelin, -sitosterol, flavonoids- quercetin-3-galactoside and rutin. Heart wood give tiglic acid ester of taraxasterol. Bark is hypoglycemic, tonic, astringent, antidiarrhoeal and antidiabetic. Latex is antirheumatic. Seed is tonic. Leaf is diaphoretic. Root fibre is antigonorrhoeic. Aerial root is used in debility and anaemic dysentery (Husain et al, 1992).

Peepal tree or Sacred fig is a large deciduous tree with few or no aerial roots. It is common throughout India, often planted in the vicinity of the temples. An aqueous extract of the bark has an antibacterial activity against Staphylococcus aureus and Escherichia coli. It is used in the treatment of gonorrhoea, diarrhoea, dysentery, haemorrhoids and gastrohelcosis. A paste of the powdered bark is a good absorbent for inflammatory swellings. It is also good for burns. Leaves and tender shoots have purgative properties and are also recommended for wounds and skin diseases. Fruits are laxative and digestive. The dried fruit pulverized and taken in water cures asthma. Seeds are refrigerant and laxative. The latex is good for neuralgia, inflammations and haemorrhages (Warrier et al, 1995). Decoction of the bark if taken in honey subdues vatarakta (Nadkarni, 1954; Aiyer and Kolammal, 1957; Mooss, 1976; Kurup et al, 1979). The important preparations using the drug are Nalpamaradi taila, Saribadyasava, Candanasava, Karnasulantaka, Valiyamarma gulika etc (Sivarajan et al, 1994). branches bearing long petioled, ovate, cordate shiny leaves. Leaves are bright green, the apex produced into a linear-lanceolate tail about half as long as the main portion of the blade. The receptacles occurring in pairs and are axillary, depressed globose, smooth and purplish when ripe. The bark is grey or ash coloured with thin or membranous flakes and is often covered with crustose lichen patches. The outer bark is not of uniform thickness, the middle bark in sections appear as brownish or light reddish brown. The inner part consists of layers of light yellowish or orange brown granular tissue (Warrier et al, 1995).

Bark gives -sitosterol and its glucoside. Bark is hypoglycaemic. Stem bark is antiprotozoal, anthelmintic and antiviral. Bark is astringent, antigonorrheic, febrifuge, aphrodisiac and antidysenteric. Syconium, leaf and young shoot is purgative (Husain et al, 1992).

Agrotechnology: Ficus species can be cultivated in rocky areas, unused lands, or other wastelands of the farmyard. The plant is vegetatively propagated by stem cuttings. A few species are also seed propagated. Stem cuttings of pencil thickness taken from the branches are to be kept for rooting. Rooted cuttings are to be transplanted to prepared pits. No regular manuring is required. Irrigation is not a must as a plant is hardy. The plant is not attacked by any serious pests or diseases. Bark can be collected after 15 years. Ficus species generally has an economic life span of more than hundred years. Hence bark can be regularly collected from the tree. Root, bark, leaves, fruits and latex form the economic parts (Prasad et al,1995).... ficus

Fungus

A simple plant that is parasitic on other plants and animals. Included in this group are mildews, moulds, mushrooms, toadstools and yeasts. Unlike other plants, they do not contain the green pigment chlorophyll. Most of the world’s 100,000 di?erent species of fungus are harmless or even bene?cial to humans. Fusobacterium

A species of gram-negative, rod-shaped BACTERIA. It occurs among the normal ?ora of the human mouth, COLON and reproductive tract. Importance: Fenugreek or Greek Hayes is cultivated as a leafy vegetable, condiment and as medicinal plant. The leaves are refrigerant and aperient and are given internally for vitiated conditions of pitta. A poultice of the leaves is applied for swellings and burns. Seeds are used for fever, vomiting, anorexia, cough, bronchitis and colonitis. In the famous Malayalam treatises like ‘Padhyam’ ‘Kairali’ and ‘Arunodhayam’, uluva is recommended for use as kalanusari in Dhanvantaram formulations of ‘Astaghradayam’. An infusion of the seeds is a good cool drink for small pox patients. Powdered seeds find application in veterinary medicine. An aqueous extract of the seeds possesses antibacterial property (Kumar et al, 1997; Warrier et al, 1995).

Distribution: Fenugreek is a native of South Eastern Europe and West Asia. In India fenugreek is grown in about 0.30 lakh ha producing annually about 30,000 tonnes of seeds. The major states growing fenugreek are Rajasthan, Madhya Pradesh, Gujarat, Uttar Pradesh, Maharashtra, Punjab and Karnataka. It is grown wild in Kashmir and Punjab.

Botany: Trigonella foenum-graecum Linn. belongs to family, Fabaceae. It is an annual herb, 30-60cm in height, leaves are light green, pinnately trifoliate, leaflets toothed, flowers are white or yellowish white, papilionaceous and axillary. Fruits are legumes, 5-7.5cm long, narrow, curved, tapering with a slender point and containing 10-20 deeply furrowed seeds per pod. There are two species of the genus Trigonella which are of economic importance viz. T. foenum graecum, the common methi and T. corniculata, the Kasuri methi. These two differ in their growth habit and yield. The latter one is a slow growing type and remains in rosette condition during most of the vegetative growth period (Kumar et al, 1997; Warrier et al, 1995).

Agrotechnology: Fenugreek has a wide adaptability and is successfully cultivated both in the tropics as well as temperate regions. It is tolerant to frost and freezing weather. It does well in places receiving moderate or low rainfall areas but not in heavy rainfall area. It can be grown on a wide variety of soils but clayey loam is relatively better. The optimum soil pH should be 6-7 for its better growth and development. Some of the improved cultivars available for cultivation are CO1 (TNAU), Rajendra Kanti (RAU), RMt-1(RAU) and Lam Selection-1 (APAU). Land is prepared by ploughing thrice and beds of uniform size are prepared. Broadcasting the seed on the bed and raking the surface to cover the seeds is normally followed. But to facilitate intercultural operations, line sowing is also advocated in rows at 20-25cm apart. Sowing in the plains is generally in September-November while in the hills it is from March. The seed rate is 20-25kg/ha and the seeds germinate within 6-8 days. Besides 15t of FYM, a fertiliser dose of 25:25:50kg NPK/ha is recommended. Entire P,K and half N are to be applied basally and the remaining half N 30 days after sowing. First irrigation is to be given immediately after sowing and subsequent irrigations at 7-10 days interval. Hoeing and weeding are to be done during the early stages of plant growth and thinning at 25-30 days to have a spacing of 10-15cm between plants and to retain 1-2 plants per hill. Root rot (caused by Rhizoctonia solani) is a serious disease and can be controlled by drenching carbendazim 0.05% first at the onset of the disease and another after one monthof first application. In about 25-30 days, young shoots are nipped off 5cm above ground level and subsequent cuttings of leaves may be taken after 15 days. It is advisable to take 1-2 cuttings before the crop is allowed for flowering and fruiting when pods are dried, the plants are pulled out, dried in the sun and seeds are threshed by beating with stick or by rubbing with hands. Seeds are winnowed, cleaned and dried in the sun. They may be stored in gunny bags lined with paper. An yield of 1200-1500kg of seeds and about 800-1000kg of leaves may be obtained per hectare in crops grown for both the purposes (Kumar et al, 1997).

Properties and activity: Seeds contain sapogenins-diosgenin, its 25-epimer(yamogenin), tigogenin, gitogenin, yuccagenin, 25-2-spirosta-3-5-diene and its -epimer. Seeds also contain a C27-steroidal sapogenin-peptide ester-fenugreekine. Seeds, in addition, contain 4-hydroxyleucine and saponins-fenugrins A-E:two furostanol glycoxides-trigonelloxide C and (255)-22-O-methyl-52-firostan-3 ,22,26,triol-3-O- -rhamnopyrans syl(1-2) C- -D-glucopyranosyl (1-3)- -D- glucopyranoxide-26-O- -D-glucopyranoxide.

Other chemical constituents are sterols- -sitosterol and cholesterol, flavone C- glycosides-vitexin, iso-vitexin, vitexin-2”-O-P-coumarate and vicenin-2. Flavonoids- quercetin and luteolin, flavonoid glycoside-vicenin-I. Invitro seedling callus culture gave flavonoids-luteolin and vitexin-1-glycoside. An essential oil is also reported from seeds. Leaves gave saponins-gracecunins A-G, flavonoids- kaempferol and quercetin; sterols- - sitosterol, sapogenins-diosgenin, gitogenin coumarin-scopoletin is also reported from the plant.

Seeds are bitter, mucilaginous, aromatic, carminative, tonic, diuretic, thermogenic, galactagogue, astringent, emollient, amophrodisiac, antirheumatic, CNS depressant and antiimplantation. Fenugreekine is hypoglycaemic, diuretic, hypotensive, cardiotonic, antiphlogistic. It showed 80% inhibition of vaccina virus.... fenugreek

Importance: Garlic is one of the important bulb crops used as a spice or condiment with medicinal value throughout the world. It possesses high nutritive value. Its preparations are useful in vitiated conditions of kapha and vata, cough, whooping cough, bronchitis, asthma, fever, facial paralysis, flatulence, colic, constipation, atonic dyspepsia, helminthiasis, duodenal ulcers, pulmonary and laryngeal tuberculosis, opthalmopathy, cardiopathy, fatigue, leucoderma, leprosy, hysteria, haemorrhoids, sciatica, otalgia, lumbago, swellings, splenopathy, hepatopathy, pneumonopathy, anthralgia, sore eyes, ear ache and dental caries (Kumar et al, 1997).

Distribution: Garlic is a native of Southern Europe and it is cultivated all over the world.. It is grown throughout India; Gujarat and Orissa being the leading states.

Botany: The genus Allium of Liliaceae family comprises a number of species. The important ones are the following:

Allium sativum is a scapigerous foetid perennial medicinal herb with underground compound bulbs covered over by outer white thin scales and with simple smooth, round stem, surrounded at the bottom by tubular leaf sheath. The leaves are simple, long, flat and linear. The flowers are small and white arranged in rounded umbels mixed with small bulbils. The entire umbel is enclosed in a tear-drop-shaped membranous spathe. Flowers are usually sterile (Warrier et al, 1993).

Agrotechnology: Garlic can be grown under a wide range of climatic conditions. It prefers moderate temperature in summer as well as in winter. Short days are very favourable for the formation of bulbs. Garlic requires well drained loamy soils rich in humus, with fairly good content of potash. Garlic is propagated by cloves or bulblets. In the hills, sowing is done in April and May. Types with bold and compact cloves and thick white covering sheath are preferred for planting. Ootty-1 garlic is an improved variety by clonal selection released from TNAU, Coimbatore. Garlic may be broadcast, planted in furrows or dibbled at the rate of 150-200kg cloves/ha. In furrow planting, cloves are dropped 7.5-10cm apart in furrows 15cm deep and covered lightly with loose soil. Cloves may be dibbled 5 to 7.5cm deep and 7.5cm apart in rows which are 15cm apart with their growing end upwards and then covered with loose soil. A basal dose of 60kg N and 50kg each of P2O5 and K2O are applied along with 25t/ha of FYM. 60kg N is given as topdressing 45 days after planting. First irrigation is given immediately after sowing and subsequent irrigations are given at 10-15 days interval depending upon the soil moisture availability. The last irrigation should be given 2-3 days before harvesting to facilitate easy harvest and minimum damage to bulbs. First weeding and hoeing is to be done at one mo nth after sowing followed by a second weeding one month after first interculture. Hoeing at about two and a half months from sowing loosens the soil and helps in setting of bigger and well-filled bulbs. Garlic is attacked by Thrips tabacii which causes withering of leaves. Application of methyl demeton 25EC or dimethoate 30EC at 1ml/l will check the pest incidence. Leaf spot caused by Alternaria solanii can be controlled by spraying Dithane M.45 at fortnightly intervals at 2.5g/l of water. Garlic is harvested when the tops turn yellowish or brownish and show signs of drying up. The plants are uprooted, tied into small bundles and kept in shade for 2-3 days for curing. Average yield of garlic is 6-8t/ha. (Kumar et al, 1997.)

Properties and activity: Garlic bulb is reported to contain volatile oil, alliin (S-allyl-L-cysteine sulfoxide), S-methyl-L-cysteine sulfoxide and allinase. It is rich in vitamins like thiamine, riboflavine and niacin. Volatile oil contains allicin (diallyl thiosulphinate), an active odour principle of garlic. Other major compounds present are diallyl disulphide, diallyl trisulphide, allyl methyl trisulphide and allyl methyl disulphide (Husain et al., 1992).

Garlic bulb is antirheumatic, stimulant, diaphoretic, expectorant, diuretic, antispasmodic, astringent, antiparalytic, antileprotic, aperient, febrifuge, carminative, stomachic, alterative and emmenagogue. The essential oil is hypocholestrolemic, hypotensive, antitumour and antidiabetic. Diallyl disulphide and diallyl trisulphide from essential oil have larvicidal action. Bulbs also have anti-bacterial, and anti-fungal activity.... garlic

Importance: Kalmegh, the Great or Green Chiretta is a branched annual herb. It is useful in hyperdipsia, burning sensation, wounds, ulcers, chronic fever, malarial and intermittent fevers, inflammations, cough, bronchitis, skin diseases, leprosy, pruritis, intestinal worms, dyspepsia, flatulence, colic, diarrhoea, dysentery, haemorrhoids and vitiated conditions of pitta (Warrier et al, 1993). It is used to overcome sannipata type of fever, difficulty in breathing, hemopathy due to the morbidity of kapha and pitta, burning sensation, cough, oedema, thirst, skin diseases, fever, ulcer and worms. It is also useful in acidity and liver complaints (Aiyer and Kolammal, 1962). The important preparations using the drug are Tiktakagheta, Gorocandi gulika, Candanasava, Panchatiktam kasaya, etc. (Sivarajan et al, 1994). A preparation called “Alui” is prepared by mixing powdered cumin (Cuminium cyminum) and large cardamom (Amomum subulatum) in the juice of this plant and administered for the treatment of malaria (Thakur et al, 1989). It is also a rich source of minerals.

Distribution: The plant is distributed throughout the tropics. It is found in the plains of India from U.P to Assam, M.P., A.P, Tamil Nadu and Kerala, also cultivated in gardens.

Botany: Andrographis paniculata (Burm.f.) Wall ex.

Nees belongs to the family Acanthaceae. It is an erect branched annual herb, 0.3-0.9m in height with quadrangular branches. Leaves are simple, lanceolate, acute at both ends, glabrous, with 4-6 pairs of main nerves. Flowers are small, pale but blotched and spotted with brown and purple distant in lax spreading axillary and terminal racemes or panicles. Calyx-lobes are glandular pubescent with anthers bearded at the base. Fruits are linear capsules and acute at both ends. Seeds are numerous, yellowish brown and sub-quadrate (Warrier et al,1993).

Another species of Andrographis is A. echioides (Linn.) Nees. It is found in the warmer parts of India. The plant is a febrifuge and diuretic. It contains flavone-echiodinin and its glucoside-echioidin (Husain et al, 1992).

Agrotechnology: The best season of planting Andrographis is May-June. The field is to be ploughed well, mixed with compost or dried cowdung and seedbeds of length 3m, breadth 1/2m and 15cm height are to be taken at a distance of 3m. The plant is seed propagated. Seeds are to be soaked in water for 6 hours before sowing. Sowing is to be done at a spacing of 20cm. Seeds may germinate within 15-20 days. Two weedings, first at one month after planting and the second at 2 month after planting are to be carried out. Irrigation during summer months is beneficial. The plant is not attacked by any serious pests or diseases. Flowering commences from third month onwards. At this stage, plant are to be collected, tied into small bundles and sun-dried for 4-5 days. Whole plant is the economic part and the yield is about 1.25t dried plants/ha (Prasad et al, 1997).

