Copper: From 4 Different Sources


Important nutrient. Required for conversion of iron into haemoglobin. See: HAEMOGLOBIN. Tyrosinase is organic copper and as such is the missing link between anaemia and iron. Copper has long been known as a preventative and treatment for anaemia. Its levels increase steadily during pregnancy. A lack of the metal turns the hair white and predisposes to schizophrenia. If allowed to accumulate in the body its effect may be toxic. High levels are mostly found in high blood pressure, smokers and those with heart failure.

While deficiencies are not common, it increases red blood cell count, maintains bone health and strength, and assists enzyme function.

Copper is “pro-oxidant”, promoting formation of free radicals (highly unstable molecules capable of damaging walls of arteries when in excess). Potential narrowing of arteries may occur when high levels of copper are present with a high LDL cholesterol and low Selenium. RDA 0.05 to 0.2mg.

Copper is an essential nutrient for humans, and all tissues in the human body contain traces of it. The total amount in the adult body is 100– 150 mg. Many essential enzyme systems are dependent on traces of copper; on the other hand, there is no evidence that dietary de?ciency of copper ever occurs in humans. Infants are born with an ample store, and the normal diet for an adult contains around 2 mg of copper a day. It is used in medicine as the two salts, sulphate of copper (blue stone) and nitrate of copper. The former is, in small doses, a powerful astringent, and in larger doses an irritant. Both are caustics when applied externally. Externally, either is used to rub on unhealthy ulcers and growths to stimulate the granulation tissue to more rapid healing.

Penicillamine

A metabolite of PENICILLIN which is one of the CHELATING AGENTS. It is sometimes used in RHEUMATOID ARTHRITIS that has not responded to the ?rst-line remedies and it is particularly useful when the disease is complicated by VASCULITIS. Penicillamine is also used as an antidote to poisoning by heavy metals, particularly copper and lead, as it is able to bind these metals and so remove their toxic effects. Because of its ability to bind copper it is also used in WILSON’S DISEASE where there is a de?ciency in the copper-binding protein so that copper is able to become deposited in the brain and liver, damaging these tissues.... penicillamine

Wilson’s Disease

A rare, inherited disorder in which copper accumulates in the liver, resulting in conditions such as hepatitis and cirrhosis. Copper is slowly released into other body parts, damaging the brain, causing mild intellectual impairment, and leading to debilitating rigidity, tremor, and dementia. Symptoms usually appear in adolescence but can occur much earlier or later. Lifelong treatment with penicillamine is needed and, if begun soon enough, can sometimes produce some improvement. If the disease is discovered before the onset of symptoms, the drug may prevent them from developing.... wilson’s disease

Ascorbic Acid

Ascorbic acid, or vitamin C (see APPENDIX 5: VITAMINS), is a simple sugar found in living tissues – its highest concentrations being in the adrenal cortex (see ADRENAL GLANDS) and the eye. Stress and CORTICOTROPIN lead to a loss of ascorbic acid from the adrenal cortex. Fresh fruit and vegetables, particularly blackcurrants, citrus fruits, berries and green vegetables, are the richest dietary sources; it may also be synthetically prepared. Ascorbic acid is easily eliminated from the diet by traditional methods of cooking, being very soluble in water and easily destroyed by heat, alkalis, traces of copper or by an oxidase released by damage to plant tissues. De?ciency may lead to SCURVY, traditionally associated with sailors, among elderly people living alone or in poor communities living at subsistence level. It has been claimed that large doses (1–2 g daily) will prevent the common cold, but few large controlled trials have been carried out and it is inadvisable for people to dose themselves with large quantities of ascorbic acid, which may result in the formation of oxalate stones in the urinary tract. Asphodelus Fistulosus

Linn.

Synonym: A. tenuifolius Cav.

Family: Liliaceae.

Habitat: Most parts of the plains from West Bengal westwards to Punjab and Gujarat, as a field weed.

English: Asphodel. Unani: Piyaazi, Khunsaa, Asraash. (Asphodelus tenuifolius Cav. is equated with Shellot, Gandanaa.)

Folk: Bokat.

Action: Seeds—diuretic; applied externally to ulcer and inflamed parts.

Seeds contain an ester, 1-O-17-me- thylstearylmyoinositol. The seed oil yields myristic, palmitic, oleic, linole- ic, linolenic acids, beta-amyrin and beta-sitosterol. The oil, due to its high linolenic content (62.62%), may be of therapeutic value in preventing atherosclerosis.

The mineral elements present in the weed are iron 178.4, zinc 44.5 and copper 6.4 ppm. A triterpenoid, lupeol and quercetin are also present.... asphodelus fistulosus

