A disease of the duodenum and small intestine caused by the protozoan, Giardia lamblia. Acquired from contaminated drinking water. Common in children. Sometimes associated with ME (chronic fatigue syndrome).

Symptoms. Explosive diarrhoea, flatulence, abdominal pain, pale fatty stools.

Treatment. Cup Fenugreek tea into which has been added 5-10 drops Tincture Goldenseal. 1 cup thrice daily.

An infection of the small intestine caused by the protozoan parasite GIARDIA LAMBLIA. Giardiasis is spread by eating or drinking food or water that is contaminated or by way of direct

contact with someone who is infected.

Most of those infected do not have symptoms. If, however, symptoms do occur, they begin 1–3 days after infection and include diarrhoea and wind. The faeces of those infected tend to be highly foul-smelling, greasy, and float. Abdominal discomfort, cramps, and swelling, loss of appetite, and nausea may also occur. In some cases, giardiasis becomes chronic.

Infection is diagnosed from examination of a faecal sample or by a jejunal biopsy.

Acute giardiasis usually clears up without treatment, but the drug metronidazole quickly relieves symptoms and prevents the spread of infection.

An intestinal tract infection caused by Giardia lamblia, a flagellate protozoa now common to much of the world. Brought in by hikers and the hoards of grazing cattle, wintering over in beavers, elk and moose, it is one of the few parasites to be encountered in the mountains and north country. A condition caused by a parasitic organism known as Giardia lamblia, which is found in the duodenum (see INTESTINE) and the upper part of the small intestine. This organism is usually harmless, but is sometimes responsible for causing diarrhoea. The illness develops one or two weeks after exposure to infection, and usually starts as an explosive diarrhoea, with the passage of pale fatty stools, abdominal pain and nausea. It responds well to METRONIDAZOLE or MEPACRINE.

(lambliasis) n. a disease caused by the parasitic protozoan Giardia lamblia in the small intestine. Individuals become infected by eating food contaminated with cysts containing the parasite. Symptoms include diarrhoea, nausea, bellyache, and flatulence, as well as the passage of pale fatty stools (steatorrhoea). Large numbers of the parasite may interfere with the absorption of food through the gut wall. The disease occurs throughout the world and is particularly common in children; it responds well to oral doses of *metronidazole.

Metronidazole

An antibiotic used widely for anaerobic bacterial infections (including pseudomembranous colitis) and also for such protozoan infections as giardiasis, trichmoniasis and amoebiasis.... metronidazole

Protozoa

A simple, primitive animal comprising a single cell. Protozoa are microscopic in size but are much larger than BACTERIA. Most protozoa live freely, but around 30 are parasitic in humans causing disease such as amoebiasis (see DYSENTERY) and GIARDIASIS (intestinal infections), MALARIA, kala-azar (see LEISHMANIASIS) and SLEEPING SICKNESS. Malabsorption

Impaired absorption of nutrients by the lining of the small intestine. Malabsorption may be caused by many conditions, including lactase deficiency, cystic fibrosis, chronic pancreatitis, coeliac disease, Crohn’s disease, amyloidosis, giardiasis, Whipple’s disease, and lymphoma. The removal of some of the small intestine, and certain operations on the stomach, may also result in malabsorption.

Common symptoms are diarrhoea and weight loss; and in severe cases, there may also be malnutrition (see nutritional disorders), vitamin deficiency, mineral deficiency, or anaemia. Diagnosis may be made by tests on faeces, blood tests, barium X-ray examination and jejunal biopsy. In most cases, dietary modifications or supplements are successful in treating the disorder. In severe cases, intravenous infusion of nutrients is needed (see feeding, artificial).... malabsorption

Habitat: Throughout India.

English: Embelia.

Ayurvedic: Vidanga, Krmighna, Krmihara, Krmiripu, Chitratandula, Jantughna, Jantunaashana, Vella, Amogha.

Unani: Baobarang, Barang Kaabuli.

Siddha/Tamil: Vaayuvidangam.

Action: Ascaricidal, anthelmintic, carminative, diuretic, astringent, anti-inflammatory, antibacterial, febrifuge. Used in diseases of chest and skin. Active principles are found to be oestrogenic and weakly progestogenic. Root—bechic, antidiarrhoeal. Seed—spermicidal, oxytocic, diuretic. The plant is also used for its blood purifying properties. It is an ingredient in cough syrups, preparations for anaemia, genitourinary tract infections, diarrhoea and diseases of the liver.

Embelin, isolated from the berries, shows significant anti-implantation and post-coital antifertility activity. (Successful trials have been carried out at the National Institute of Immunology, New Delhi on human beings.) It is found to be a potential male antifer- tility agent. Spermatogenesis has been impaired and sperm count reduced to the level of infertility. The antisper- matogenic changes are found to be reversible without any toxic effects.

Aqueous and EtOH extract of the fruit—anthelmintic against earthworms. Fruit powder (200 mg/kg), taken with curd on empty stomach, expelled tapeworm within 6-24 h. The treatment was also found effective in giardiasis. EtOH (50%) of the plant was found slightly active against E.coli. Di-salts of embelin—an- thelmintic. Amino salts exhibited less side effects than embelin. The effect of di-isobutyl amino derivatives lasted up to 10 h, also showed anti-inflammatory, hypotensive and antipyretic activities.

Berries gave quinones—embelin, ra- panone, homoembelin, homorapnone and vilangin.

Enteritis

Inflammation of the small intestine. The inflammation may be the result of infection, particularly giardiasis and tuberculosis, or of Crohn’s disease. Enteritis usually causes diarrhoea. (See also gastroenteritis; colitis.)... enteritis

Intestine, Disorders Of

The intestine is subject to various structural abnormalities and to the effects of many infective organisms and parasites; it may also be affected by tumours and other disorders.