Properties and activity: Leaves contain two bitter substances lactone “andrographolid” and “kalmeghin”. The ash contains sodium chloride and potassium salts. Plant is very rich in chlorophyte. Kalmeghin is the active principle that contains 0.6% alkaloid of the crude plant. The plant contains diterpenoids, andrographolide, 14-deoxy-11-oxo-andrographolide, 14-deoxy-11,12-dihydroandrographolide, 14-deoxy andrographolide and neoandrographolide (Allison et al, 1968). The roots give flavones-apigenin-7,4-dio-O-methyl ether, 5-hydroxy-7,8,2’,3’- tetramethoxyflavone, andrographin and panicolin and -sitosterol (Ali et al, 1972; Govindachari et al, 1969). Leaves contain homoandrographolide, andrographosterol and andrographone.

The plant is vulnerary, antipyretic, antiperiodic, anti-inflammatory, expectorant, depurative, sudorific, anthelmintic, digestive, stomachic, tonic, febrifuge and cholagogue. The plant is antifungal, antityphoid, hepatoprotective, antidiabetic and cholinergic. Shoot is antibacterial and leaf is hypotensive(Garcia et al, 1980). This is used for the inflammation of the respiratory tract. In China, researchers have isolated the andrographolide from which soluble derivative such as 14-deoxy-11, 12-dehydro-andrographolide which forms the subject of current pharmacological and clinical studies. Apigenin 7,4’-O-dimethyl ether isolated from A. paniculata exhibits dose dependent, antiulcer activity in shay rat, histamine induced ulcer in guinea pigs and aspirin induced ulcers in rats. A crude substance isolated from methanolic extract of leaves has shown hypotensive activity. Pre-treatment of rats with leaf (500mg/kg) or andrographolide (5mg/kg) orally prevented the carbon tetrachloride induced increase of blood serum levels of glutamate-oxaloacetate transaminase in liver and prevented hepatocellular membrane.... green chiretta

Haemophilus

Gram-negative (see GRAM’S STAIN), rod-like, aerobic, non-sporing and non-motile parasitic bacteria. Mostly found in the respiratory tract, they may be part of the normal ?ora, but may also be responsible for several diseases. The main pathogenic species of haemophilus is H. in?uenzae, which may cause severe exacerbations of chronic BRONCHITIS, as well as MENINGITIS, EPIGLOTTITIS, SINUSITIS, and otitis media (see EAR, DISEASES OF). Other species may cause conjunctivitis (see EYE, DISORDERS OF) or CHANCROID. Haemophilus species are sensitive to a wide range of antibiotics, though generally resistant to penicillin. Infants are routinely immunised with Haemophilus B vaccine to prevent meningitis, septicaemia and epiglottitis

Importance: Indian Senna or Tinnevelly senna is a shrub very highly esteemed in India for its medicinal value. The leaves are useful in constipation, abdominal disorders, leprosy, skin diseases, leucoderma, splenomegaly, hepatopathy, jaundice, helminthiasis, dyspepsia, cough, bronchitis, typhoid fever, anaemia, tumours and vitiated conditions of pitta and vata (Warrier et al,1994). It is used in Ayurvedic preparations; “Pancha Sakara Churna”, “Shat Sakara Churna” and “Madhu Yastyadi Churna” used for constipation. Its use is widespread in Unani system and some of the important products of this system containing senna are “Itrifal Mulayyin”, “Jawarish Ood Mulayyin”, “Hab Shabyar”, “Sufuf Mulliyin”, “Sharbat Ahmad Shahi”, etc. used as a mild laxative (Thakur et al, 1989).

Distribution: The plant is of Mediterranean origin. It is found in Somalia, Saudi Arabia, parts of Pakistan and Kutch area of Gujarat. It is largely cultivated in Tirunelveli, Ramanathapuram, Madurai and Salem districts of Tamil Nadu.

Botany: The genus Cassia, belonging to the family Caesalpiniaceae, comprises of a number of species, namely,

C. burmanni Wight. syn. C. obovata (Linn.) Collad.

C. senna is a shrub or undershrub, 60-75cm in height with pale subterete or obtusely angled erect or spreading branches. Leaves are paripinnate. Leaflets are 5-8 in number, ovate-lanceolate and glabrous. Flowers are yellowish, many and arranged in axillary racemes. Fruits are flat legumes, greenish brown to dark brown and nearly smooth (Chopra et al,1980, Warrier et al,1994).

In commerce, the leaves and pods obtained from C. senna are known as “ Tinnevelly Senna” and those from C. acutifolia Delile. as “Alexandrian Senna”. The leaves of C. acutifolia are narrower than C. senna, otherwise both resemble to a large extent (Thakur et al, 1989). All the true Sennas have the portions of their leaves unequally divided. In some kinds the lower part of one side is reduced to little more than a line in breadth, while the other is from a quarter to half an inch in breadth. The drug known under the name of East Indian Senna is nearly free from adulteration; and as its properties appear identical with those of the Alexandrian and the price being less, it probably will supersede it in general practice. Its size and shape readily identify it (Graves, 1996).

Agrotechnology: The plant requires a mild subtropical climate with warm winters which are free from frost for its growth. Semiarid areas with adequate irrigation facilities are ideal for cultivation. Areas having high rainfall, humidity and poor drainage are not suitable. Light or medium loamy soils with adequate drainage and pH varying from 7.0-8.2 are preferable. In South India both summer and winter crops are possible. The plant is propagated by seeds. The seed rate required is 15-20kg/ha. Seeds are sown in October-November (winter rainfed crop) or in February-March (irrigated crop). Higher seed rate is required for unirrigated crop. Seeds are sown in lines 30cm apart. Application of 5-10t of FYM/ha before planting or raising a green manure crop is beneficial. About 40kg N and 25-50kg P2O5/ha applied as basal dressing and 40kg N/ha applied in 2 split dozes as top dressing gave better yield. While the rainfed crop is grown without irrigation, the irrigated crop requires 5-8 light irrigations during the entire growing season. The crop requires 2-3 weedings and hoeings in order to keep it free from weeds. Alternaria alternata causes leaf spot and dieback but the disease is not serious. In North India, the plant is attacked by the larvae of butterfly Catopsilia pyranthe which can be controlled by planting the crop in March-April instead of June-July. Under irrigated conditions, the first crop is obtained after 90 days of planting. The leaves are stripped by hand when they are fully green, thick and bluish-green in colour. The second crop is taken 4 weeks after the first harvest and the third 4-6 weeks after the second one. The last harvest of leaves is done when the entire crop is harvested along with the pods. Yield under irrigated conditions is nearly1.4t of leaves and 150kg pods/ha and under unirrigated conditions is 500-600kg leaves and 80-100kg pods/ha. The leaves are dried in thin layers under shade so as to retain the green colour and the pods are hung for 10-12 days to get dried. The leaves and pods are cleaned, graded and marketed (Husain et al, 1993).

Properties and Activity: Leaves contain glucose, fructose, sucrose and pinnitol. Mucilage consists of galactose, arabinose, rhamnose and galacturonic acid. Leaves also contain sennoside-C(8,8’- diglucoside of rhein-aloe-emodin-dianthrone). Pods contain sennosides A and B, glycoside of anthraquinones rhein and chrysophanic acid. Seeds contain -sitosterol (Husain et al, 1992). Leaves and pods also contain 0.33% -sterol and flavonols-kaempferol, kaempferin, and iso-rhamnetin. Sennoside content of C. acutifolia is higher ranging from 2.5% to 4.5% as compared to C. angustifolia ranging from 1.5 % to 2.5%.

The purgative activity of Senna is attributed to its sennosides. The pods cause lesser griping than the leaves. Leaf and pod is laxative. The leaves are astringent, bitter, sweet, acrid, thermogenic, cathartic, depurative, liver tonic, anthelmintic, cholagogue, expectorant and febrifuge.... indian senna

Juniper

Juniperus species

Description: Junipers, sometimes called cedars, are trees or shrubs with very small, scalelike leaves densely crowded around the branches. Each leaf is less than 1.2 centimeters long. All species have a distinct aroma resembling the well-known cedar. The berrylike cones are usually blue and covered with a whitish wax.

Habitat and Distribution: Look for junipers in open, dry, sunny areas throughout North America and northern Europe. Some species are found in southeastern Europe, across Asia to Japan, and in the mountains of North Africa.

Edible Parts: The berries and twigs are edible. Eat the berries raw or roast the seeds to use as a coffee substitute. Use dried and crushed berries as a seasoning for meat. Gather young twigs to make a tea.

CAUTION

Many plants may be called cedars but are not related to junipers and may be harmful. Always look for the berrylike structures, needle leaves, and resinous, fragrant sap to be sure the plant you have is a juniper.... juniper

Leishmaniasis

A group of infections caused by parasites transmitted to humans by sand?ies.

Visceral leishmaniasis (kala-azar) A systemic infection caused by Leishmania donovani which occurs in tropical and subtropical Africa, Asia, the Mediterranean littoral (and some islands), and in tropical South America. Onset is frequently insidious; incubation period is 2–6 months. Enlargement of spleen and liver may be gross; fever, anaemia, and generalised lymphadenopathy are usually present. Diagnosis is usually made from a bone-marrow specimen, splenic-aspirate, or liver-biopsy specimen; amastigotes (Leishman-Donovan bodies) of L. donovani can be visualised. Several serological tests are of value in diagnosis.

Untreated, the infection is fatal within two years, in approximately 70 per cent of patients. Treatment traditionally involved sodium stibogluconate, but other chemotherapeutic agents (including allupurinol, ketoconazole, and immunotherapy) are now in use, the most recently used being liposomal amphotericin B. Although immunointact persons usually respond satisfactorily, they are likely to relapse if they have HIV infection (see AIDS/HIV).

Cutaneous leishmaniasis This form is caused by infection with L. tropica, L. major,

L. aethiopica, and other species. The disease is widely distributed in the Mediterranean region, Middle East, Asia, Africa, Central and South America, and the former Soviet Union. It is characterised by localised cutaneous ulcers

– usually situated on exposed areas of the body. Diagnosis is by demonstration of the causative organism in a skin biopsy-specimen; the leishmanin skin test is of value. Most patients respond to sodium stibogluconate (see above); local heat therapy is also used. Paromomycin cream has been successfully applied locally.

Mucocutaneous leishmaniasis This form is caused by L. braziliensis and rarely L. mexicana. It is present in Central and South America, particularly the Amazon basin, and characterised by highly destructive, ulcerative, granulomatous lesions of the skin and mucous membranes, especially involving the mucocutaneous junctions of the mouth, nasopharynx, genitalia, and rectum. Infection is usually via a super?cial skin lesion at the site of a sand?y bite. However, spread is by haematogenous routes (usually after several years) to a mucocutaneous location. Diagnosis and treatment are the same as for cutaneous leishmaniasis.... leishmaniasis

Habitat and Distribution: The yellow-flowered lotus is native to North America. The pink-flowered species, which is widespread in the Orient, is planted in many other areas of the world. Lotuses are found in quiet fresh water.

Edible Parts: All parts of the plant are edible raw or cooked. The underwater parts contain large quantities of starch. Dig the fleshy portions from the mud and bake or boil them. Boil the young leaves and eat them as a vegetable. The seeds have a pleasant flavor and are nutritious. Eat them raw, or parch and grind them into flour.... lotus

Importance: Purging croton or croton oil plant, a small evergreen tree with separate male and female flowers, is one among the seven poisons described in Ayurveda. The drug is well known for its drastic purgative property. The drug is found to be useful in ascites, anasarca, cold, cough, asthma, constipation, calculus, dropsy, fever and enlargement of the abdominal viscera. The seed paste is a good application for skin diseases, painful swellings and alopacia. The seed-oil is useful in chronic bronchitis, laryngeal affections, arthritis and lock jaw. Misraka-sneham is an important preparation using the drug (Nadkarni, 1954; Dey, 1980; Sharma, 1983).

Distribution: It is distributed throughout North India. It is cultivated in Assam, West Bengal and South India.

Botany: Croton tiglium Linn. belongs to the family Euphorbiaceae. It is a small evergreen tree, 4.5-6.0m in height with ash coloured smooth bark and young shoots sprinkled with stellate hairs. Leaves are oblong to ovate-lanceolate, obtuse or rounded at the 2-glanded box, acuminate, membraneous, yellowish green and minutely toothed. Flowers are small, unisexual, males on slender pedicels, females larger and on short thick pedicels. Fruits are ovoid or oblong trigonous capsules. Seeds are smooth, testa black and enclosing reddish brown oily endosperm (Warrier et al,1994). Other species belonging to the genus Croton are as follows:

Agrotechnology: The plant is propagated by seeds. Seeds are to be sown on seedbeds and about 2 months old seedlings are used for transplanting. Pits of size 50cm cube are to be taken at 3m spacing and filled with dried cowdung, sand and topsoil and formed into a mound. The seedlings are to be planted on these mounds. Irrigation during summer months is beneficial. Application of organic manure after every 6 months is desirable. Weeding is to be carried out one month after transplanting. The plant is not attacked by any serious pests or diseases. Fruits are formed at the end of first year. Fruits when ripen and start to crack are to be collected, dried in sun, then the outer shell is removed and again dried for one day before marketing (Prasad et al,1997).

Properties and activity: Oil contains phorbol myristate acetate (Husain et al, 1992). Seeds contain upto 20% protein and 30-50% lipids. Iso-guanine-D-ribose (crotoniside) and saccharose were isolated from the seeds. In fractionation of croton oil, liquid-liquid distribution procedures proved to be the separation tools of choice. The per hydrogenated parent hydrocarbon of phorbol is a perhydrocyclopropabenzulene called tigliane and phorbol is 1, 1a , 1b , 4, 4a, 7a , 7b, 8, 9, 9a-decahydro-4a , 7 , 9 , 9a - tetrahydroxy-3-(hydroxymethyl)-1, 1, 6, 8 tetramethyl-5-H-cyclopropa[3,4] benz [1.2-e]azulen-5- one. Phorbol, a tetracylic diterpene with a 5, 7, 6 and 3- membered ring has 6 oxygen functions. Phorbol accounts for 3.4% and 4- deoxy- 4 - phorbol for 0.29% of the weight of croton oil. Twenty- five phorbol-12, 13-diesters have been detected (Hecker et al, 1974). A toxin croton 1, mol. wt 72,000 has been isolated from the seeds (Lin et al, 1978).

Phorbol myristate acetate activates nitroblue tetrazolium reduction in human polymorphs. Seed and oil is purgative, rubefacient and anti-dote for snakebite. The seeds and oil are acrid, bitter, thermogenic, emollient, drastic purgative, digestive, carminative, anthelmintic, antiinflammatory, vermifuge, deterent, diaphoretic, expectorant, vesicant, irritant and rubefacient.... purging croton

Ringworm

Ringworm, or tinea, is the name given to in?ammatory rashes caused by DERMATOPHYTES of the genera microsporum, epidermophyton and trichophyton. These fungi can infect skin, hair and nails. The important clinical patterns are:

Tinea capitis Usually seen in children in Britain and caused by microsporum species of human or animal (frequently a kitten) origin. Typically, patches of ALOPECIA are seen with broken-o? hair stumps which ?uoresce bright green under an ultraviolet (Wood’s) lamp. In Asia a chronic, scarring alopecia may be caused by a speci?c trichophyton (favus).

Tinea corporis is usually due to trichophyton species and forms ringed (hence ‘ringworm’) patches of redness and scaling on the trunk or limbs.