About the Nutrients in This Food Cocoa beans are high-carbohydrate, high-protein food, with less dietary fiber and more fat than all other beans, excepting soy beans. The cocoa bean’s dietary fiber includes pectins and gums. Its proteins are limited in the essential amino acids lysine and isoleucine. Cocoa butter, the fat in cocoa beans, is the second most highly saturated vegetable fat (coconut oil is number one), but it has two redeeming nutritional qualities. First, it rarely turns rancid. Second, it melts at 95°F, the temperature of the human tongue. Cocoa butter has no cholesterol; neither does plain cocoa powder or plain dark chocolate. Cocoa beans have B vitamins (thiamine, riboflavin, niacin) plus min- erals (iron, magnesium, potassium, phosphorus, and copper). All chocolate candy is made from chocolate liquor, a thick paste pro- duce by roasting and grinding cocoa beans. Dark (sweet) chocolate is made of chocolate liquor, cocoa butter, and sugar. Milk chocolate is made of choc- olate liquor, cocoa butter, sugar, milk or milk powder, and vanilla. White * These values apply to plain cocoa powder and plain unsweetened chocolate. Add- ing other foods, such as milk or sugar, changes these values. Because chocolate is made from a bean, it also contains dietary fiber and measurable amounts of certain minerals. For example, one ounce of dark chocolate, the most nutritious “eating” chocolate, has 1.6 g dietary fiber, 0.78 mg iron (4 percent of the R DA for a woman, 10 percent of the R DA for a man), 32 mg magnesium (11 percent of the R DA for a woman, 8 percent of the R DA for a man), and .43 mg zinc (5 percent of the R DA for a woman, 4 percent of the R DA for a man). Cocoa beans, cocoa, and chocolate contain caffeine, the muscle stimulant theobro- mine, and the mood-altering chemicals phenylethylalanine and anandamide (see below).

The Most Nutritious Way to Serve This Food With low-fat milk to complete the proteins without adding saturated fat and cholesterol. NOTE : Both cocoa and chocolate contain oxalic acid, which binds with calcium to form cal- cium oxalate, an insoluble compound, but milk has so much calcium that the small amount bound to cocoa and chocolate hardly matters. Chocolate skim milk is a source of calcium.

Buying This Food Look for: Tightly sealed boxes or bars. When you open a box of chocolates or unwrap a candy bar, the chocolate should be glossy and shiny. Chocolate that looks dull may be stale, or it may be inexpensively made candy without enough cocoa butter to make it gleam and give it the rich creamy mouthfeel we associate with the best chocolate. (Fine chocolate melts evenly on the tongue.) Chocolate should also smell fresh, not dry and powdery, and when you break a bar or piece of chocolate it should break cleanly, not crumble. One exception: If you have stored a bar of chocolate in the refrigerator, it may splinter if you break it without bringing it to room temperature first.

Storing This Food Store chocolate at a constant temperature, preferably below 78°F. At higher temperatures, the fat in the chocolate will rise to the surface and, when the chocolate is cooled, the fat will solidif y into a whitish powdery bloom. Bloom is unsightly but doesn’t change the chocolate’s taste or nutritional value. To get rid of bloom, melt the chocolate. The chocolate will turn dark, rich brown again when its fat recombines with the other ingredients. Chocolate with bloom makes a perfectly satisfactory chocolate sauce. Dark chocolate (bitter chocolate, semisweet chocolate) ages for at least six months after it is made, as its flavor becomes deeper and more intense. Wrapped tightly and stored in a cool, dry cabinet, it can stay fresh for a year or more. Milk chocolate ages only for about a month after it is made and holds its peak flavor for about three to six months, depending on how carefully it is stored. Plain cocoa, with no added milk powder or sugar, will stay fresh for up to a year if you keep it tightly sealed and cool.

What Happens When You Cook This Food Chocolate burns easily. To melt it without mishap, stir the chocolate in a bowl over a pot of hot water or in the top of a double boiler or put the chocolate in a covered dish and melt it in the microwave (which does not get as hot as a pot on the store). Simple chemistry dictates that chocolate cakes be leavened with baking soda rather than baking powder. Chocolate is so acidic that it will upset the delicate balance of acid (cream of tartar) and base (alkali = sodium bicarbonate = baking soda) in baking powder. But it is not acidic enough to balance plain sodium bicarbonate. That’s why we add an acidic sour-milk product such as buttermilk or sour cream or yogurt to a chocolate cake. Without the sour milk, the batter would be so basic that the chocolate would look red, not brown, and taste very bitter.

How Other Kinds of Processing Affect This Food Freezing. Chocolate freezes and thaws well. Pack it in a moistureproof container and defrost it in the same package to let it reabsorb moisture it gave off while frozen.

Medical Uses and/or Benefits Mood elevator. Chocolate’s reputation for making people feel good is based not only on its caffeine content—19 mg caffeine per ounce of dark (sweet) chocolate, which is one-third the amount of caffeine in a five-ounce cup of brewed coffee—but also on its naturally occurring mood altering chemicals phenylethylalanine and anandamide. Phenylethylalanine is found in the blood of people in love. Anandamide stimulates areas of your brain also affected by the active ingredients in marijuana. (NOTE : As noted by the researchers at the Neurosci- ences Institute in San Diego who identified anandamide in chocolate in 1996, to get even the faintest hint of marijuana-like effects from chocolate you would have to eat more than 25 pounds of the candy all at once.) Possible heart health benefits. Chocolate is rich in catechins, the antioxidant chemicals that give tea its reputation as a heart-protective anticancer beverage (see tea). In addition, a series of studies beginning with those at the USDA Agricultural Research Center in Peoria, Illinois, suggest that consuming foods rich in stearic acid like chocolate may reduce rather than raise the risk of a blood clot leading to a heart attack. Possible slowing of the aging process. Chocolate is a relatively good source of copper, a mineral that may play a role in slowing the aging process by decreasing the incidence of “protein glycation,” a reaction in which sugar molecules ( gly = sugar) hook up with protein molecules in the bloodstream, twisting the protein molecules out of shape and rendering them unusable. This can lead to bone loss, rising cholesterol, cardiac abnormalities, and a slew of other unpleasantries. In people with diabetes, excess protein glycation may be one factor involved in complications such as loss of vision. Ordinarily, increased protein glyca- tion is age-related. But at the USDA Grand Forks Human Nutrition Research Center in North Dakota, agricultural research scientist Jack T. Saari has found that rats on copper-deficient diets experience more protein glycation at any age than other rats. A recent USDA survey of American eating patterns says that most of us get about 1.2 mg copper a day, considerably less than the Estimated Safe and Adequate Daily Dietary Intake (ESADDI) or 1.5 mg to 3 mg a day. Vegetarians are less likely to be copper deficient because, as Saari notes, the foods highest in copper are whole grains, nuts, seeds, and beans, including the cocoa bean. One ounce of dark chocolate has .25 mg copper (8 –17 percent of the ESADDI).