Structural abnormalities may be present from birth (congenital) or may develop later. They cause blockage of the intestine (see intestine, blockage of) and include atresia, stenosis, and volvulus. In newborns, meconium (fetal intestinal contents) may block the intestine.

Generalized inflammation of the intestine may result from viral or bacterial infections or from noninfectious causes, as in ulcerative colitis and Crohn’s disease. Gastroenteritis is the term commonly applied to inflammation of the stomach and intestines. Infection encompasses food poisoning, traveller’s diarrhoea, typhoid fever, cholera, amoebiasis, and giardiasis. Intestinal worm infestations include roundworms and tapeworms. Sometimes inflammation is localized, such as in appendicitis and diverticular disease.

Tumours of the small intestine are rare, but noncancerous growths, lymphomas, and carcinoid tumours (causing carcinoid syndrome) occur. Tumours of the large intestine are common (see colon, cancer of; rectum, cancer of). Some forms of familial polyposis may progress to cancer. Impaired blood supply (ischaemia) to the intestine may occur as a result of partial or complete obstruction of the arteries in the abdominal wall (from diseases such as atherosclerosis) or from the blood vessels being compressed or trapped, as in intussusception or hernias. Loss of blood supply may cause gangrene.

Giardia

n. a genus of parasitic pear-shaped protozoa inhabiting the human small intestine. They have four pairs of *flagella, two nuclei, and two sucking discs used for attachment to the intestinal wall. Giardia is usually harmless but may occasionally cause diarrhoea (see giardiasis).... giardia

Importance: Pomegranate has long been esteemed as food and medicine and as a diet in convalescence after diarrhoea. The rind of the fruit is highly effective in chronic diarrhoea and dysentery, dyspepsia, colitis, piles and uterine disorders. The powdered drug boiled with buttermilk is an efficacious reme dy for infantile diarrohoea. The root and stem bark are good for tapeworm and for strengthening the gums. The flowers are useful in vomiting, vitiated conditions of pitta, ophthalmodynia, ulcers, pharyngodynia and hydrocele. An extract of the flowers is very specific for epistaxis. The fruits are useful in anaemia, hyperdipsia, pharyngodynia, ophthalmodynia, pectoral diseases, splenopathy, bronchitis and otalgia. The fruit rind is good for dysentery, diarrhoea and gastralgia. Seeds are good for scabies, hepatopathy and splenopathy. The important preparations using the drug are Dadimadighrtam, Dadimastaka churnam, Hinguvacadi churnam, Hingvadi gulika, etc (Sivarajan et al, 1994, Warrier et al, 1995).

Distribution: Pomegranate is a native of Iran, Afghanistan and Baluchistan. It is found growing wild in the warm valleys and outer hills of the Himalaya between 900m and 1800m altitude. It is cultivated throughout India, the largest area being in Maharastra.

Botany: Punica granatum Linn. belongs to the family Punicaceace. It is a large deciduous shrub up to 10m in height with smooth dark grey bark and often spinescent branchlets. Leaves are opposite, glabrous, minutely pellucid-punctuate, shining above and bright green beneath. Flowers are scarlet red or sometime yellow, mostly solitary, sometimes 2-4 held together. Stamens are numerous and inserted on the calyx below the petals at various levels. Fruits are globose, crowned by the persistent calyx. Rind is coriaceous and woody, interior septate with membraneous walls containing numerous seeds. Seeds are angular with red, pink or whitish, fleshy testa (Warrier et al, 1995).

Agrotechnology: Pomegranate is of deciduous nature in areas where winters are cold, but on the plains it is evergreen. A hot dry summer aids in the production of best fruits. Plants are grown from seeds as well as cuttings. Mature wood pieces cut into lengths of about 30cm are planted for rooting. The rooted plants are planted 4.5-6m apart. When planted close, they form a hedge which also yields fruits. Normal cultivation and irrigation practices are satisfactory for the pomegranate. An application of 30-45kg of FYM annually to each tree helps to produce superior quality fruits. The pomegranate may be trained as a tree with a single stem for 30-45cm or as a bush with 3 or 4 main stems. In either case suckers arising from the roots and similar growths from the trunk and main branches are removed once a year. Shortening of long slender branches and occasional thinning of branches should be done. The fruit has a tough rind and hence transportation loss is minimum (ICAR, 1966).

Properties and activity: Pomegranate fruit rind gives an ellagitannin named granatin B, punicalagin, punicalin and ellagic acid. Bark contains the alkaloids such as iso-pelletierine, pseudopelletierine, methyl isopelletierine, methyl pelletierine, pelletierine as well as iso-quercetin, friedelin, D- mannitol and estrone. Flowers give pelargonidin-3, 5-diglucoside apart from sitosterol, ursolic acid, maslinic acid, asiatic acid, sitosterol- -D-glucoside and gallic acid. Seeds give malvidin pentose glycoside. Rind gives pentose glycosides of malvidin and pentunidin. Fluoride, calcium, magnesium, vitamin C and phosphate are also reported from fruits. Leaves give elligatannins-granatins A and B and punicafolin.

Rind of fruit is astringent, fruit is laxative. Bark of stem and root is anthelmintic, and febrifuge. Rind of fruit and bark of stem and root is antidiarrhoeal. Pericarp possesses antifertility effect. Fixed oil from seeds are antibacterial. Bark, fruit pulp, flower and leaf are antifungal. Aerial part is CNS depressant, diuretic and hypothermic. The flower buds of pomegranate in combination with other plants showed excellent response to the patients of Giardiasis (Mayer et al, 1977; Singhal et al, 1983).... pomegranate