Tinea pedis (athlete’s foot) is caused by epidermophyton or trichophyton species. Its minor form manifests as itching, scaling or blistering in the lateral toe clefts. More severe forms can be extensive on the sole. Trichophyton rubrum can cause a chronic, dry, scaling in?ammation of the foot, eventually extending into the nails and on to the soles and top of the foot which may persist for years if untreated.

Tinea cruris typically causes a ‘butter?y’ rash on the upper inner thighs in young adult males. It is usually caused by spread from the feet.

Tinea unguium (onychomycosis) Affecting the nails, especially of the toes, T. rubrum is the usual cause and may persist for decades.

Tinea barbae This rash of the face and beard is rare. It may be very in?ammatory and is usually contracted from cattle by farm workers.

Treatment Tinea of the toe clefts and groin will usually respond to an antifungal cream containing terbina?ne or an azole. Tinea capitis, barbae, extensive tinea corporis and all nail infections require oral treatment with terbina?ne or itraconazole (a triazole antifungal agent taken orally and used for candidiasis of the mouth, throat and vulgovaginal area as well as for ringworm) which have largely superseded the earlier treatment with the antiobiotic griseofulvin. Schistosomiasis

Also known as BILHARZIASIS. This infection results from one of the human Schistosoma species. It is common in Africa, South America, the Far East, Middle East, and, to a limited extent, the Caribbean. The life-cycle is dependent on fresh-water snails which act as the intermediate host for the ?uke; the cercarial stage of the ?uke enters via intact human skin and matures in the portal circulation. Clinically, ‘swimmers’ itch’ may occur at the site of cercarial skin penetration. Acute schistosomiasis (Katayama fever) can result in fever, an urticarial rash (see URTICARIA), and enlargement of LIVER and SPLEEN. The adult male is about 12 mm and the female 24 mm in length.

S. haematobium causes CYSTITIS and haematuria – passage of blood in the urine; bladder cancer and ureteric obstruction, giving rise to hydronephrosis and kidney failure, are long-term sequelae in a severe case. S. mansoni can cause colonic symptoms and in a severe case, POLYPOSIS of the COLON; diarrhoea, which may be bloody, can be a presenting feature. In a heavy infection, eggs surrounded by granulomas are deposited in the liver, giving rise to extensive damage (pipe-stem ?brosis) associated with PORTAL HYPERTENSION, oesophageal varices, etc. However, unlike in CIRRHOSIS, hepatocellular function is preserved until late in the disease. S. japonicum (which is con?ned to the Far East, especially Indonesia) behaves similarly to S. mansoni infection; liver involvement is often more severe.

Diagnosis can be made by microscopic examination of URINE or FAECES. The characteristic eggs are usually detectable. Alternatively, rectal or liver BIOPSY are of value. Serological tests, including an ELISA (see ENZYME-LINKED IMMUNOSORBENT ASSAY (ELISA)), have now largely replaced invasive procedures used in making a parasitological diagnosis.

Treatment CHEMOTHERAPY has been revolutionised by the introduction of praziquantel (administered orally); this compound has no serious side-effects, although its cost may limit its use in developing countries. Oxamniquine is cheaper and e?ective in S. mansoni infection, although evidence of resistance has been recorded in several countries. Metriphonate is also relatively cheap and is of value in S. haematobium infection. Prevention is by complete avoidance of exposure to contaminated water; all travellers to infected areas should know about this disease. It is increasing in frequency as new expanses of fresh water appear as a result of irrigation schemes and dam projects. Molluscicides can be employed for snail-control.... schistosomiasis

Streptococcus

Streptococcus is a variety of gram-positive bacterium (see GRAM’S STAIN; BACTERIA) which under the microscope has much the appearance of a string of beads. Most species are saprophytic (see SAPROPHYTE); a few are PATHOGENIC and these include haemolytic types which can destroy red blood cells in a culture of blood agar. This o?ers a method of classifying the varying streptococcal strains. Alphahaeomolytic streptococci are usually associated with bacterial ENDOCARDITIS. SCARLET FEVER is caused by a ?-haeomolytic streptococcus called S. pyogenes. S. pneumoniae, also called PNEUMOCOCCUS, causes respiratory-tract infections, including PNEUMONIA. S. pyogenes may on its own, or with other bacteria, cause severe NECROTISING FASCIITIS or CELLULITIS in which oedema and death of subcutaneous tissues occur. The infection can spread very rapidly and, unless urgently treated with ANTIBIOTICS and sometimes surgery, death may quickly result. This spread is related to the ability of S. pyogenes to produce toxic substances called exotoxins. Although drug-resistant forms are occurring, streptococcal infections usually respond to treatment with antibiotics.... streptococcus

Tobacco

The leaf of several species of nicotiana, especially of the American plant Nicotiana tabacum.

The smoking of tobacco is the most serious public-health hazard in Britain today. It causes 100,000 premature deaths a year in the United Kingdom alone. In addition to the deaths caused by cigarette smoking, it is also a major cause of disability and illness in the form of myocardial infarction (see HEART, DISEASES OF), PERIPHERAL VASCULAR DISEASE, and EMPHYSEMA. Tobacco-smoking is also a serious hazard to the FETUS if the mother smokes. Furthermore, passive smoking – inhalation of other people’s tobacco smoke – has been shown to be a health hazard to non-smokers.

Composition In addition to vegetable ?bre, tobacco leaves contain a large quantity of ash, the nature of this depending predominantly upon the minerals present in the ground where the tobacco plant has been grown. Of the organic constituents, the brown ?uid alkaloid known as NICOTINE is the most important. The nicotine content of di?erent tobacco varies, and the amount absorbed depends upon whether or not the smoker inhales. Nicotine is the substance that causes a person to become addicted to tobacco smoking (see DEPENDENCE).

Tobacco smoke also contains some 16 substances capable of inducing cancer in experimental animals. One of the most important of these is benzpyrene, a strongly carcinogenic hydrocarbon. As this is present in coal tar pitch, it is commonly referred to in this context as tar. Other constituents of tobacco smoke include pyridine, ammonia and carbon monoxide.

Nicotine addiction is a life-threatening but treatable disorder, and nicotine-replacement treatment is available on NHS prescription. This includes the provision of bupropion – trade name Zyban®. The availability of this drug – which should be used with caution as it has unwelcome side-effects in some people – and the introduction of specialist smoking-cessation services to provide behavioural support to people who wish to stop smoking should result in a reduction in tobacco-related diseases. Given the critical position of nicotine in leading people to become addicted to smoking, it is anomalous that there are no e?ective government regulations covering the sales of tobacco. Because it is not a food, tobacco is not regulated by the Food Standards Agency; it is not classi?ed as a drug so is not controlled by legislation on medicines. Furthermore, despite being a consumer product, tobacco is exempt from the Consumer Protection Act (1987) and other government safety regulations. So the NHS is left to try to ameliorate the serious health consequences – lung cancer, cardiovascular disease, peripheral vascular disease, chronic bronchitis, and emphysema – of a substance for which there are no e?ective preventive measures except the willpower of the individual smoker or non-smoker. (Escalating taxation of tobacco seems to have been circumvented as a deterrent by the rising incidence of smuggling cigarettes into Britain.)

Action on Smoking and Health (ASH) is a small charity founded by the Royal College of Physicians in 1971 that attempts to alert and inform the public to the dangers of smoking and to try to prevent the disability and death which it causes.... tobacco

Yellow Fever

An acute arbovirus (see ARBOVIRUSES) infection caused by a ?avivirus of the togavirus family, transmitted from animals to humans by various species of forest mosquito (jungle/sylvan yellow fever), and from human to human by Aëdes aegypti (urban yellow fever). Mosquito transmission was shown by Walter Reed and his colleagues in 1900. It is ENDEMIC in much of tropical Africa and Central and South America but does not occur in Asia. In the urban cycle, humans constitute the reservoir of infection, and in the jungle/sylvan variety, mammals – especially subhuman primates – are involved in transmission. Historically, yellow fever was enormously important, causing devastating epidemics (see EPIDEMIC); it also carried a high mortality rate in travellers and explorers. Differentiation from other infections associated with JAUNDICE was often impossible.

Clinically, yellow fever is characterised by jaundice, fever, chills, headache, gastrointestinal haemorrhage(s), and ALBUMINURIA. The incubation period is 3–6 (up to 10) days. Differentiation from viral hepatitides, other viral haemorrhagic fevers, severe Plasmodium falciparum malaria, and several other infections is often impossible without sophisticated investigative techniques. Infection carries a high mortality rate. Liver histology (biopsy is contraindicated due to the haemorrhagic diathesis) shows characteristic changes; a fulminating hepatic infection is often present. Acute in?ammation of the kidneys and an in?amed, congested gastric mucosa, often accompanied by haemorrhage, are also demonstrable; myocardial involvement often occurs. Diagnosis is primarily based on virological techniques; serological tests are also of value. Yellow fever should be suspected in any travellers from an endemic area.

Management consists of instituting techniques for acute hepatocellular (liver-cell) failure. The affected individual should be kept in an isolation unit, away from mosquitoes which could transmit the disease to a healthy individual. Formerly, laboratory infections were occasionally acquired from infected blood samples. Prophylactically, a satisfactory attenuated VACCINE (17D) has been available for around 60 years; this is given subcutaneously and provides an individual with excellent protection for ten years; international certi?cates are valid for this length of time. Every traveller to an endemic area should be immunised; this is mandatory for entry to countries where the infection is endemic.... yellow fever

Importance: The bracteated birthwort or worm killer is a perennial prostrate herb. As the name suggests it is a killer of intestinal worms especially roundworms. It is also used in vitiated conditions of kapha and vata, constipation, inflammations, amenorrhoea, dysmenorrhoea, foul ulcers, boils, syphilis, gonorrhoea, dyspepsia, colic, skin diseases, eczema, artheralgia and intermittent fevers. The plant is an insect repellent due to the presence of aristolochic acid, which is poisonous to man and livestock. Plant is also used against scorpion sting. Seeds ground in water to form a lotion and used for softening hair. Powdered root is used in fertility control.

Distribution: The plant is found in Sri Lanka, Arabian countries and tropical Africa. In India, the plant is grown in Deccan and Carnatic Plateau.

Botany: Aristolochia bracteolata Lam. syn. A. bracteata Retz. belongs to the family Aristolochiaceae. It is a perennial prostrate herb with weak, glabrous stems. Leaves are simple, alternate, reniform or broadly ovate, cordate at the base with a wide sinus upto 7.5cm in diameter, reticulately veined. Flowers are solitary with a large sessile orbicular bract at the base. Perianth tube is cylindric with dark purple tip having revolute margins. Fruits are oblong-ellipsoid 12-ribbed glabrous capsules. Seeds are deltoid with slightly cordate base (Warrier et al, 1993) Another important species belonging to the genus Aristolochia is A. indica Linn. The plant grows wild throughout the low hills and plains of India from Nepal to West Bengal and South India. It is a valuable anti-dote to snake bite and to bites of poisonous insects as scorpion, etc. It is given in cases of cholera and diarrhoea after macerating with black pepper corns. The juice of the leaves has stimulant, tonic and antiperiodic properties.

Agrotechnology: Shady areas and well-drained soils are most suited to Aristolochia. The plant can be seed propagated. 3-month-old seedlings raised in polybags are required for transplanting. Pits of size 50cm cube are to be taken at a distance of 3m and filled with sand, topsoil and dried cowdung. To these pits, the seedlings are to be transplanted. Regular irrigation and organic manure application is beneficial. The plant is to be trailed on iron wires tied to poles. The plant is not attacked by any serious pests or diseases. Plant attains good spread within one year. Leaves can be collected for the next 10 years. Roots and leaves constitute the economi c parts (Prasad et al, 1997).

Properties and activity: Leaves and fruits yield ceryl alcohol, -sitosterol and aristolochic acid. Root contains aristolochic acid. Seeds give an alkaloid magnoflorine, aristolochic acid, fatty oil comprising palmitic, stearic, lignoseric and oleic acids and -sitosterol.

The plant is anthelmintic, cathartic, antiperiodic and emmenagogue. Leaf is antigonorrhoeic, larvicidal and used in eczema on children’s leg and ulcers. The plant is oxytocic (Husain et al,1992).... worm killer

Galangal

Colic root. Chinese Ginger. Alpinia officinarum, Hance. Dried rhizome.

Keynote: colic.

Constituents: galangol, oil.

Action: carminative, stomachic, stimulant, diaphoretic, anti-neoplastic. “Antifungal against various candida species.” (Planta Medica 1988, 54(2), pp 117-20)

Uses: flatulent indigestion, chronic nausea and vomiting, seasickness. Ulceration of gums and skin. Benign tumour.

Preparations: Average dose: 1-2g. Thrice daily.

Decoction. Half a teaspoon to each cup water gently simmered 20 minutes. Dose: quarter to half cup morning and evening.

Liquid Extract BHP (1983) 1:1 in 25 per cent alcohol. Dose, 1-2ml. Tincture BHP (1983) 1:5 in 45 per cent alcohol. Dose, 2-4ml. Powder (internal use) 1-2 grams. Also as snuff. ... galangal

Acacia

Acacia farnesiana

Description: Acacia is a spreading, usually short tree with spines and alternate compound leaves. Its individual leaflets are small. Its flowers are ball-shaped, bright yellow, and very fragrant. Its bark is a whitish-gray color. Its fruits are dark brown and podlike.

Habitat and Distribution: Acacia grows in open, sunny areas. It is found throughout all tropical regions.

Note: There are about 500 species of acacia. These plants are especially prevalent in Africa, southern Asia, and Australia, but many species are found in the warmer and drier parts of America.

Edible Parts: Its young leaves, flowers, and pods are edible raw or cooked.... acacia

Aconitum Atrox

(Bruchl) Mukherjee.

Synonym: Aconitum balfourii Stapf.

Family: Ranunculaceae.

Habitat: The sub-alpine and alpine Himalayas between 3,300 and 3,900 m.

Ayurvedic: Vatsanaabha (related sp.).

Folk: Banwaa.

Action: Poisonous, highly toxic.

Air-dried roots contain 1.2% total alkaloids of which pseudoaconitine is 0.4%. Pseudoaconitine is biologically 1.5 times as active as aconitine. (A. atrox is a poisonous species and is one of the common constituents of Aconi- tumferox of commerce.)... aconitum atrox

Aerva Lanata

(L.) Juss. ex Schult.

Family: Amaranthaceae.

Habitat: The warmer parts of India, ascending to 1,000 m.

Ayurvedic: Paashaanabheda. Gorakshaganjaa, Aadaanpaaki, Shatkabhedi.

Siddha/Tamil: Sirupeelai.

Folk: Paashaanabheda (southern India), Gorakhagaanjaa.

Action: Anticalculus (used in lithiasis), diuretic, demulcent, anthelmintic, antidiarrhoeal, anticholerin, bechic; leaf used in hepatitis, root in strangury. A decoction of the plant is used in catarrh of bladder. The flowers and roots are used for headache.

Key application: As diuretic and lithontriptic. (The British Herbal Pharmacopoeia.)

The plant contains palmitic acid, beta-sitosterol and alpha-amyrin.

Aerva scandens Wall., synonym A. sanguinolenta Blume, is also known as Paashaanabheda in the south.

Species used as Paashaanabheda: Bergenia ligulata (north), Aerva lanata (south), Coleus amboinicus (east) and Bryophyllum pinnatum (west).