Adverse Effects Associated with This Food Possible loss of bone density. In 2008, a team of Australian researchers at Royal Perth Hos- pital, and Sir Charles Gairdner Hospital published a report in the American Journal of Clinical Nutrition suggesting that women who consume chocolate daily had 3.1 percent lower bone density than women who consume chocolate no more than once a week. No explanation for the reaction was proposed; the finding remains to be confirmed. Possible increase in the risk of heart disease. Cocoa beans, cocoa powder, and plain dark chocolate are high in saturated fats. Milk chocolate is high in saturated fats and cholesterol. Eating foods high in saturated fats and cholesterol increases the amount of cholesterol in your blood and raises your risk of heart disease. NOTE : Plain cocoa powder and plain dark chocolate may be exceptions to this rule. In studies at the USDA Agricultural Research Center in Peoria, Illinois, volunteers who consumed foods high in stearic acid, the saturated fat in cocoa beans, cocoa powder, and chocolate, had a lower risk of blood clots. In addition, chocolate is high in flavonoids, the antioxidant chemicals that give red wine its heart-healthy reputation. Mild jitters. There is less caffeine in chocolate than in an equal size serving of coffee: A five- ounce cup of drip-brewed coffee has 110 to 150 mg caffeine; a five-ounce cup of cocoa made with a tablespoon of plain cocoa powder ( 1/3 oz.) has about 18 mg caffeine. Nonetheless, people who are very sensitive to caffeine may find even these small amounts problematic. Allergic reaction. According to the Merck Manual, chocolate is one of the 12 foods most likely to trigger the classic food allergy symptoms: hives, swelling of the lips and eyes, and upset stomach.* The others are berries (blackberries, blueberries, raspberries, strawberries), corn, eggs, fish, legumes (green peas, lima beans, peanuts, soybeans), milk, nuts, peaches, pork, shellfish, and wheat (see wheat cer ea ls).

Food/Drug Interactions Monoamine oxidase (MAO) inhibitors. Monoamine oxidase inhibitors are drugs used to treat depression. They inactivate naturally occurring enzymes in your body that metabolize tyra- mine, a substance found in many fermented or aged foods. Tyramine constricts blood vessels and increases blood pressure. Caffeine is a substance similar to tyramine. If you consume excessive amounts of a caffeinated food, such as cocoa or chocolate, while you are taking an M AO inhibitor, the result may be a hypertensive crisis. False-positive test for pheochromocytoma. Pheochromocytoma, a tumor of the adrenal gland, secretes adrenalin, which the body converts to VM A (vanillylmandelic acid). VM A is excreted in urine, and, until recently, the test for this tumor measured the level of VM A in the urine. In the past, chocolate and cocoa, both of which contain VM A, were eliminated from the patient’s diet prior to the test lest they elevate the level of VM A in the urine and produce a false-positive result. Today, more finely drawn tests usually make this unnecessary. * The evidence link ing chocolate to allergic or migraine headaches is inconsistent. In some people, phenylet hylamine (PEA) seems to cause headaches similar to t hose induced by t yramine, anot her pressor amine. The PEA-induced headache is unusual in t hat it is a delayed react ion t hat usually occurs 12 or more hours after t he chocolate is eaten.... chocolate

The genus Curcuma belonging to the family Zingiberaceae comprises of a number of species which are medicinally very important. Among them, the most important species are described below.

1. C. amada Roxb.

Mango ginger is cultivated in Gujarat and found wild in parts of West Bengal, U. P, Karnataka and Tamil Nadu. It is a rhizomatous aromatic herb with a leafy tuft and 60-90cm in height. Leaves are long, petiolate, oblong-lanceolate, tapering at both ends, glabrous and green on both sides. Flowers are white or pale yellow, arranged in spikes in the centre of tuft of the leaves. Lip is semi -elliptic, yellow, 3-lobbed with the mid lobe emarginate. The rhizomes are useful in vitiated conditions of pitta, anorexia, dyspepsia, flatulence, colic, bruises, wounds, chronic ulcers, skin diseases, pruritus, fever, constipations, strangury, hiccough, cough, bronchitis, sprains, gout, halitosis, otalgia and inflammations (Warrier et al, 1994). The fresh root possesses the smell of green mango and hence the name mango ginger. The rhizomes are used externally in the form of paste as an application for bruises and skin diseases generally combined with other medicines. Tubers rubbed with the leaf- juice of Caesalpinia bonduc is given for worms (Nadkarni, 1982).

The essential oil contains -pinene, -and -curcumene, camphor, cuminyl alcohol, myristic acid and turmerone. Car-3-ene and cis-ocimene contribute the characteristic mango odour of the rhizome. Rhizome is CNS active, hypothermic and it shows potentiation of amphetamine toxicity. Tuber is trypsin inhibitor and is effective against Vibrio cholerae (Husain et al, 1992). The rhizomes are bitter, sweet sour, aromatic, cooling, appetiser, carminative, digestive, stomachic, demulcent, vulnerary, febrifuge, alexertic, aphrodisiac, laxative, diurectic, expectorant, antiinflammatory and antipyretic (Warrier et al, 1994).

Wild turmeric or Cochin turmeric or Yellow zeodoary is found wild throughout India and cultivated in Bengal and Kerala. It is a perennial tuberous herb with annulate, aromatic yellow rhizome which is internally orange-red in colour. Leaves are elliptic or lanceolate- oblong, caudate-acuminate, 30-60cm long, petioles as long or even longer, bracts ovate, recurved, more or less tinged with red or pink. Flowers are pink, lip yellow, obovate, deflexed, sub-entire or obscurely three lobed. Fruits are dehiscent, globose, 3-valved capsules. Rhizomes are used in combination with astringents and aromatics for bruises, sprains, hiccough, bronchitis, cough, leucoderma and skin eruptions (Warrier et al, 1994). The rhizomes have an agreeable fragrant smell and yield a yellow colouring matter like turmeric, and the fresh root has a camphoraceous odour. The dried rhizome is used as a carminative and aromatic adjunctant to other medicines (Nadkarni, 1998).

Essential oil contains -and - -curcumene, d-camphene and p-methoxy cinnamic acid. The colouring matter is curcumin. Numerous sesquiterpenoids of germacrone and guaiane skeletons have been identified recently. Rhizome has effect on respiration. It is spasmolytic and shows antagonism of amphetamine hyperactivity. Rhizome is an anti-dote for snakebite and carminative (Husain et al, 1992).

Turmeric is cultivated all over India, particularly in W. Bengal, T. N and Maharashtra. It is a perennial herb, 60-90cm in height, with a short stem and tufts of erect leaves. Rhizome is cylindric, ovoid, orange coloured and branched. Leaves are simple, very large, petiole as long as the blade, oblong-lanceolate, tapering to the base upto 45cm long. Flowers are pale yellow, arranged in spikes concealed by the sheathing petioles and flowering bracts are pale green (Warrier et al, 1994). Turmeric occupies an important position in the life of Indian people as it forms an integral part of the rituals, ceremonies and cuisine. Due to the strong antiseptic properties, turmeric has been used as a remedy for all kinds of poisonous affections, ulcers and wounds. It gives good complexion to the skin and so it is applied to face as a depilatory and facial tonic. The drug cures diseases due to morbid vata, pitta and kapha, diabetes, eye diseases, ulcers, oedema, anaemia, anorexia, leprosy and scrofula. It purifies blood by destroying the pathogenic organisms. A paste of turmeric alone, or combined with a paste of neem (Azadirachta indica) leaves, is used to cure ringworm, obstinate itching, eczema and other parasitic skin diseases and in chicken pox and small pox. The drug is also useful in cold, cough, bronchitis, conjunctivitis and liver affections (Nadkarni, 1954; Kurup et al,1979; Kolammal, 1979). The rhizome is the officinal part and is an important ingredient of formulations like Nalpamaradi taila, Jatyadi taila, Narayana gula, etc. (Sivarajan et al, 1994).

Turmeric paste mixed with a little limejuice and saltpetre and applied hot is a popular application to sprains and bruises. In smallpox and chickenpox, a coating of turmeric is applied to facilitate the process of scabbing. The smoke produced by sprinkling powdered turmeric over burnt charcoal will relieve scorpion sting when the part affected is exposed to the smoke for a few minutes. Turmeric and alum powder in the proportion of 1:20 is blown into the ear in chronic otorrhoea (Nadkarni, 1998). “Haridra Khand”, a compound containing powdered turmeric, sugar and many other ingredients is a well-known preparation for cold, cough and flu, and for skin diseases. In Unani system, roasted turmeric is an ingredient of “Hab Narkachur”, used as antidysenteric for children (Thakur et al, 1989).

Essential oil contains ar-turmerone, and ar-curcumene as ma jor constituents. Some of the other compounds are -and -pinene, sabinene, myrcene, -terpinene, limonene, p- cymene, perillyl alcohol, turmerone, eugenol, iso-eugenol, eugenol methyl ether and iso- eugenol methyl ether. Curcumin and related compounds have also been reported as major constituents of the rhizomes. Recently a number of sesquiterpenes have been reported from C. longa, viz., the sesquiterpenoids of germacrane, bisabolane and guainane skeletons (Husain et al, 1992). The study of sesquiterpenes has revealed a new compound curlone (Kisoy et al, 1983). The crystalline colouring matter curcumin (0. 6%) is diferuloyl methane (Mathews et al, 1980). Stigmasterol, cholestrol, -sitosterol and fatty acids, mainly straight chain dienoic acids are reported (Moon et al, 1977). Curcumin, the colouring agent and major constituent of C. longa, is said to possess local as well as systemic antiinflammatory property which has been found to compare favourably with phenylbutazone (Srimal and Dhawan, 1973). An extract of the crude drug ‘akon’ containing the rhizomes exhibited intensive preventive activity against carbon tetrachloride induced liver injury invivo and invitro. The liver protecting effects of some analogs of ferulic acid and p-coumaric acid, probable metabolites of the curcuminoids have been also evaluated (Kiso et al, 1983). Curcumin is antiinflammatory. Rhizome is antiprotozoal, spasmolytic, CNS active, antiparasitic, antispasmodic, antibacterial, antiarthritic, anthelmintic, carminative, antiperiodic, emo llient, anodyne, laxative, diruretic, expectorant, alterative, alexertive, febrifuge, opthalmic and tonic.