Agaricus Tea: A Mushroom Tea

Agaricus tea is the beverage resulting from brewing the dried Agaricus mushrooms.   • once it starts to boil, reduce the flame and let it simmer at low flame for about 20 to 30 minutes until the mixture has been reduced about 1/4 or when you have about 750ml of liquid left

Aids/hiv

Acquired Immune De?ciency Syndrome (AIDS) is the clinical manifestation of infection with Human Immunode?ciency Virus (HIV). HIV belongs to the retroviruses, which in turn belong to the lentiviruses (characterised by slow onset of disease). There are two main HIV strains: HIV-1, by far the commonest; and HIV-2, which is prevalent in Western Africa (including Ivory Coast, Gambia, Mali, Nigeria and Sierra Leone). HIV attacks the human immune system (see IMMUNITY) so that the infected person becomes susceptible to opportunistic infections, such as TUBERCULOSIS, PNEUMONIA, DIARRHOEA, MENINGITIS and tumours such as KAPOSI’S SARCOMA. AIDS is thus the disease syndrome associated with advanced HIV infection.

Both HIV-1 and HIV-2 are predominantly sexually transmitted and both are associated with secondary opportunistic infections. However, HIV-2 seems to result in slower damage to the immune system. HIV-1 is known to mutate rapidly and has given rise to other subtypes.

HIV is thought to have occurred in humans in the 1950s, but whether or not it infected humans from another primate species is uncertain. It became widespread in the 1970s but its latency in causing symptoms meant that the epidemic was not noticed until the following decade. Although it is a sexually transmitted disease, it can also be transmitted by intravenous drug use (through sharing an infected needle), blood transfusions with infected blood (hence the importance of e?ective national blood-screening programmes), organ donation, and occupationally (see health-care workers, below). Babies born of HIV-positive mothers can be infected before or during birth, or through breast feeding.

Although HIV is most likely to occur in blood, semen or vaginal ?uid, it has been found in saliva and tears (but not sweat); however, there is no evidence that the virus can be transmitted from these two body ?uids. There is also no evidence that HIV can be transmitted by biting insects (such as mosquitoes). HIV does not survive well in the environment and is rapidly destroyed through drying.

Prevalence At the end of 2003 an estimated 42 million people globally were infected with HIV – up from 40 million two years earlier. About one-third of those with HIV/AIDS are aged 15–24 and most are unaware that they are carrying the virus. During 2003 it is estimated that 5 million adults and children worldwide were newly infected with HIV, and that 3 million adults and children died. In Africa in 2003,

3.4 million people were newly infected and 2.3 million died, with more than 28 million carrying the virus. HIV/AIDS was the leading cause of death in sub-Saharan Africa where over half of the infections were in women and 90 per cent of cases resulted from heterosexual sex. In some southern African countries, one in three pregnant women had HIV.

In Asia and the Paci?c there were 1.2 million new infections and 435,000 deaths. The area with the fastest-growing epidemic is Eastern Europe, especially the Russian Federation where in 2002 around a million people had HIV and there were an estimated 250,000 new infections, with intravenous drug use a key contributor to this ?gure. Seventy-?ve per cent of cases occurred in men, with male-to-male sexual transmission an important cause of infection, though heterosexual activity is a rising cause of infection.

At the end of 2002 the UK had an estimated 55,900 HIV-infected adults aged between 15 and 59. More than 3,600 individuals were newly diagnosed with the infection in 2000, the highest annual ?gure since the epidemic started

– in 1998 the ?gure was 2,817 and in 1999 just over 3,000 (Department of Health and Communicable Disease Surveillance Centre). The incidence of AIDS in the UK has declined sharply since the introduction of highly active antiretroviral therapy (HAART) and HIV-related deaths have also fallen: in 2002 there were 777 reported new AIDS cases and 395 deaths, compared with 1,769 and 1,719 respectively in 1995. Poverty is strongly linked to the spread of AIDS, for various reasons including lack of health education; lack of e?ective public-health awareness; women having little control over sexual behaviour and contraception; and, by comparison with the developed world, little or no access to antiretroviral drugs.

Pathogenesis The cellular target of HIV infection is a subset of white blood cells called T-lymphocytes (see LYMPHOCYTE) which carry the CD4 surface receptor. These so-called ‘helper T-cells’ are vital to the function of cell-mediated immunity. Infection of these cells leads to their destruction (HIV replicates at an enormous rate – 109) and over the course of several years the body is unable to generate suf?cient new cells to keep pace. This leads to progressive destruction of the body’s immune capabilities, evidenced clinically by the development of opportunistic infection and unusual tumours.

Monitoring of clinical progression It is possible to measure the number of viral particles present in the plasma. This gives an accurate guide to the likely progression rate, which will be slow in those individuals with fewer than 10,000 particles per ml of plasma but progressively more rapid above this ?gure. The main clinical monitoring of the immune system is through the numbers of CD4 lymphocytes in the blood. The normal count is around 850 cells per ml and, without treatment, eventual progression to AIDS is likely in those individuals whose CD4 count falls below 500 per ml. Opportunistic infections occur most frequently when the count falls below 200 per ml: most such infections are treatable, and death is only likely when the CD4 count falls below 50 cells per ml when infection is developed with organisms that are di?cult to treat because of their low intrinsic virulence.

Simple, cheap and highly accurate tests are available to detect HIV antibodies in the serum. These normally occur within three months of infection and remain the cornerstone of the diagnosis.

Clinical features Most infected individuals have a viral illness some three weeks after contact with HIV. The clinical features are often non-speci?c and remain undiagnosed but include a ?ne red rash, large lymph nodes, an in?uenza-like illness, cerebral involvement and sometimes the development of opportunistic infections. The antibody test may be negative at this stage but there are usually high levels of virus particles in the blood. The antibody test is virtually always positive within three months of infection. HIV infection is often subsequently asymptomatic for a period of ten years or more, although in most patients progressive immune destruction is occurring during this time and a variety of minor opportunistic infections such as HERPES ZOSTER or oral thrush (see CANDIDA) do occur. In addition, generalised LYMPHADENOPATHY is present in a third of patients and some suffer from severe malaise, weight loss, night sweats, mild fever, ANAEMIA or easy bruising due to THROMBOCYTOPENIA.

The presentation of opportunistic infection is highly variable but usually involves either the CENTRAL NERVOUS SYSTEM, the gastrointestinal tract or the LUNGS. Patients may present with a sudden onset of a neurological de?cit or EPILEPSY due to a sudden onset of a STROKE-like syndrome, or epilepsy due to a space-occupying lesion in the brain – most commonly TOXOPLASMOSIS. In late disease, HIV infection of the central nervous system itself may produce progressive memory loss, impaired concentration and mental slowness called AIDS DEMENTIA. A wide variety of opportunistic PROTOZOA or viruses produces DYSPHAGIA, DIARRHOEA and wasting. In the respiratory system the commonest opportunistic infection associated with AIDS, pneumonia, produces severe shortness of breath and sometimes CYANOSIS, usually with a striking lack of clinical signs in the chest.

In very late HIV infection, when the CD4 count has fallen below 50 cells per ml, infection with CYTOMEGALOVIRUS may produce progressive retinal necrosis (see EYE, DISORDERS OF) which will lead to blindness if untreated, as well as a variety of gastrointestinal symptoms. At this stage, infection with atypical mycobacteria is also common, producing severe anaemia, wasting and fevers. The commonest tumour associated with HIV is Kaposi’s sarcoma which produces purplish skin lesions. This and nonHodgkin’s lymphoma (see LYMPHOMA), which is a hundred times more frequent among HIV-positive individuals than in the general population, are likely to be associated with or caused by opportunistic viral infections.

Prevention There is, as yet, no vaccine to prevent HIV infection. Vaccine development has been hampered

by the large number of new HIV strains generated through frequent mutation and recombination.

grammes underway to develop vaccines that are either prophylactic or therapeutic. Vaccine-development strategies have included: recombinant-vector vaccines, in which a live bacterium or virus is genetically modi?ed to carry one or more of the HIV genes; subunit vaccines, consisting of small regions of the HIV genome designed to induce an immune response without infection; modi?ed live HIV, which has had its disease-promoting genes removed; and DNA vaccines – small loops of DNA (plasmids) containing viral genes – that make the host cells produce non-infectious viral proteins which, in turn, trigger an immune response and prime the immune system against future infection with real virus.

In the absence of an e?ective vaccine, preventing exposure remains the chief strategy in reducing the spread of HIV. Used properly, condoms are an extremely e?ective method of preventing exposure to HIV during sexual intercourse and remain the most important public-health approach to countering the further acceleration of the AIDS epidemic. The spermicide nonoxynol-9, which is often included with condoms, is known to kill HIV in vitro; however, its e?ectiveness in preventing HIV infection during intercourse is not known.

Public-health strategies must be focused on avoiding high-risk behaviour and, particularly in developing countries, empowering women to have more control over their lives, both economically and socially. In many of the poorer regions of the world, women are economically dependent on men and refusing sex, or insisting on condom use, even when they know their partners are HIV positive, is not a straightforward option. Poverty also forces many women into the sex industry where they are at greater risk of infection.

Cultural problems in gaining acceptance for universal condom-use by men in some developing countries suggests that other preventive strategies should also be considered. Microbicides used as vaginal sprays or ‘chemical condoms’ have the potential to give women more direct control over their exposure risk, and research is underway to develop suitable products.

Epidemiological studies suggest that male circumcision may o?er some protection against HIV infection, although more research is needed before this can be an established public-health strategy. Globally, about 70 per cent of infected men have acquired the virus through unprotected vaginal sex; in these men, infection is likely to have occurred through the penis with the mucosal epithelia of the inner surface of the foreskin and the frenulum considered the most likely sites for infection. It is suggested that in circumcised men, the glans may become keratinised and thus less likely to facilitate infection. Circumcision may also reduce the risk of lesions caused by other sexually transmitted disease.

Treatment AIDS/HIV treatment can be categorised as speci?c therapies for the individual opportunistic infections – which ultimately cause death – and highly active antiretroviral therapy (HAART) designed to reduce viral load and replication. HAART is also the most e?ective way of preventing opportunistic infections, and has had a signi?cant impact in delaying the onset of AIDS in HIV-positive individuals in developed countries.

Four classes of drugs are currently in use. Nucleoside analogues, including ZIDOVUDINE and DIDANOSINE, interfere with the activity of the unique enzyme of the retrovirus reverse transcriptase which is essential for replication. Nucleotide analogues, such as tenofovir, act in the same way but require no intracellular activation. Non-nucleoside reverse transcriptase inhibitors, such as nevirapine and EFAVIRENZ, act by a di?erent mechanism on the same enzyme. The most potent single agents against HIV are the protease inhibitors, such as lopinavir, which render a unique viral enzyme ineffective. These drugs are used in a variety of combinations in an attempt to reduce the plasma HIV viral load to below detectable limits, which is achieved in approximately 90 per cent of patients who have not previously received therapy. This usually also produces a profound rise in CD4 count. It is likely, however, that such treatments need to be lifelong – and since they are associated with toxicities, long-term adherence is di?cult. Thus the optimum time for treatment intervention remains controversial, with some clinicians believing that this should be governed by the viral load rising above 10,000 copies, and others that it should primarily be designed to prevent the development of opportunistic infections – thus, that initiation of therapy should be guided more by the CD4 count.

It should be noted that the drug regimens have been devised for infection with HIV-1; it is not known how e?ective they are at treating infection with HIV-2.

HIV and pregnancy An HIV-positive woman can transmit the virus to her fetus, with the risk of infection being particularly high during parturition; however, the risk of perinatal HIV transmission can be reduced by antiviral drug therapy. In the UK, HIV testing is available to all women as part of antenatal care. The bene?ts of antenatal HIV testing in countries where antiviral drugs are not available are questionable. An HIV-positive woman might be advised not to breast feed because of the risks of transmitting HIV via breastmilk, but there may be a greater risk associated with not breast feeding at all. Babies in many poor communities are thought to be at high risk of infectious diseases and malnutrition if they are not breast fed and may thus be at greater overall risk of death during infancy.

Counselling Con?dential counselling is an essential part of AIDS management, both in terms of supporting the psychological wellbeing of the individual and in dealing with issues such as family relations, sexual partners and implications for employment (e.g. for health-care workers). Counsellors must be particularly sensitive to culture and lifestyle issues. Counselling is essential both before an HIV test is taken and when the results are revealed.

Health-care workers Health-care workers may be at risk of occupational exposure to HIV, either through undertaking invasive procedures or through accidental exposure to infected blood from a contaminated needle (needlestick injury). Needlestick injuries are frequent in health care – as many as 600,000 to 800,000 are thought to occur annually in the United States. Transmission is much more likely where the worker has been exposed to HIV through a needlestick injury or deep cut with a contaminated instrument than through exposure of mucous membranes to contaminated blood or body ?uids. However, even where exposure occurs through a needlestick injury, the risk of seroconversion is much lower than with a similar exposure to hepatitis C or hepatitis B. A percutaneous exposure to HIV-infected blood in a health-care setting is thought to carry a risk of about one infection per 300 injuries (one in 1,000 for mucous-membrane exposure), compared with one in 30 for hepatitis C, and one in three for hepatitis B (when the source patient is e-antigen positive).

In the event of an injury, health-care workers are advised to report the incident immediately where, depending on a risk assessment, they may be o?ered post-exposure prophylaxis (PEP). They should also wash the contaminated area with soap and water (but without scrubbing) and, if appropriate, encourage bleeding at the site of injury. PEP, using a combination of antiretroviral drugs (in a similar regimen to HAART – see above), is thought to greatly reduce the chances of seroconversion; it should be commenced as soon as possible, preferably within one or two hours of the injury. Although PEP is available, safe systems of work are considered to o?er the greatest protection. Double-gloving (latex gloves remove much of the blood from the surface of the needle during a needlestick), correct use of sharps containers (for used needles and instruments), avoiding the resheathing of used needles, reduction in the number of blood samples taken from a patient, safer-needle devices (such as needles that self-blunt after use) and needleless drug administration are all thought to reduce the risk of exposure to HIV and other blood-borne viruses. Although there have been numerous cases of health-care workers developing HIV through occupational exposure, there is little evidence of health-care workers passing HIV to their patients through normal medical procedures.... aids/hiv

Description: These plants have large clusters of thick, fleshy leaves borne close to the ground and surrounding a central stalk. The plants flower only once, then die. They produce a massive flower stalk.

Habitat and Distribution: Agaves prefer dry, open areas. They are found throughout Central America, the Caribbean, and parts of the western deserts of the United States and Mexico.

Edible Parts: Its flowers and flower buds are edible. Boil them before eating.

CAUTION

The juice of some species causes dermatitis in some individuals.

Other Uses: Cut the huge flower stalk and collect the juice for drinking. Some species have very fibrous leaves. Pound the leaves and remove the fibers for weaving and making ropes. Most species have thick, sharp needles at the tips of the leaves. Use them for sewing or making hacks. The sap of some species contains a chemical that makes the sap suitable for use as a soap.... agave

Albizia Tea Fights Insomnia

Albizia tea is largely-spread worldwide and it provides plenty of health benefits to consumers. It is mainly recommended to patients suffering from ailments afflicting the nerve and brain. Albizia Tea description Albizia is a genus of more than 150 species of trees, occurring in large areas of the world, but mainly in the Old World tropics. It is regarded as an invasive species, growing in dry plains and sandy valleys. The parts considered to have healthy properties are the flowers and the bark. The heads of the Albizia flower are said to have sedative and tonic properties, whereas the bark has proven a stimulant and diuretic action. In ancient traditional Chinese literature, the use of the Albizia herb was related to promoting joy, assuaging sorrow and brightening the eyes.   • alternatively, place a teaspoon of dried Albizia herb powder in newly-boiled water and similarly steep for about 5 minutes

Allium Cepa

Habitat: Cultivated as an annual all over the country. The most important onion-growing states are Maharashtra, Tamil Nadu, Andhra Pradesh., Karnataka and Madhya Pradesh.

English: Onion.

Ayurvedic: Palaandu, Durgandh.

Unani: Piyaaz.

Siddha/Tamil: Venkaayam.

Action: Antibiotic, antibacterial, antisclerotic, anticoagulant, anti-inflammatory, antiasthmatic, expectorant, carminative, anti- spasmodic, diuretic, hypotensive, antidiabetic.

Key application: For the prevention of atherosclerosis (German Commission E) and age-dependent changes in the blood vessels, and loss of appetite (WHO).

The official onion bulb of the Pharmacopoeia of the People's Republic of China is a different species, Allium macrostemon Bge., than that of the German Commission E monographs, A. cepa. Chinese onion is used for cough, dyspnoea, angina pectoris and dysentery.

Scallions or Spring Onion of Chinese medicine are equated with Allium fistulosum.

Onion bulbs contain a volatile oil with sulphurous constituents, including allylpropyldisulphide; sulphur- containing compounds, including al- licin, alliin; flavonoids; phenolic acids and sterols.

Hypoglycaemic activity of the onion is attributed to the allylpropyldisul- phide and allicin. Diphenylamine, isolated from mature bulbs, also exhibits potent antihyperglycaemic activity.

Alliin and allicin have an inhibitory effect on platelet aggregation. Antibiotic activity is due mainly to alli-cin.

Regular use of onion (50 g/day) reduces insulin requirement of a diabetic patient from 40 to 20 units a day.

Thiosulphinates, isolated from onion juice, exhibited antiasthmatic activity in vivo.

Antivenom

A therapeutic substance used to counteract the toxic action(s) of a speci?c animal toxin (see TOXINS) or venom. They are normally sterilised, proteinaceous globulins (see GLOBULIN) extracted from the SERUM of animals, usually horses, immunised against the speci?c toxin/ venom. Most are given by intravenous or intramuscular injection and are most e?ective when given shortly after the bite or sting has occurred. Some antivenoms may be e?ective against the venoms of several closely related animal species.... antivenom

Arrowroot

Maranta and Sagittaria species

Description: The arrowroot is an aquatic plant with arrow-shaped leaves and potatolike tubers in the mud.

Habitat and Distribution: Arrowroot is found worldwide in temperate zones and the tropics. It is found in moist to wet habitats.

Edible Parts: The rootstock is a rich source of high quality starch. Boil the rootstock and eat it as a vegetable.... arrowroot

Alquitira

Prickly pear cactus (Opuntia ficus-indica).

Plant Part Used: Cactus pad (leaf-stem).

Dominican Medicinal Uses: The fresh cactus pad is traditionally prepared as a juice taken orally for diabetes, high blood pressure, heart disease, stomach ailments and indigestion. The gel from inside the leaf-stem is typically applied topically for wound-healing.

Safety: The cactus pad and fruit are widely consumed and generally considered safe. Caution is advised during handling due to sharp spines and glochids which cover the surface; these spiny projections should be removed before use. Cases of contact dermatitis have been reported.

Contraindications: Contraindicated in individuals with a history of allergy or hypersensitivity to Opuntia and other cactus species. Due to lack of available data, avoid use during pregnancy or breastfeeding and in small children.

Drug Interactions: Besides potential synergistic effects with drugs that share similar biological activities to this plant (see “Laboratory and Preclinical Data”), particularly diabetes and blood-sugar modulating medications, insufficient information has been identified on herb-drug interactions in the available literature.

Clinical Data: The following effects of this plant have been investigated in human clinical trials: anti-hangover, anti-inflammatory, antioxidant and anti-lipid peroxidation.

Laboratory & Preclinical Data: The following activities of this plant have been shown in laboratory and preclinical studies: anti-inflammatory, antioxidant, chondroprotective, gastro-protective, hypoglycemic, immunomodulatory, neuroprotective, radical scavenging and wound-healing.

Atrial Natriuretic Peptide

The atria (see ATRIUM) of the heart contain peptides with potent diuretic and vasodilating properties. It has been known since 1980 that extracts of human atria have potent diuretic and natriuretic effects in animals (see DIURETICS). In 1984 three polypeptide species were isolated from human atria and were called alpha, beta and gamma human atrial natriuretic peptides. Plasma concentration of immunoreactive atrial natriuretic peptide can now be measured: the levels are low in healthy subjects and are increased in patients with congestive heart failure. Infusion of the peptides reduces blood pressure and causes a natriuresis and diuresis.... atrial natriuretic peptide

Alstonia Scholaris

R. Br.

Family: Apocynaceae.

Habitat: Throughout moist regions of India, especially in West Bengal and west-coast forests of southern India.

English: Devil's tree, Dita Bark tree.

Ayurvedic: Saptaparna, Sapta- chhada, Saptaparni, Saptaahvaa, Vishaaltvak, Shaarada, Visham- chhada.

Unani: Chhaatim, Kaasim (Kaasim Roomi, Anjudaan Roomi is equated with Myrrhis odorata Scope.)

Siddha/Tamil: Ezhilamippalai, Mukkampalai.

Folk: Chhitavan, Sataunaa.

Action: Bark—febrifuge, antiperi- odic, spasmolytic, antidysenteric, uterine stimulant, hypotensive; used for internal fevers.

Along with other therapeutic applications, The Ayurvedic Pharmacopoeia of India indicates the use of stembark in phosphaturia and recommends it as a blood purifier.

Alstonia sp. is known as Fever Bark. A. constricta is native to Australia; A. scholaris to Australia and Southeast Asia. The bark of both the species contains indole alkaloids. A. constric- ta contains reserpine (a hyptotensive agent). A. scholaris contains echita- mine, which has also demonstrated hypotensive effects. Though A. schol- aris produces fall in the temperature of human patients with fever, there are conflicting reports about the activity of echitamine against Plasmodium berghei.

Dosage: Stembark—20-30 g for decoction. (API Vol. I.)... alstonia scholaris

Altamisa

Ragweed (Ambrosia artemisiifolia and A. peruviana).

Plant Part Used: Leaf, aerial parts.

Dominican Medicinal Uses: The leaf is traditionally prepared as a tea and taken orally for arthritis, delayed menses, diarrhea (in children and adults), infections, kidney ailments, menstrual pain, postpartum cleansing and stomach ache. It is also used externally as a poultice for menstrual pain and as a bath for energetic cleansing, good luck and spiritual protection.

Safety: The pollen of Ambrosia species is a common allergen and may cause symptoms of hayfever in hypersensitive individuals. Plant material should be washed thoroughly before use to remove pollen. Cases of contact dermatitis, eczema, allergic conjunctivitis and other adverse effects have been associated with this plant.

Contraindications: Due to lack of available safety information, avoid use during pregnancy or lactation and in children under 5 years of age.

Drug Interactions: Synergistic interactions may occur with medications that share similar biological activities to those demonstrated by this herb (see “Clinical Data” and “Laboratory & Preclinical Data” below).

Clinical Data: The following effects of Ambrosia artemisiifolia have been investigated in human clinical trials: allergenic, immunotherapeutic and irritant.

Amaranth

Amaranthus species

Description: These plants, which grow 90 centimeters to 150 centimeters tall, are abundant weeds in many parts of the world. All amaranth have alternate simple leaves. They may have some red color present on the stems. They bear minute, greenish flowers in dense clusters at the top of the plants. Their seeds may be brown or black in weedy species and light-colored in domestic species.

Habitat and Distribution: Look for amaranth along roadsides, in disturbed waste areas, or as weeds in crops throughout the world. Some amaranth species have been grown as a grain crop and a garden vegetable in various parts of the world, especially in South America.

Edible Parts: All parts are edible, but some may have sharp spines you should remove before eating. The young plants or the growing tips of alder plants are an excellent vegetable. Simply boil the young plants or eat them raw. Their seeds are very nutritious. Shake the tops of alder plants to get the seeds. Eat the seeds raw, boiled, ground into flour, or popped like popcorn.... amaranth

Anchusa Italica

Retz.

Synonym: A. azurea Mill.

Family: Boraginaceae.

Habitat: Indian gardens, and hills.

English: Cow's Tongue Plant.

Ayurvedic: Gojihvikaa (considered as a vegetable, equated with Launaea asplenifolia Hook. f., Compositae, Asteraceae.) Unani Gaozabaan is a different drug.

Unani: Gaozabaan. (Now equated with species of Boraginaceae, particularly Borago officinalis Linn. Imported Unani drug Gaozabaan consists of the leaves and nutlets of Anchusa strigosa Labill and Echium amoenum Fisch. and Mey.)

Action: Stimulant, tonic, demulcent; used in bilious complaints, fever, cough, asthma; as diuretic in bladder and kidney stones. Oil—a rich source of vitamin E (0.72%), more than that of wheat-germ oil (0.18%). The nutlets show positive tests for alkaloids and tannins. The flowers yield anthocyanins and the leafy stems yield bornesitol.... anchusa italica

Balanophora Involucrata

Hook. f.

Family: Balanophoraceae.

Habitat: The Himalayas from Kashmir to Sikkim and Darjeeling at altitudes of 1,800-3,400 m

Ayurvedic: Chavya (tentative synonym).

Action: Astringent. Used in piles, also in rheumatism.

A related species, B.polyandra Griff., found in Nagaland, Manipur, West Bengal, Bihar, Orissa and Andhra Pradesh at 2,000 m, gave a phenolic gly- coside, coniferin. The plant is used as an antiasthmatic.... balanophora involucrata

Bartonella

Ancylostomiasis

A parasitic infection caused by the nematodes Ancylostoma duodenale and Necator americanus, resulting in hookworm disease. These infections are exceedingly common in tropical and developing countries, millions of people being affected. Classically, A. duodenale occurred in the Far East, Mediterranean littoral, and Middle East, and N. americanus in tropical Africa, Central and South America, and the Far East; however, in recent years, geographical separation of the two human species is less distinct. In areas where standards of hygiene and sanitation are unsatisfactory, larvae (embryos) enter via intact skin, usually the feet. ‘Ground itch’ occasionally occurs as larvae enter the body. They then undergo a complex life-cycle, migrating through the lungs, trachea, and pharynx. Adult worms are 5–13 (mean 12) mm in length; their normal habitat is the small INTESTINE – especially the jejunum – where they adhere to the mucosa by hooks, thus causing seepage of blood into the lumen. A worm-pair produces large numbers of eggs, which are excreted in faeces; when deposited on moist soil they remain viable for many weeks or months. Clinical manifestations include microcytic hypochromic ANAEMIA, hypoalbuminaemia (low serum protein) and, in a severe case, OEDEMA. A chronic infection in childhood can give rise to physical, mental and sexual retardation. Treatment is with one of the benzimidazole compounds, usually mebendazole or albendazole; however, in developing countries, cheaper preparations are used, including tetrachloroethylene, bephenium hydroxynaphthoate, and pyrantel embonate. Anaemia usually responds to iron supplements; blood transfusion is rarely indicated.

Ancylostoma braziliensis A nematode infection of dogs, which in humans causes local disease (larva migrans) only, generally on the soles of the feet. It is usually acquired by walking on beaches contaminated with dog faeces in places such as the Caribbean.... ancylostomiasis

Anís

There are at least five different species of anise-like medicinal plants that are recognized in Dominican healing traditions. Those that are included in the present edition of this book are listed below in bold along with their other common Spanish names:

- Anís chiquito = anís de comer, anís de cocinar, anís pequeño, aniscito (Pimpinella anisum)

- Anís comino = comino (Cuminum cyminum) or hinojo

- Hinojo = anís hinojo (Foeniculum vulgare) or anís comino

- Anís de estrella = anís estrellada, anís grande (Illicium verum)

- Aniseto = aniceto (Piper marginatum)

The common names of the first three types of anís are easily confused because their dried fruits or “seeds” (the part of the plant traditionally used for medicine) are similar in appearance, taste and shape. Anís De Estrella

Chinese star anise (Illicium verum).

Plant Part Used: Fruit, seed.

Dominican Medicinal Uses: The fruits or seeds are traditionally prepared as a decoction and taken orally for flatulence, headache, indigestion, stomach ache, upper respiratory tract infection and cleansing the intestines.

Safety: The fruit is generally considered safe for human consumption in small amounts and is widely used as a culinary spice. When taken in excessive quantities, isolated compounds from the fruit have shown neurotoxic effects in animal studies. Caution is advised due to possible adulteration with the highly poisonous look-alike, Japanese star anise (Illicium anisatum).

Contraindications: Avoid use in small children due to potential contamination with misidentified toxic look-alike. Caution and avoidance is advised in patients with a history of convulsive disorders including epilepsy due to case reports of seizures associated with internal use of the tea. Caution advised in patients prior to surgery due to potential risk of increased bleeding.

Drug Interactions: Anticoagulants, antiplatelet medications and NSAIDS: based on animal studies in mice, star anise increases cytochrome P450 dependent 7-ethoxycoumarin O-deethylase activity which may affect the metabolism of these drugs.

Clinical Data: No human clinical trials evaluating this plant species have been identified in the available literature.

About the Nutrients in This Food Beer and ale are fermented beverages created by yeasts that convert the sugars in malted barley and grain to ethyl alcohol (a.k.a. “alcohol,” “drink- ing alcohol”).* The USDA /Health and Human Services Dietary Guidelines for Americans defines one drink as 12 ounces of beer, five ounces of wine, or 1.25 ounces of distilled spirits. One 12-ounce glass of beer has 140 calo- ries, 86 of them (61 percent) from alcohol. But the beverage—sometimes nicknamed “liquid bread”—is more than empty calories. Like wine, beer retains small amounts of some nutrients present in the food from which it was made. * Because yeasts cannot digest t he starches in grains, t he grains to be used in mak ing beer and ale are allowed to germinate ( “malt” ). Storing This Food Store beer in a cool place. Beer tastes best when consumed within two months of the day it is made. Since you cannot be certain how long it took to ship the beer to the store or how long it has been sitting on the grocery shelves, buy only as much beer as you plan to use within a week or two. Protect bottled beer and open bottles or cans of beer from direct sunlight, which can change sulfur compounds in beer into isopentyl mercaptan, the smelly chemical that gives stale beer its characteristic unpleasant odor.

When You Are Ready to Serve This Food Serve beer only in absolutely clean glasses or mugs. Even the slightest bit of grease on the side of the glass will kill the foam immediately. Wash beer glasses with detergent, not soap, and let them drain dry rather than drying them with a towel that might carry grease from your hands to the glass. If you like a long-lasting head on your beer, serve the brew in tall, tapering glasses to let the foam spread out and stabilize. For full flavor, serve beer and ales cool but not ice-cold. Very low temperatures immo- bilize the molecules that give beer and ale their flavor and aroma.

What Happens When You Cook This Food When beer is heated (in a stew or as a basting liquid), the alcohol evaporates but the flavor- ing agents remain intact. Alcohol, an acid, reacts with metal ions from an aluminum or iron pot to form dark compounds that discolor the pot or the dish you are cooking in. To prevent this, prepare dishes made with beer in glass or enameled pots.