The round zedoary or Zerumbet is mostly found in India and S. E. Asia. The plant has 4-6 leaves with 20-60cm long lamina. The leaf lami na is oblong-lanceolate, finely acuminate and glabrous on both the surfaces. Flower stalk is 20-25cm long, emerging before the leaves. Flowers are yellow, while the flowering bract is green tinged with red. Calyx is 8mm long, corolla tube is twice as long as the calyx. Capsule is ovoid, trigonous, thin smooth and bursting irregularly. Tubers are palmately branched and camphoraceous (Thakur et al, 1989). The identity of the plant sources of the drug Karcura is a matter of debate. There is difference of opinion among men of Ayurveda, as to whether Sati and Karcura are the same drug or different. Many authors consider them different and equate Sati with Hedychium spicatum Smith. and Karcura with C. zedoaria, both belonging to Zingiberaceae (Kurup et al,1979; Chunekar 1982; Sharma, 1983). Some others treat them to be the same and equate it with C. zedoaria (Kirtikar and Basu, 1918; Vaidya, 1936; Nadkarni, 1954; Kapoor and Mitra, 1979). However, the source of Karcura in Kerala in the recent times has been Kaempferia galanga of the same family. The rhizome of C. zedoaria is used as appetiser and tonic, particularly prescribed to ladies after childbirth. In case of cold, a decoction of long pepper (Piper longum), cinnamon (Cinnamomum verum), zedoary and honey is given. In Ayurveda it is an ingredient of “Braticityadi kwatha”, used in high fever (Thakur et al, 1989). Root is useful in flatulence and dyspepsia, and as a corrector of purgatives. Fresh root checks leucorrhoeal and gonorrhoeal discharges. Root powder is a good substitute for many foreign foods for infants. For worms, the juice from the tubers is given to children. Juice of the leaves is given in dropsy (Nadkarni, 1982). It is an odoriferous ingredient of the cosmetics used for the cure of chronic skin diseases caused by impure or deranged blood (Nadkarni, 1998).

Essential oil from rhizomes contains -pinene, d-camphene, cineole, d-camphor, sesquiterpenes and sesquiterpene alcohols (Husain et al, 1992). The novel sesquiterpenoids which have been isolated and characterised are cuzerenone, epi-cuzerenone, iso- furanogermerene, curcumadiol, curcumol, curcumenol, iso-curcumenol, procurcumenol, dehydrocurdione (Hikino et al, 1968, 1971, 1972), germacrone-4, 5-epoxide, germacrone, germacrone furanodienone, curcumenol, iso-curcumenol, curcumanolides A and B and curcumenone (Shiobara et al, 1985). The starch left after the extraction is purified and sold as a commodity of cottage industry in West-Bengal under the name ‘Shoti’ (Rao et al, 1928). Ethyl-p methoxy-cinnamate has been isolated from the alcoholic extract of the plant (Gupta et al, 1976). Rhizome is stomachic, diuretic, and carminative and gastrointestinal stimulant.

Agrotechnology: Curcuma species are tropical herbs and can be grown on different types of soils both under irrigated and rainfed conditions. Rich loamy soils having good drainage are ideal for the crop. The plant is propagated by whole or split mother rhizomes. Well developed, healthy and disease free rhizomes are to be selected. Rhizomes are to be treated with copper oxychloride fungicides and stored in cool, dry place or earthen pits plastered with mud and cowdung. The best season of planting is during April with the receipt of pre-monsoon showers. The land is to be prepared to a fine tilth during February-March. On receipt of pre- monsoon showers in April, beds of size 3x1.2m with a spacing of 40cm between beds are to be prepared. Small pits are to be taken in the beds in rows with a spacing of 25-40cm.

Finger rhizomes are to be planted flat with buds facing upwards and covered with soil or dry powdered cattle ma nure. The crop is to be mulched immediately after planting and 50 days after first mulching. Cattle manure or compost is to be applied as basal dose at 20-40t/ha at the time of land preparation or by spreading over the beds after planting. Application of NPK fertilizers is beneficial and found to increase the yield considerably. Weeding is to be done twice at 60 and 120 days after planting, depending upon weed intensity. Earthing up is to be done after 60 days. No major incidence of pest or disease is noticed in this crop. Leaf blotch and leaf spot can be controlled by spraying Bordeaux mixture or 0.2% Mancozeb. Shoot borers can be controlled by spraying 0.05% Dimethoate or 0.025% Quinalphos. Time of harvest usually extends from January-March. Harvesting is generally done at about 7-10 months after planting depending upon the species and variety. Harvested rhizomes are to be cleaned of mud and other materials adhering to them. The egg package must have a “refrigera- tion required” label plus safe-handling instructions on eggs that have not been treated to kill Salmonella. Look for: Eggs that fit your needs. Eggs are graded by the size of the yolk and the thick- ness of the white, qualities that affect appearance but not nutritional values. The higher the grade, the thicker the yolk and the thicker the white will be when you cook the egg. A Grade A A egg fried sunny side up will look much more attractive than a Grade B egg prepared the same way, but both will be equally nutritions. Egg sizes ( Jumbo, Extra large, Large, Medium, Small) are determined by how much the eggs weigh per dozen. The color of the egg’s shell depends on the breed of the hen that laid the egg and has nothing to do with the egg’s food value.