Medical Uses and/or Benefits Reduced risk of heart attack. Data from the American Cancer Society’s Cancer Prevention Study 1, a 12-year survey of more than 1 million Americans in 25 states, shows that men who take one drink a day have a 21 percent lower risk of heart attack and a 22 percent lower risk of stroke than men who do not drink at all. Women who have up to one drink a day also reduce their risk of heart attack. Numerous later studies have confirmed these findings. Lower risk of stroke. In January 1999, the results of a 677-person study published by researchers at New York Presbyterian Hospital-Columbia University showed that moder- ate alcohol consumption reduces the risk of stroke due to a blood clot in the brain among older people (average age: 70). How the alcohol prevents stroke is still unknown, but it is clear that moderate use of alcohol is a key. Heavy drinkers (those who consume more than seven drinks a day) have a higher risk of stroke. People who once drank heavily, but cut their consumption to moderate levels, can also reduce their risk of stroke. Numerous later studies have confirmed these findings. Lower cholesterol levels. Beverage alcohol decreases the body’s production and storage of low-density lipoproteins (LDLs), the protein and fat particles that carr y cholesterol into your arteries. As a result, people who drink moderately tend to have lower cholesterol levels and higher levels of high density lipoproteins (HDLs), the fat and protein particles that carr y cholesterol out of the body. The USDA /Health and Human Services Dietar y Guidelines for Americans defines moderation as two drinks a day for a man, one drink a day for a woman. Stimulating the appetite. Alcoholic beverages stimulate the production of saliva and the gastric acids that cause the stomach contractions we call hunger pangs. Moderate amounts of alcoholic beverages, which may help stimulate appetite, are often prescribed for geriatric patients, convalescents, and people who do not have ulcers or other chronic gastric problems that might be exacerbated by the alcohol. Dilation of blood vessels. Alcohol dilates the capillaries (the tiny blood vessels just under the skin), and moderate amounts of alcoholic beverages produce a pleasant flush that temporar- ily warms the drinker. But drinking is not an effective way to warm up in cold weather since the warm blood that flows up to the capillaries will cool down on the surface of your skin and make you even colder when it circulates back into the center of your body. Then an alco- hol flush will make you perspire, so that you lose more heat. Excessive amounts of beverage alcohol may depress the mechanism that regulates body temperature.

Adverse Effects Associated with This Food Increased risk of breast cancer. In 2008, scientists at the National Cancer Institute released data from a seven-year survey of more than 100,000 postmenopausal women showing that even moderate drinking (one to two drinks a day) may increase by 32 percent a woman’s risk of developing estrogen-receptor positive (ER+) and progesterone-receptor positive (PR+) breast cancer, tumors whose growth is stimulated by hormones. No such link was found between consuming alcohol and the risk of developing ER-/PR- tumors (not fueled by hor- mones). The finding applies to all types of alcohol: beer, wine, and spirits. Increased risk of oral cancer (cancer of the mouth and throat). Numerous studies confirm the American Cancer Society’s warning that men and women who consume more than two drinks a day are at higher risk of oral cancer than are nondrinkers or people who drink less. Note: The Dietary Guidelines for Americans describes one drink as 12 ounces of beer, five ounces of wine, or 1.5 ounces of distilled spirits. Increased risk of cancer of the colon and rectum. In the mid-1990s, studies at the University of Oklahoma suggested that men who drink more than five beers a day are at increased risk of rectal cancer. Later studies suggested that men and women who are heavy beer or spirits drinkers (but not those who are heavy wine drinkers) have a higher risk of colorectal cancers. Further studies are required to confirm these findings. Fetal alcohol syndrome. Fetal alcohol syndrome is a specific pattern of birth defects—low birth weight, heart defects, facial malformations, and mental retardation—first recognized in a study of babies born to alcoholic women who consumed more than six drinks a day while pregnant. Subsequent research has found a consistent pattern of milder defects in babies born to women who consume three to four drinks a day or five drinks on any one occasion while pregnant. To date, there is no evidence of a consistent pattern of birth defects in babies born to women who consume less than one drink a day while pregnant, but two studies at Columbia University have suggested that as few as two drinks a week while preg- nant may raise a woman’s risk of miscarriage. (“One drink” means 12 ounces of beer, five ounces of wine, or 1.25 ounces of distilled spirits.) Alcoholism. Alcoholism is an addiction disease, the inability to control one’s alcohol consumption. It is a potentially life-threatening condition, with a higher risk of death by accident, suicide, malnutrition, or acute alcohol poisoning, a toxic reaction that kills by para- lyzing body organs, including the heart. Malnutrition. While moderate alcohol consumption stimulates appetite, alcohol abuse depresses it. In addition, an alcoholic may drink instead of eating. When an alcoholic does eat, excess alcohol in his/her body prevents absorption of nutrients and reduces the ability to synthesize new tissue. Hangover. Alcohol is absorbed from the stomach and small intestine and carried by the bloodstream to the liver, where it is oxidized to acetaldehyde by alcohol dehydrogenase (ADH), the enzyme our bodies use to metabolize the alcohol we produce when we digest carbohydrates. The acetaldehyde is converted to acetyl coenzyme A and either eliminated from the body or used in the synthesis of cholesterol, fatty acids, and body tissues. Although individuals vary widely in their capacity to metabolize alcohol, on average, normal healthy adults can metabolize the alcohol in one quart of beer in approximately five to six hours. If they drink more than that, they will have more alcohol than the body’s natural supply of ADH can handle. The unmetabolized alcohol will pile up in the bloodstream, interfering with the liver’s metabolic functions. Since alcohol decreases the reabsorption of water from the kidneys and may inhibit the secretion of an antidiuretic hormone, they will begin to urinate copiously, losing magnesium, calcium, and zinc but retaining more irritating uric acid. The level of lactic acid in the body will increase, making them feel tired and out of sorts; their acid-base balance will be out of kilter; the blood vessels in their heads will swell and throb; and their stomachs, with linings irritated by the alcohol, will ache. The ultimate result is a “hangover” whose symptoms will disappear only when enough time has passed to allow their bodies to marshal the ADH needed to metabolize the extra alcohol in their blood. Changes in body temperature. Alcohol dilates capillaries, tiny blood vessels just under the skin, producing a “flush” that temporarily warms the drinker. But drinking is not an effective way to stay warm in cold weather. Warm blood flowing up from the body core to the surface capillaries is quickly chilled, making you even colder when it circulates back into your organs. In addition, an alcohol flush triggers perspiration, further cooling your skin. Finally, very large amounts of alcohol may actually depress the mechanism that regulates body temperature. Impotence. Excessive drinking decreases libido (sexual desire) and interferes with the ability to achieve or sustain an erection. “Beer belly.” Data from a 1995, 12,000 person study at the University of North Carolina in Chapel Hill show that people who consume at least six beers a week have more rounded abdomens than people who do not drink beer. The question left to be answered is which came first: the tummy or the drinking.

Food/Drug Interactions Acetaminophen (Tylenol, etc.). The FDA recommends that people who regularly have three or more drinks a day consult a doctor before using acetaminophen. The alcohol/acetamino- phen combination may cause liver failure. Disulfiram (Antabuse). Taken with alcohol, disulfiram causes flushing, nausea, low blood pressure, faintness, respiratory problems, and confusion. The severity of the reaction gener- ally depends on how much alcohol you drink, how much disulfiram is in your body, and how long ago you took it. Disulfiram is used to help recovering alcoholics avoid alcohol. (If taken with alcohol, metronidazole [Flagyl], procarbazine [Matulane], quinacrine [Atabrine], chlorpropamide (Diabinase), and some species of mushrooms may produce a mild disulfi- ramlike reaction.) Anticoagulants. Alcohol slows the body’s metabolism of anticoagulants (blood thinners) such as warfarin (Coumadin), intensif ying the effect of the drugs and increasing the risk of side effects such as spontaneous nosebleeds. Antidepressants. Alcohol may increase the sedative effects of antidepressants. Drinking alcohol while you are taking a monoamine oxidase (M AO) inhibitor is especially hazard- ous. M AO inhibitors inactivate naturally occurring enzymes in your body that metabolize tyramine, a substance found in many fermented or aged foods. Tyramine constricts blood vessels and increases blood pressure. If you eat a food containing tyramine while you are taking an M AO inhibitor, you cannot effectively eliminate the tyramine from your body. The result may be a hypertensive crisis. Ordinarily, fermentation of beer and ale does not produce tyramine, but some patients have reported tyramine reactions after drinking some imported beers. Beer and ale are usually prohibited to those using M AO inhibitors. Aspirin, ibuprofen, ketoprofen, naproxen, and nonsteroidal anti-inflammatory drugs. Like alcohol, these analgesics irritate the lining of the stomach and may cause gastric bleeding. Combining the two intensifies the effect. Insulin and oral hypoglycemics. Alcohol lowers blood sugar and interferes with the metabo- lism of oral antidiabetics; the combination may cause severe hypoglycemia. Sedatives and other central nervous system depressants (tranquilizers, sleeping pills, antidepres- sants, sinus and cold remedies, analgesics, and medication for motion sickness). Alcohol inten- sifies sedation and, depending on the dose, may cause drowsiness, respiratory depression, coma, or death.... beer

Description: These plants have prickly stems (canes) that grow upward, arching back toward the ground. They have alternate, usually compound leaves. Their fruits may be red, black, yellow, or orange.

Habitat and Distribution: These plants grow in open, sunny areas at the margin of woods, lakes, streams, and roads throughout temperate regions. There is also an arctic raspberry.

Edible Parts: The fruits and peeled young shoots are edible. Flavor varies greatly.

Ayurvedic: Aguru, Krimij, Krish- naaguru, Jongaka, Maaliyaka, Loha, Kaalaloha, Asitaka.

Unani: Ood-ul Hindi, Ood Gharqi.

Siddha/Tamil: Akil kattai, Agil.

Action: Heartwood—astringent, carminative, antiasthmatic, anti- diarrhoeal, antidysenteric; used in gout, rheumatism and paralysis; as a stimulant in sexual debility; as a liniment in skin diseases.

The agarwood or eaglewood of commerce is derived from the fungus- infected tree through wounds caused by the species of Aspergillus, Fusarium, Penicillium, and also by some of Fungi Imperfecti. Agarwood on distillation yields an essential oil, known as Agar Oil.

The essential oil yields a number of agarofurans, sesquiterpene alcohols and spirosesquiterpene alcohols.

The stemwood yields sesquiterpe- noids—gmelofuran and agarol; also a coumarinolignan—aquillochin.

(Agar is a different drug—extract of a seaweed, Gelidium Amansii, used as a mild laxative.)

Argyreia Speciosa

Sweet.

Habitat: Found all over India, ascending to 300 m.

Ayurvedic: Vriddhadaaruka, Vriddhadaaru, Vriddhadaaraka, Bastaantri, Sthavira, Sthaviradaaru, Atarunadaaru, Samudrashosha. (Seeds of Salvia plebeia R. Br. are also known as Samudrashosha.)

Unani: Samunder sokh.

Siddha/Tamil: Ambgar, Samuddira- pacchai

Folk: Bidhaaraa.

Action: Root—aphrodisiac (considered as a rejuvenator), nervine (used in diseases of nervous system, sexual disorders), diuretic (used in strangury), antirheumatic. Seeds—hypotensive, spasmolytic. Leaves—used externally in skin diseases (ringworm, eczema, boils, swellings); rubefacient, topically stimulant.

The seeds contain hallucinogenic ergoline alkaloids, the main ones being ergine and isoergine. EtOH (50%) extract of seeds exhibits hypotensive activity. (Seeds of all species of Argyreia contain ergoline alkaloids and are hypotensive.) Leaves of Argyreia sp. contain sitosterol and are antiphlogistic.

In Indian medicine, A. speciosa is not used as a single drug for sexual disorders in men, but as a supporting drug for exerting its antiphlogistic, spasmolytic and hypotensive actions on the central nervous system. The drug, in itself, did not show anabolic- cum-androgen-like or spermogenetic activity experimentally.

Ipomoea petaloidea Chois and Ipo- moea biloba Forskofthe Convolvulacae family are also used as Vriddhadaaru.

In Western herbal medicine, Hawaiian Baby Woodrose is equated with Argyreia nervosa (synonym Argyreia speciosa; grows in Florida, California and Hawaii). The seed is used for pain relief and as a hallucinogen.

The seeds contain hallucinogens including ergonovine, isoergine (isoly- sergic acid amide) and ergine (lysergic acid amide). Four to eight seeds are equivalent to 10-100 mcg of LSD, a potent serotonin-1A (5-HT1A) agonist. The effects last 6-8 h. (Natural Medicines Comprehensive Database, 2007.)

Bubonic Plague

A severe illness caused by the Gram negative rod, Yersinia pestis. The reservoirs for the infection are various species of rodent and the bacteria are transmitted through the bite of the rat flea, Xenopsylla cheopis. Patients present with enlarged lymph glands (‘buboes’) often in the groin or armpit. Can become septicaemic or develop into a pneumo nia (‘Pneumonic Plague’) and spread by droplet. Musa species

Description: These are treelike plants with several large leaves at the top. Their flowers are borne in dense hanging clusters.

Habitat and Distribution: Look for bananas and plantains in open fields or margins of forests where they are grown as a crop. They grow in the humid tropics.

Edible Parts: Their fruits are edible raw or cooked. They may be boiled or baked. You can boil their flowers and eat them like a vegetable. You can cook and eat the rootstocks and leaf sheaths of many species. The center or “heart” or the plant is edible year-round, cooked or raw.

Other Uses: You can use the layers of the lower third of the plants to cover coals to roast food. You can also use their stumps to get water (see Chapter 6). You can use their leaves to wrap other foods for cooking or storage.... banana and plantain

Importance: In traditional medicine, Bauhinia is extensively used in glandular diseases and as an antidote to poison. The drug is also reported to be useful in dysentery, diarrhoea, piles and worms (Kurup et al, 1979; Sharma et al, 1983). They are useful in vitiated conditions of kapha and pitta, diarrhoea, dysentery, skin diseases, leprosy, intestinal worms, tumours, wounds, ulcers, inflammations, scrofula, protoptosis, haemorrhoids, haemoptysis, cough, menorrhagia and diabetics. Usirasavam and Candanasavam are some of the preparations using the drug. An important Ayurvedic preparation, “Kanchnar Guggal” contains bark of this plant. In Unani system, the flowers are used in “Hab Mussafi Khun”, for skin diseases, the bark is used in “Sufuf Kalan”-an aphrodisiac.

Distribution: The plant is distributed in the Sub-Himalayan tracts from the Indus eastwards and throughout the dry forests of India, ascending to 1300m. It is also cultivated throughout the plains.

Botany: Bauhinia variegata Linn. syn. B.

candida Roxb. belonging to the family Caesalpiniaceae is a moderate sized deciduous tree with vertically cracked grey bark, wood moderately hard, greyish brown with irregular darker patches. Leaves are of 2 leaflets, connate for about two-thirds up. Leaflets are ovate with rounded apex, 10-15cm long, pubescent beneath when young and coriaceous. Flowers are white or pink, the uppermost petal darker and variegated usually appearing before the leaves in short axillary or terminal racemes. Stamens are 5 and stamenodes absent. Fruits are flat dehiscent pods with 10-15 seeds (Warrier et al, 1993).

Other important species of the genus Bauhinia are as follows.

1. B. tomentosa Linn.

It is the yellow or golden flowered one, commonly known as Manja Mandaram. It is found in Africa and Asia. In India it is found wild in dry deciduous forests and often cultivated. The plant is antidysenteric, antidote for snakebite and scorpion sting and also used in liver complaints. The bark is astringent. Root bark is vermifuge. Fruit is diuretic. Seed is tonic, wound healing and aphrodisiac.

2. B. purpurea Linn.

Pink Bauhinia or Camel’s Foot tree is found in South and S. E. Asia. In India, it is found in deciduous forests. Root is carminative and tonic. Bark is astringent and antidiarrhoeal and is used in ulcer and goitre. Flowers are laxative. The experimental studies conducted by Sijoria and Prasad (1979) on animals indicate that B. purpurea is very effective in normalising the thyroid gland.