Storing This Food Store fresh eggs with the small end down so that the yolk is completely submerged in the egg white (which contains antibacterial properties, nature’s protection for the yolk—or a developing chick embryo in a fertilized egg). Never wash eggs before storing them: The water will make the egg shell more porous, allowing harmful microorganisms to enter. Store separated leftover yolks and whites in small, tightly covered containers in the refrigerator, where they may stay fresh for up to a week. Raw eggs are very susceptible to Salmonella and other bacterial contamination; discard any egg that looks or smells the least bit unusual. Refrigerate hard-cooked eggs, including decorated Easter eggs. They, too, are suscep- tible to Salmonella contamination and should never be left at room temperature.

Preparing This Food First, find out how fresh the eggs really are. The freshest ones are the eggs that sink and lie flat on their sides when submerged in cool water. These eggs can be used for any dish. By the time the egg is a week old, the air pocket inside, near the broad end, has expanded so that the broad end tilts up as the egg is submerged in cool water. The yolk and the white inside have begun to separate; these eggs are easier to peel when hard-cooked. A week or two later, the egg’s air pocket has expanded enough to cause the broad end of the egg to point straight up when you put the egg in water. By now the egg is runny and should be used in sauces where it doesn’t matter if it isn’t picture-perfect. After four weeks, the egg will float. Throw it away. When you whip an egg white, you change the structure of its protein molecules which unfold, breaking bonds between atoms on the same molecule and forming new bonds to atoms on adjacent molecules. The result is a network of protein molecules that hardens around air trapped in bubbles in the net. If you beat the whites too long, the foam will turn stiff enough to hold its shape even if you don’t cook it, but it will be too stiff to expand natu- rally if you heat it, as in a soufflé. When you do cook properly whipped egg white foam, the hot air inside the bubbles will expand. Ovalbumin, an elastic protein in the white, allows the bubble walls to bulge outward until they are cooked firm and the network is stabilized as a puff y soufflé. The bowl in which you whip the whites should be absolutely free of fat or grease, since the fat molecules will surround the protein molecules in the egg white and keep them from linking up together to form a puff y white foam. Eggs whites will react with metal ions from the surface of an aluminum bowl to form dark particles that discolor the egg-white foam. You can whip eggs successfully in an enamel or glass bowl, but they will do best in a copper bowl because copper ions bind to the egg and stabilize the foam.

What Happens When You Cook This Food When you heat a whole egg, its protein molecules behave exactly as they do when you whip an egg white. They unfold, form new bonds, and create a protein network, this time with molecules of water caught in the net. As the egg cooks, the protein network tightens, squeez- ing out moisture, and the egg becomes opaque. The longer you cook the egg, the tighter the network will be. If you cook the egg too long, the protein network will contract strongly enough to force out all the moisture. That is why overcooked egg custards run and why overcooked eggs are rubbery. If you mix eggs with milk or water before you cook them, the molecules of liquid will surround and separate the egg’s protein molecules so that it takes more energy (higher heat) to make the protein molecules coagulate. Scrambled eggs made with milk are softer than plain scrambled eggs cooked at the same temperature. When you boil an egg in its shell, the air inside expands and begins to escape through the shell as tiny bubbles. Sometimes, however, the force of the air is enough to crack the shell. Since there’s no way for you to tell in advance whether any particular egg is strong enough to resist the pressure of the bubbling air, the best solution is to create a safety vent by sticking a pin through the broad end of the egg before you start to boil it. Or you can slow the rate at which the air inside the shell expands by starting the egg in cold water and letting it warm up naturally as the water warms rather than plunging it cold into boiling water—which makes the air expand so quickly that the shell is virtually certain to crack. As the egg heats, a little bit of the protein in its white will decompose, releasing sulfur that links up with hydrogen in the egg, forming hydrogen sulfide, the gas that gives rot- ten eggs their distinctive smell. The hydrogen sulfide collects near the coolest part of the egg—the yolk. The yolk contains iron, which now displaces the hydrogen in the hydrogen sulfide to form a green iron-sulfide ring around the hard-cooked yolk.

How Other Kinds of Processing Affect This Food Egg substitutes. Fat-free, cholesterol-free egg substitutes are made of pasteurized egg whites, plus artificial or natural colors, flavors, and texturizers (food gums) to make the product look and taste like eggs, plus vitamins and minerals to produce the nutritional equivalent of a full egg. Pasteurized egg substitutes may be used without additional cooking, that is, in salad dressings and eggnog. Drying. Dried eggs have virtually the same nutritive value as fresh eggs. Always refrigerate dried eggs in an air- and moistureproof container. At room temperature, they will lose about a third of their vitamin A in six months.

Medical Uses and/or Benefits Protein source. The protein in eggs, like protein from all animal foods, is complete. That is, protein from animal foods provides all the essential amino acids required by human beings. In fact, the protein from eggs is so well absorbed and utilized by the human body that it is considered the standard by which all other dietary protein is measured. On a scale known as biological value, eggs rank 100 ; milk, 93; beef and fish, 75; and poultry, 72. Vision protection. The egg yolk is a rich source of the yellow-orange carotenoid pigments lutein and zeaxanthin. Both appear to play a role in protecting the eyes from damaging ultraviolet light, thus reducing the risk of cataracts and age-related macular degeneration, a leading cause of vision of loss in one-third of all Americans older than 75. Just 1.3 egg yolks a day appear to increase blood levels of lutein and zeaxanthin by up to 128 percent. Perhaps as a result, data released by the National Eye Institute’s 6,000-person Beaver Dam ( Wisconsin) Eye Study in 2003 indicated that egg consumption was inversely associated with cataract risk in study participants who were younger than 65 years of age when the study started. The relative risk of cataracts was 0.4 for people in the highest category of egg consumption, compared to a risk of 1.0 for those in the lowest category. External cosmetic effects. Beaten egg whites can be used as a facial mask to make your skin look smoother temporarily. The mask works because the egg proteins constrict as they dry on your face, pulling at the dried layer of cells on top of your skin. When you wash off the egg white, you also wash off some of these loose cells. Used in a rinse or shampoo, the pro- tein in a beaten raw egg can make your hair look smoother and shinier temporarily by filling in chinks and notches on the hair shaft.