3. B. racemosa Lam.

The plant is found in Sub-Himalayan tracts, in U.P, West Bengal, Central and South India. The leaf is anticephalalgic and antimalarial. Bark is astringent, antidiarrhoeal. The seeds are antibacterial. Stem-bark is CVS and CNS active, hypothermic and anticancerous.

4. B. malabarica Roxb.

Malabar Mountain Ebony is found in Sub-Himalayan tracts, from Kumaon to West Bengal, ascending to 1350m, Assam, Bihar and South India. The flowers of this plant are antidysenteric.

5. B. retusa Roxb.

The plant is distributed in north-western Himalayas from the Beas eastwards, Himachal Pradesh, U.P., Orissa, M.P. and A.P. The gum of the plant is emmenagogue, diuretic and can be used externally in sores. The seed is hypoglycaemic and hypocholesterolaemic. The aerial part is CVS active and has effect on respiration.

6. B. vahlii W.&A.

Camel’s Foot climber is found in Punjab, Bihar, Assam, Madhy Pradesh, Andra Pradesh and Tamil Nadu. Leaf is demulcent. Seed is tonic and aphrodisiac. Stem is CVS active, antiarrhythmic and spasmolytic.

Agrotechnology: Well drained hilly areas are ideal for the cultivation of Bauhinia. The plant is seed propagated. Seeds are formed in February-March. Seeds are to be collected from the dried pods, soaked in water for 12 hours before sowing in seedbeds. At four-leaved stage they are to be transferred to polybags. Two month old seedlings from polybags are used for field planting. Pits of size 60cm cube are to be taken and filled with 10kg dried cowdung mixed with topsoil and formed into a mound. On these seedlings are to be planted at a distance of 6-7.5m. Irrigation is to be given in the first year. Two weedings and application of organic manure once is required in a year. The plant is not attacked by any serious pests and diseases. The plant flowers on the third year. At the end of tenth year the tree can be cut and wood used for medicinal purposes (Prasad et al, 1997).

Properties and activity: Flowers contain flavanoids-kaempferol-3-galactoside and kaempferol-3- rhamnoglucoside. Stem bark yields hentriacontane, octacosanol and stigmasterol. Stem yields -sitisterol, lupiol and a flavanone glycoside-5, 7-dimethoxy flavanone 4-O- -L- rhamnopyranoside- -D-glucopyranoside. Seeds possess human blood agglutinating activity. Stem bark is hypothermic, CNS active and depressant. Bud, flower, leaf and stembark are antibacterial. Stem possesses juvenoid activity. Bark is alterative, tonic, antileprotic and antirheumatic. Bud is antidysenteric. Root is carminative and antidote for snakebite. Bark, flower and root promote suppuration. Bark and bud are astringent and vermifuge (Husain et al, 1992).... bauhinia

Description: Beech trees are large (9 to 24 meters), symmetrical forest trees that have smooth, light-gray bark and dark green foliage. The character of its bark, plus its clusters of prickly seedpods, clearly distinguish the beech tree in the field.

Habitat and Distribution: This tree is found in the Temperate Zone. It grows wild in the eastern United States, Europe, Asia, and North Africa. It is found in moist areas, mainly in the forests. This tree is common throughout southeastern Europe and across temperate Asia. Beech relatives are also found in Chile, New Guinea, and New Zealand.

Edible Parts: The mature beechnuts readily fall out of the husklike seedpods. You can eat these dark brown triangular nuts by breaking the thin shell with your fingernail and removing the white, sweet kernel inside. Beechnuts are one of the most delicious of all wild nuts. They are a most useful survival food because of the kernel’s high oil content. You can also use the beechnuts as a coffee substitute. Berberine hydrochloride and sulphate help in the diagnosis of latent malaria by releasing the parasites into the blood stream.

Alkaloid berberine possesses antibacterial and anti-inflammatory activities. It is used as an intestinal antiseptic and bitter stomachic. It also exhibits antineoplastic properties. (Its synthetic derivative dihydroberberine is used in brain tumour.)

Berberine has been found to inhibit the activity of enzymes trypsin (32%) and chymotrypsin (60%) in in-vitro studies.

Brazilwood (Caesalpinia brasiliensis and related species).

Plant Part Used: Wood.

Dominican Medicinal Uses: The wood is traditionally prepared as a cold infusion and taken orally for diabetes, high blood pressure, kidney infections, women’s health conditions, menstrual disorders, poor circulation, uterine fibroids and cysts.

Safety: No studies on the safety of this plant in humans have been identified in the available literature. However, a related species has shown relatively low toxicity in animal studies.

Laboratory & Preclinical Data: The following activities have been reported in Caesalpinia species related to Brasil and may not reflect the bioactivity of Caesalpinia brasiliensis. In animal studies the seed kernel extract has shown antidiabetic and hypoglycemic activity and the leaf extract has shown muscle stimulant activity. In vitro, plant extracts have shown anticancer, antibacterial, antioxidant, antitumor and inhibition of nitric oxide formation, serine proteinase and xanthine oxidase effects.

Brucea Sativa

National Formulary of UnaniMedicine, Part I, equated Jirjeer with Brucea sativa Mill. or Nasturtium officinale R. Br.

Nasturtium officinale, found in Europe, is known as watercress. Indian cress is cultivated in gardens as a creeper. Brucea is a totally different species (Simaroubaceae). Taraamirra of Unani medicine should be equated with Eru- ca sativa and not with Brucea sativa.

Action: Taraamiraa (Jirjeer)— used in Unani medicine as a spermatic tonic (powder of seeds is administered with a half-fried egg), also as a blood purifier, diuretic, emmenagogue and deobstruent. Leaf juice—used as a lotion for blotches, spots and blemishes.

Nasturtium officinale (Brassicaceae): Antiscorbutic and stimulant. A rich source of vitamins A and E, also of ascorbic acid. Seeds contain glucon- casturtin. Used for metabolic disorders, anaemia, strangury, kidney and bladder disorders and catarrh of the respiratory tract.

Eruca sativa Mill.: Cultivated in North India; known as Taraamiraa, Siddhaartha, Bhutaghna. Seeds are used like mustard. Seeds—antibacterial. Crude juice of the plant inhibited E. coli, S. typhi and B. subtlis. Seeds contain (4-Me-thio)-Bu-glucosinolate (glucoerucin) as K and tetra-Me-N salts. A composition is used in induration of liver.... brucea sativa

Caccinia Crassifolia

O. Kuntze.

Synonym: C. glauca Savi.

Family: Boraginaceae.

Habitat: Native to Baluchistan. Available in Indian market as Gaozabaan.

Unani: Gaozabaan. (According to National Formulary of Unani Medicine, Borago officinalis Linn. and other species of Boraginaceae are used as Gaozabaan.)

Action: Leaf—diuretic, antiinflammatory, demulcent; used for strangury, asthma and cough.

The stems and leaves gave sapoge- nins—caccigenin, caccigenin lactone and 23-deoxycaccigenin; rutin and a saponin derived from caccigenin. The leaves also gave a glucoside, cac- cinin. Caccinin and its aglucone cac- cinetin (which is the dimethylallyl ester of caffeic acid) is diuretic; saponins exhibit anti-inflammatory activity.

Flowers contain pyrrolizidine alkaloid, the diester of retronecine. Ben- zoic acid has also been isolated from the flowers.... caccinia crassifolia

Caesalpinia Bonduc

(L.) Roxb. Dandy & Exell.

Synonym: C. bonducella Flem. C. crista Linn.

Family: Caesalpiniaceae.

Habitat: Throughout the hotter parts of India. Common in West Bengal and South India. Often grown as hedge plant.

English: Fever Nut, Bonduc Nut, Nikkar Nut.

Ayurvedic: Puutikaranja, Lataa- karanja, Kantaki Karanja, Karanjin, Kuberaakshi (seed).

Unani: Karanjwaa.

Siddha/Tamil: Kazharchikkaai.

Action: Seed—antiperiodic, antirheumatic. Roasted and used as an antidiabetic preparation. Leaf, bark and seed—febrifuge. Leaf and bark—emmenagogue, anthelmintic. Root—diuretic, anticalculous.

The seeds contain an alkaloid cae- salpinine; bitter principles such as bon- ducin; saponins; fixed oil.

The seed powder, dissolved in water, showed hypoglycaemic activity in alloxanized hyperglycaemic rabbits. Aqueous extract of the seeds produced similar effects in rats. The powder forms a household remedy for treatment of diabetes in Nicobar Islands. In Kangra, Himachal Pradesh, roots are used in intermittent fevers and diabetes.

In homoeopathy, the plant is considered an excellent remedy for chronic fever.

(Three plant species—Pongamia pinnata Pierre, Holoptelea integrifo- lia (Roxb.) Planch. and Caesalpinia bonduc (L.) Roxb. are being used as varieties of Karanja (because flowers impart colour to water). P. pinnata is a tree and is equated with Karanja, Naktamaala and Udakirya; H. integri- folia, also a tree, with Chirabilva, Puti- ka (bad smell) and Prakiryaa; and C. bonduc, a shrub, with Kantaki Karanja or Lataa Karanja.)

The family Cucurbitaceae includes a large group of plants which are medicinally valuable. The important genera belonging to the family are Trichosanthes, Lagenaria, Luffa, Benincasa, Momordica, Cucumis, Citrullus, Cucurbita, Bryonopsis and Corallocarpus. The medicinally valuable species of these genera are discussed below.

1. Trichosanthes dioica Roxb.

Wild snake-gourd is a slender-stemmed, extensively climbing, more or less scabrous and woolly herb found throughout the plains of N. India, extending to Assam and W. Bengal. Tendrils are 2-4 fid. Leaves are 7.5x5cm in size, ovate-oblong, cordate, acute, sinuate- dentate, not lobed, rigid, rough on both surface and with a petiole of 2cm. Flowers are unisexual. Male flowers are not racemed but woolly outside. Calyx tube is 4.5cm long, narrow, teeth linear and erect. Anthers are free. Fruit is 5.9cm long, oblong or nearly spherical, acute, smooth and orange-red when ripe. Seeds are half-ellipsoid, compressed and corrugated on the margin (Kirtikar and Basu, 1988). The unripe fruit of this is generally used as a culinary vegetable and is considered very wholesome and specially suited for the convalescent. The tender shoots are given in decoction with sugar to assist digestion. The seeds are useful for disorders of the stomach. The leaf juice is rubbed over the chest in liver congestion and over the whole body in intermittent fevers (Nadkarni, 1998). The fruit is used as a remedy for spermatorrhoea. The fresh juice of the unripe fruit is often used as a cooling and laxative adjunct to some alterative medicines. In bilious fever, a decoction of patola leaves and coriander in equal parts is given. The fruit in combination with other drugs is prescribed in snakebite and scorpion sting (Kirtikar and Basu, 1988).

Fruits contain free amino acids and 5-hydroxy tryptamine. Fatty acids from seeds comprise elaeostearic, linoelic, oleic and saturated acids. The aerial part is hypoglycaemic. Leaf and root is febrifuge. Root is hydragogue, cathartic and tonic. Unripe leaf and fruit is laxative (Husain et al, 1992). The plant is alterative and tonic. Leaves are anthelmintic. Flower is tonic and aphrodisiac. The ripe fruit is sour to sweet, tonic, aphrodisiac, expectorant and removes blood impurities.

Bottle gourd is a large softly pubescent climbing or trailing herb which is said to be indigenous in India, the Molucas and in Abyssinia. It has stout 5-angled stems with bifid tendrils. Leaves are ovate or orbiculate, cordate, dentate, 5-angular or 5-lobed, hairy on both surfaces. Flowers are large, white, solitary, unisexual or bisexual, the males long and females short peduncled. Ovary is oblong, softly pubescent with short style and many ovules. Fruits are large, usually bottle or dumb-bell-shaped, indehiscent and polymorphous. Seeds are many, white, horizontal, compressed, with a marginal groove and smooth. There are sweet fruited and bitter-fruited varieties (Kirtikar and Basu, 1988). The fruit contains a thick white pulp which, in the cultivated variety (kodu) is sweet and edible, while in the smaller wild variety (tamri) it is bitter and a powerful purgative. The seeds yield clear limpid oil which is cooling and is applied to relieve headache. The pulp of the cultivated forms is employed as and adjunct to purgatives and considered cool, diuretic and antibilious, useful in cough, and as an antidote to certain poisons. Externally it is applied as a poultice. The leaves are purgative and recommended to be taken in the form of decoction for jaundice (Nadkarni, 1998). In the case of sweet-fruited variety, the stem is laxative and sweet. The fruit is sweet oleagenous, cardiotonic, general tonic, aphrodisiac, laxative and cooling. In the case of bitter-fruited variety, the leaves are diuretic, antibilious; useful in leucorrhoea, vaginal and uterine complaints and earache. The fruit is bitter, hot, pungent, emetic, cooling, cardiotonic, antibilious; cures asthma, vata, bronchitis, inflammations ulcers and pains.

Ridged gourd or ribbed gourd is a large monoecious climber cultivated throughout India. It is with 5-angled glabrous stems and trifid tendrils. Leaves are orbicular-cordate, palmately 5-7 lobed, scabrous on both sides with prominent veins and veinlets. Flowers are yellow, males arranged in 12-20 flowered axillary racemes. Female flowers are solitary, arranged in the axils of the males. Ovary is strongly ribbed. Fruits are oblong-clavate with 10-sharp angles 15-30cm long, tapering towards the base. Seeds are black, ovoid-oblong, much compressed and not winged (Warrier et al, 1995). The leaves are used in haemorrhoids, leprosy, granular-conjunctivitis and ringworm. The seeds are useful in dermatopathy. The juice of the fresh leaves is dropped into the eyes of children in granular conjunctivitis, also to prevent the lids from adhering at night on account of excessive meihomian secretion (Nadkarni, 1998). Fruits are demulcent, diuretic, tonic, expectorant, laxative and nutritive. The seeds are bitter, emetic, cathartic, expectorant and purgative.

Ash gourd or White gourd melon is a large trailing gourd climbing by means of tendrils which is widely cultivated in tropical Asia. Leaves are large and hispid beneath. Flowers are yellow, unisexual with male peduncle 7.5-10cm long and female peduncle shorter. Fruits are broadly cylindric, 30-45cm long, hairy throughout and ultimately covered with a waxy bloom. The fruits are useful in asthma, cough, diabetes, haemoptysis, hemorrhages from internal organs, epilepsy, fever and vitiated conditions of pitta. The seeds are useful in dry cough, fever, urethrorrhea, syphilis, hyperdipsia and vitiated conditions of pitta (Warrier et al,1993). It is a rejuvenative drug capable of improving intellect and physical strength. In Ayurveda, the fresh juice of the fruit is administered as a specific in haemoptysis and other haemorrhages from internal organs. The fruit is useful in insanity, epilepsy and other nervous diseases, burning sensation, diabetes, piles and dyspepsia. It is a good antidote for many kinds of vegetable, mercurial and alcoholic poisoning. It is also administered in cough, asthma or respiratory diseases, heart diseases and catarrah. Seeds are useful in expelling tapeworms and curing difficult urination and bladder stones. The important formulations using the drug are Kusmandarasayana, Himasagarataila, Dhatryadighrita, Vastyamantakaghrita, Mahaukusmandakaghrita, etc. (Sivarajan et al, 1994).

Fruits contain lupeol, -sitosterol, n-triacontanol, vitamin B, mannitol and amino acids. The fruit is alterative, laxative, diuretic, tonic, aphrodisiac and antiperiodic. Seed and oil from seed is anthelmintic (Husain et al, 1992).

5. Momordica charantia Linn.

Bitter gourd or Carilla fruit is a branched climbing annual which is cultivated throughout India. It is a monoecious plant with angled and grooved stems and hairy or villous young parts. Tendrils are simple, slender and elongate. Leaves are simple, orbicular, cordate and deeply divided into 5-7 lobes. Flowers are unisexual, yellow and arranged on 5-10cm long peduncles. Fruits are 5-15cm long with 3-valved capsules, pendulous, fusiform, ribbed and beaked bearing numerous triangular tubercles. Seeds are many or few with shining sculptured surface. The roots are useful in coloptosis and ophthalmopathy. The leaves are useful in vitiated conditions of pita, helminthiasis, constipation, intermittent fever, burning sensation of the sole and nyctalopia. The fruits are useful in skin diseases, leprosy, ulcers, wounds, burning sensation, constipation, anorexia, flatulence, colic, helminthiasis, rheumatalgia, gout, diabetes, asthma, cough, dysmenorrhoea, impurity of breast milk, fever and debility. Seeds are useful in the treatment of ulcers, pharyngodynia, and obstructions of the liver and spleen. The leaves and fruits are used for external application in lumbago, ulceration and bone fractures and internally in leprosy, haemorrhoids and jaundice (Warrier et al, 1995). The drug improves digestion, calms down sexual urge, quells diseases due to pitta and kapha and cures anaemia, anorexia, leprosy, ulcers, jaundice, flatulence and piles. Fruit is useful in gout, rheumatism and complaints of liver and spleen (Nadkarni, 1954; Aiyer and Kolammal, 1966; Mooss, 1976; Kurup et al, 1979). Kaccoradi taila is an important preparation using the drug (Sivarajan et al, 1994).

The seeds give triterpene glycosides, named momordicosides A, B, C, D and E, which are glycosides of cucurbit-5-en-triol, tetraol or pentaol. Leaves and vines give tetracyclic triterpenes-momordicines I, II and III (bitter principles). Immature fruits give several non-bitter and 2 bitter cucurbitacin glycosides. Four of the non-bitter glycosides, momordicosides F1, F2, G and I and the bitter momordicosides; K and L have also been characterized. Fruits, seeds and tissue culture give a polypeptide which contained 17 types of amino acids and showed hypoglycaemic activity. Fruits also give 5-hydroxy tryptamine and a neutral compound charantin (a steroidal glucoside), diosgenin, cholesterol, lanosterol and -sitosterol. Leaf is emetic, purgative and antibilious. Fruit is stomachic, tonic, carminative, febrifuge, antirheumatic and hypoglycaemic. Root is astringent. Fruit and leaf is anti-leprotic. Fruit, leaf and root are abortifacient and anti-diabetic. Leaf and seed is anthelmintic. Seed oil possesses antifeeding and insecticidal properties. Unsaponifiable matter from seed oil exhibited pronounced inhibitory activity against gram negative bacteria. Seed and fruit are hypoglycaemic, cytotoxic and anti-feedant (Husain et al, 1992).

Tel: Kharbuja-doshavSweet melon is a creeping annual extensively cultivated throughout India, found wild in India, Baluchistan and tropical Africa. The stem is creeping, angular and scabrous. Leaves are orbicular-reniform in outline, 5-angled or lobed, scabrous on both surfaces and often with soft hairs. Lobes of leaves are not very deep nor acute and with 5cm long petiole. Female peduncle is 5cm. Fruit is spherical, ovoid, elongate or contorted, glabrous or somewhat hairy, not spinous nor tuberculate.

Cucumis melo includes two varieties, namely,

C. melo var. momordica syn. C. momordica Roxb.

C. melo var. utilissimus Duthie & Fuller. syn. C. utilissimus Roxb.

The fruit is eaten raw and cooked. Its pulp forms a nutritive, demulcent, diuretic and cooling drink. It is beneficial as a lotion in chronic and acute eczema as well as tan and freckles and internally in cases of dyspepsia. Pulp mixed with cumin seeds and sugar candy is a cool diet in hot season. Seeds yield sweet edible oil which is nutritive and diuretic, useful in painful discharge and suppression of urine. The whole fruit is useful in chronic eczema (Kirtikar & Basu, 1988).

Seeds contain fatty acids-myristic, palmitic, oleic, linoleic; asparagine, glutamine, citrulline, lysine, histidine, arginine, phenylalanine, valine, tyrosine, leucine, iso-leucine, methionine, proline, threonine, tryptophan and crystine. Seed is tonic, lachrymatory, diuretic and urease inhibitor. Fruit pulp is eczemic. Fruit is tonic, laxative, galactagogue, diuretic and diaphoretic. The rind is vulnerary (Husain et al, 1992).

Cucumber is a climbing annual which is cultivated throughout India, found wild in the Himalayas from Kumaon to Sikkim. It is a hispidly hairy trailing or climbing annual. Leaves are simple, alternate, deeply cordate, 3-5 lobed with both surfaces hairy and denticulate margins. Flowers are yellow, males clustered, bearing cohering anthers, connective crusted or elevated above the cells. Females are solitary and thickly covered with very bulbous based hairs. Fruits are cylindrical pepo of varying sizes and forms. Seeds are cream or white with hard and smooth testa. The fruits are useful in vitiated conditions of pitta, hyperdipsia, burning sensation, thermoplegia, fever, insomnia, cephalgia, bronchitis, jaundice, haemorrhages, strangury and general debility. The seeds are useful in burning sensation, pitta, constipation, intermittent fevers, strangury, renal calculus, urodynia and general debility (Warrier et al, 1994). The leaves boiled and mixed with cumin seeds, roasted, powdered and administered in throat affections. Powdered and mixed with sugar, they are powerful diuretic (Nadkarni, 1998). The fruits and seeds are sweet, refrigerant, haemostatic, diuretic and tonic. Other important species belonging to the genus are:

Colocynth or Bitter apple is found, cultivated and wild, throughout India in warmer areas. It is an extensively trailing annual herb with bifid tendrils angular branching stems and wooly tender shoots. Leaves are deeply divided, lobes narrow thick, glabrous or somewhat hairy. Flowers are unisexual, yellow, both males and females solitary and with pale-yellow corolla. Fruit is a globose or oblong fleshy indehiscent berry, 5-7.5cm in diameter and variegated with green and white. Seeds are pale brown. The fruits are useful in tumours, ascites, leucoderma, ulcers, asthma, bronchitis, urethrorrhea, jaundice, dyspepsia, constipations, elephantiasis, tubercular glands of the neck and splenomegaly (Warrier et al, 1994). It is useful in abnormal presentations of the foetus and in atrophy of the foetus. In addition to the above properties, the root has a beneficial action in inflammation of the breasts, pain in the joints; externally it is used in ophthalmia and in uterine pains. The fruit and root, with or without is rubbed into a paste with water and applied to boils and pimples. In rheumatism, equal parts of the root and long pepper are given in pill. A paste of the root is applied to the enlarged abdomen of children (Kirtikar and Basu, 1988). The fruit is useful in ascites, biliousness, jaundice, cerebral congestion, colic, constipation dropsy, fever, worms and sciatica. Root is given in cases of abdominal enlargement, cough, asthma, inflammation of the breast, ulcers, urinary diseases and rheumatism. Oil from seeds is used for poisonous bites, bowel complaints, epilepsy and also for blackening the hair (Nadkarni, 1954; Dey, 1980). The important formulations using the root and fruit are Abhayarista, Mahatiktakam kasaya, Manasamitravatakam, Cavikasava, Madhuyastyadi taila, etc. (Sivarajan et al, 1994). The powder is often used as an insecticide. The extract should never be given without some aromatic to correct its griping tendency (Nadkarni, 1998).

Fruit contains a glycoside- colocynthin, its aglycone- -elaterin, citrulluin, citrullene and citrullic acid. Unripe fruit contains p-hydroxy benzyl methyl ester. Roots contain - elaterin and hentriacontane (Husain et al, 1992). Colocynth is, in moderate doses, drastic, hydrogogue, cathartic and diuretic. In large doses, it is emetic and gastro-intestinal irritant and in small doses, it is expectorant and alterative. Colocynthin is a cathartic and intensely bitter principle. It has a purgative action. All parts of the plant are very bitter. The fruit has been described as cathartic (Nadkarni, 1982).

9. Citrullus vulgaris Schrad. syn. C. lanatus (Thunb.) Mats. & Nakai.

Watermelon is an extensively climbing annual which is largely cultivated throughout India and in all warm countries. It has thick angular branching stems. Tendrils are bifid, stout and pubescent. Leaves are long, deeply divided or moderately lobed, glabrous or somewhat hairy and hardly scabrous. Petiole is a little shorter than the limb and villous. Calyx-lobes are narrowly lanceolate, equalling the tube. Corolla is yellow within, greenish outside and villous. Lobes are ovate-oblong, obtuse and prominently 5-nerved. Fruit is sub-globose or ellipsoid, smooth, greenish or clouded, often with a glaucous waxy coating. Flesh is juicy, red or yellowish white. Seeds are usually margined. C. vulgaris var. fistulosus Duthie & Fuller. syn. C. fistulosus has its fruit about the size of small turnip, the seeds of which are used medicinally. The fruit is tasteless when unripe and sweet when ripe. The unripe fruit is used to cure jaundice. Ripe fruit cures kapha and vata and causes biliousness. It is good for sore eyes, scabies and itching. The seeds are tonic to the brain and used as a cooling medicine. An emulsion of the seeds is made into a poultice with the pounded leaves and applied hot in cases of intestinal inflammations (Kirtikar and Basu, 1988). Fruit juice is good in quenching thirst and it is used as an antiseptic in typhus fever with cumin and sugar. It is used as a cooling drink in strangury and affections of urinary organs such as gonorrhoea; in hepatic congestion and intestinal catarrh. The bitter watermelon of Sind is known as “Kirbut” and is used as a purgative.

Seeds yield a fixed oil and proteids; citrullin. Seeds are cooling, demulcent, diuretic, vermifuge and nutritive. Pulp is cooling and diuretic. Fruit-juice is cooling and refreshing (Nadkarni, 1982).

Pompion or Pumpkin is a climbing herb which is considered to be a native of America and cultivated in many parts of India. The stem and leaves are with a harsh prickly armature. Foliage is stiff, more or less rigid and erect. Leaves are with a broad triangular pointed outline and often with deep lobes. Corolla is mostly with erect or spreading (not drooping) pointed lobes, the tube narrowing towards the base. Peduncle is strongly 5-angled and little or much expanding near the fruit. The fruit is cooling and astringent to the bowels, increases appetite, cures leprosy, ‘kapha and vata’, thirst, fatigue and purifies the blood. The leaves are used to remove biliousness. Fruit is good for teeth, throat and eyes and allays thirst. Seeds cure sore chests, haemoptysis, bronchitis and fever. It is good for the kidney and brain. The leaves are used as an external application for burns. The seeds are considered anthelmintic. The seeds are largely used for flavouring certain preparations of Indian hemp, and the root for a nefarious purpose, viz., to make the preparation more potent. The seeds are taeniacide, diuretic and demulcent. The fruit is cooling, laxative and astringent. The leaves are digestible, haematinic and analgesic.

The other important species belonging to the genus Cucurbita is C. maxima Duchena, the seeds of which are a popular remedy for tape-worm and oil as a nervine tonic (Kirtikar & Basu, 1988).

11. Corallocarpus epigaeus Benth. ex Hook. f. syn. Bryonia epigaea Wight.

Corallocarpus is a prostrate or climbing herb distributed in Punjab, Sind, Gujarat, Deccan, Karnataka and Sri Lanka. It is monoecious with large root which is turnip-shaped and slender stem which is grooved, zigzag and glabrous. Tendrils are simple, slender and glabrous. Leaves are sub-orbicular in outline, light green above and pale beneath, deeply cordate at the base, angled or more or less deeply 3-5 lobed. Petiole is long and glabrous. Male flowers are small and arranged at the tip of a straight stiff glabrous peduncle. Calyx is slightly hairy, long and rounded at the base. Corolla is long and greenish yellow. Female flowers are usually solitary with short, stout and glabrous peduncles. Fruit is stalked, long, ellipsoid or ovoid. Seeds are pyriform, turgid, brown and with a whitish corded margin. It is prescribed in later stages of dysentery and old veneral complaints. For external use in chronic rheumatism, it is made into a liniment with cumin seed, onion and castor oil. It is used in case of snakebite where it is administered internally and applied to the bitten part. The root is given in syphilitic rheumatism and later stages of dysentery. The plant is bitter, sweet, alexipharmic and emetic. The root is said to possess alterative and laxative properties (Kirtikar and Basu, 1988). Root contains a bitter principle like Breyonin (Chopra et al, 1980).

Agrotechnology: Cucurbits can be successfully grown during January-March and September- December. For the rainfed crop, sowing can also be started after the receipt of the first few showers.

Pits of 60cm diameter and 30-45cm depth are to be taken at the desired spacing. Well rotten FYM or vegetable mixture is to be mixed with topsoil in the pit and seeds are to be sown at 4-5/pit. Unhealthy plants are to be removed after 2 weeks and retained 2-3 plants/pit. FYM is to be applied at 20-25t/ha as basal dose along with half dose of N (35kg/ha) and full dose of P (25kg) and K (25kg). The remaining dose of N (35kg) can be applied in 2 equal split doses at fortnightly intervals. During the initial stages of growth, irrigation is to be given at an interval of 3-4 days and at alternate days during flowering and fruiting periods. For trailing cucumber, pumpkin and melon, dried twigs are to be spread on the ground. Bitter gourd, bottle gourd, snake gourd and ash gourd are to be trailed on Pandals. Weeding and raking of the soil are to be conducted at the time of fertilizer application. Earthing up may be done during rainy season. The most dreaded pest of cucurbits is fruit flies which can be controlled by using fruit traps, covering the fruits with polythene, cloth or paper bags, removal and destruction of affected fruits and lastly spraying with Carbaryl or Malathion 0. 2% suspension containing sugar or jaggery at 10g/l at fortnightly intervals after fruit set initiation. During rainy season, downy mildew and mosaic diseases are severe in cucurbits. The former can be checked by spraying Mancozeb 0.2%. The spread of mosaic can be checked by controlling the vectors using Dimethoate or Phosphamidon 0.05% and destruction of affected plants and collateral hosts. Action: Bark—demulcent (in coughs and colds), antipyretic and antipruritic (in eruptive fevers), anthelmintic, antidiarrhoeal. An infusion of flowers is given after child birth.

Seeds contain triterpenoid sapo- genols, sterols; leaves contain a tri- terpene ester, beta-amyrin, hexaco- sanol, taraxerol, beta-sitosterol, quer- cetin and taraxeryl acetate.

Careya herbacea Roxb., a related species, is known as Kumbhaadu-lataa in Bengal.

Action: Flowers and pods— purgative, febrifugal, astringent, antibilious. Seed powder—used in amoebiasis.

The Ayurvedic Pharmacopoeia of India indicated the fruit pulp for constipation, colic, chlorosis and urinary disorders.

Pulp of the pod contains anthraqui- none glycosides, sennosides A and B, rhein and its glucoside, barbaloin, aloin, formic acid, butyric acid, their ethyl esters and oxalic acid. Presence of pectin and tannin is also reported.

Seeds gave galactomannan free sugars and free amino acids; extract laxative, carminative, cooling and antipyretic properties.

Flowers gave ceryl alcohol, kaem- pferol, rhein and a bianthraquinone glycoside, fistulin.

Leaves gave free rhein, its glyco- sides—sennosides A and B.

Cassia javanica L., a related species found in West Bengal, Maharashtra and Tamil Nadu, is used as a substitute for Cassia fistula.