Adverse Effects Associated with This Food Increased risk of cardiovascular disease. Although egg yolks are high in cholesterol, data from several recent studies suggest that eating eggs may not increase the risk of heart disease. In 2003, a report from a 14-year, 177,000-plus person study at the Harvard School of Public Health showed that people who eat one egg a day have exactly the same risk of heart disease as those who eat one egg or fewer per week. A similar report from the Multiple R isk Factor Intervention Trial showed an inverse relationship between egg consumption and cholesterol levels—that is, people who ate more eggs had lower cholesterol levels. Nonetheless, in 2006 the National Heart, Lung, and Blood Institute still recommends no more than four egg yolks a week (including the yolk in baked goods) for a heart-healthy diet. The American Heart Association says consumers can have one whole egg a day if they limit cholesterol from other sources to the amount suggested by the National Cholesterol Education Project following the Step I and Step II diets. (Both groups permit an unlimited number of egg whites.) The Step I diet provides no more than 30 percent of total daily calories from fat, no more than 10 percent of total daily calories from saturated fat, and no more than 300 mg of cholesterol per day. It is designed for healthy people whose cholesterol is in the range of 200 –239 mg/dL. The Step II diet provides 25– 35 percent of total calories from fat, less than 7 percent of total calories from saturated fat, up to 10 percent of total calories from polyunsaturated fat, up to 20 percent of total calories from monounsaturated fat, and less than 300 mg cho- lesterol per day. This stricter regimen is designed for people who have one or more of the following conditions: •  Existing cardiovascular disease •  High levels of low-density lipoproteins (LDLs, or “bad” cholesterol) or low levels of high-density lipoproteins (HDLs, or “good” cholesterol) •  Obesity •  Type 1 diabetes (insulin-dependent diabetes, or diabetes mellitus) •  Metabolic syndrome, a.k.a. insulin resistance syndrome, a cluster of risk fac- tors that includes type 2 diabetes (non-insulin-dependent diabetes) Food poisoning. Raw eggs (see above) and egg-rich foods such as custards and cream pies are excellent media for microorganisms, including the ones that cause food poisoning. To protect yourself against egg-related poisoning, always cook eggs thoroughly: poach them five minutes over boiling water or boil at least seven minutes or fry two to three minutes on each side (no runny center) or scramble until firm. Bread with egg coating, such as French toast, should be cooked crisp. Custards should be firm and, once cooked, served very hot or refrigerated and served very cold. Allergic reaction. According to the Merck Manual, eggs are one of the 12 foods most likely to trigger the classic food allergy symptoms: hives, swelling of the lips and eyes, and upset stomach. The others are berries (blackberries, blueberries, raspberries, strawberries), choco- late, corn, fish, legumes (green peas, lima beans, peanuts, soybeans), milk, nuts, peaches, pork, shellfish, and wheat (see wheat cer ea ls).

Food/Drug Interactions Sensitivity to vaccines. Live-virus measles vaccine, live-virus mumps vaccine, and the vac- cines for influenza are grown in either chick embryo or egg culture. Roots contain beta-amyrin, friede- lin, genianine and 16 xanthones including mangiferin. Mangiferin is protective activity against induced liver injury in albino rats. Xanthones also showed activity against Mycobacterium tuberculosis.

Dried crude powder of the whole plant exhibited anticonvulsant activity in albino rats.

Action: Fruit pulp—sedative, emollient and refrigerant; used as poultice, applied to burns, inflammations, boils, and burns. Seeds—diuretic, anthelmintic (for tapeworm). Because of their zinc content and antimitotic effect, seeds are used to arrest enlargement of prostate gland. Also used in cystitis and minor kidney dyfunction.

Pumpkin (Cucurbita maxima and C. pepo) seeds contain B vitamins, Vitamin A; minerals—calcium, iron, phosphorus, zinc; cucurbitacins; linonelic acid. An infusion of seeds (2-3 teaspoons) is taken as a diuretic and in hypertrophy of prostate.

Seeds of C. maxima contain sterol glycosides and sterol fatty acid esters which showed antitumour activity in mice.

The leaves contain calcium 36.38; magnesium 38.80; iron 2.04; zinc 0.76; and copper 0.42 (mg/100 g).

The plant contains tannins, saponins and allantoin, and monomethyl ethers of cyclitols. Leaves yielded a pyrrolizidine alkaloid, creatinine. arsenic effectively. It can be used in purification of silver-containing waste water, also for the treatment of low- level liquid radioactive wastes and mercurial waste water. The plant has a strong capacity for removing phenol. Biomass of non-living dried water Hyacinth roots showed high absorption of copper from aqueous solutions.

Action: Weed—one of the richest source of minerals, chiefly iodine, sodium, manganese, sulphur, silicon, zinc and copper. Effective against obesity, antirheumatic. Stimulates circulation of lymph. Endocrine gland stimulant. Allays onset of arteriosclerosis by maintaining elasticity of walls of blood vessels. Mild diuretic, bulk, laxative, antibiotic. High sodium content may reduce effectiveness of diuretics.

(The herb contains trace metal, particularly iodine from 0.03-1.0%. It may contain waste metals such as cadmium and strontium, when grown in a polluted environment. Variable iodine content and arsenic contamination make the herb unsafe.)

The herb should be used with caution in hyperthyroidism and hypothyroidism. Excess thyroid activity maybe aggravated by the iodine content of the herb; it may disrupt thyroid function. One gram of Bladderwrack might contain as much as 600 mcg iodine (Ingesting more than 150 mcg iodine per day may cause hyperthyroidism or exacerbate existing hyperthyroidism.) Wilson’s Disease

Wilson’s disease, or hepatolenticular degeneration, is a familial disease in which there is an increased accumulation of COPPER in the liver, brain, and other tissues including the kidneys. Its main manifestation is the development of tremor and rigidity, with di?culty in speech. In many cases there is improvement following the administration of dimercaprol, penicillamine, or trientine dihydrochloride; these substances cause an increased excretion of copper.... wilson’s disease

Aids

Acquired Immune Deficiency Syndrome. Infection by HIV virus may lead to AIDS, but is believed to be not the sole cause of the disease. It strikes by ravaging the body’s defence system, destroying natural immunity by invading the white blood cells and producing an excess of ‘suppressant’ cells. It savages the very cells that under normal circumstances would defend the body against the virus. Notifiable disease. Hospitalisation. AIDS does not kill. By lacking an effective body defence system a person usually dies from another infection such as a rare kind of pneumonia. There are long-term patients, more than ten years after infection with HIV who have not developed AIDS. There are some people on whom the virus appears to be ineffective. The HIV virus is transmitted by infected body fluids, e.g. semen, blood or by transfusion.

A number of co-factors are necessary for AIDS to develop: diet, environment, immoral lifestyle, drugs, etc also dispose to the disease which, when eliminated, suggest that AIDS needs not be fatal. However, there is no known cure. Smoking hastens onset. Causes include needle-sharing and sexual contacts. Also known as the ‘Gay Plague’ it can be transmitted from one member of the family to another non-sexual contact.

The virus kills off cells in the brain by inflammation, thus disposing to dementia.

Symptoms. Onset: brief fever with swollen glands. “Feeling mildly unwell”. This may pass off without incident until recurrence with persistent diarrhoea, night sweats, tender swollen lymph nodes, cough and shortness of breath. There follows weight loss, oral candida. Diagnosis is confirmed by appearance of ugly skin lesions known as Kaposi’s sarcoma – a malignant disease. First indication is the appearance of dark purple spots on the body followed by fungoid growths on mouth and throat.

While some cases of STDs have been effectively treated with phytotherapy, there is evidence to suggest it may be beneficial for a number of reasons. Whatever the treatment, frequent blood counts to monitor T-4 cells (an important part of the immune system) are necessary. While a phytotherapeutic regime may not cure, it is possible for patients to report feeling better emotionally and physically and to avoid some accompanying infections (candida etc).

Treatment. Without a blood test many HIV positives may remain ignorant of their condition for many years. STD clinics offer free testing and confidential counselling.

Diet. Vitamin C-rich foods, Lecithin, Egg Yolk, Slippery Elm gruel, Red Beet root, Artichokes. Garlic is particularly indicated as an anti-infective.

Nutrition. Vitamin A is known to increase resistance by strengthening the cell membrane; preferably taken as beta carotene 300,000iu daily as massive doses of Vitamin A can be toxic. Amino acid – Glutathione: Garlic’s L-cysteine relates.

Vitamin C. “The virus is inactivated by this vitamin. Saturating cells infected with the HIV virus with the vitamin results in 99 per cent inactivation of the virus. The vitamin is an anti-viral and immune system modulator without unwanted side-effects. The ascorbate, when added to HIV cells, substantially reduced the virus’s activity without harming the cells at specific concentrations. Patients taking large doses report marked improvement in their condition. Minimum daily oral dose: 10 grams.” (Linus Pauling Institute, Science and Medicine, Palo Alto, California, USA)

Periwinkle. An anti-AIDS compound has been detected in the Madagascan Periwinkle (Catharanthus roseus), at the Chelsea Physic Garden.

Mulberry. The black Mulberry appears to inhibit the AIDS virus.

Hyssop. An AIDS patient improved to a point where ulcers were healed, blood infection eliminated, and Kaposi’s sarcoma started to clear when her mother gave her a traditional Jamaican tea made from Hyssop, Blessed Thistle and Senna. From test-tube research doctors found that Hyssopus officinalis could be effective in treatment of HIV/AIDS. (Medical Journal Antiviral Research, 1990, 14, 323-37) Circumcision. Studies have shown that uncircumcised African men were more than five to eight times more likely to contract AIDS than were circumcised men; life of the virus being short-lived in a dry environment. Study. A group of 13 HIV and AIDS patients received 200mg capsules daily of a combination of Chelidonium (Greater Celandine) 175mg; Sanguinaria (Blood root) 5mg; and Slippery Elm (Ulmus fulva) 20mg. More than half the patients enjoyed increased energy and improved immune function with reduction in both size and tenderness of lymph nodes. Bastyr College of Naturopathy, Seattle, MA, USA. During 1991 the College carried out a study which claimed that a combination of natural therapies including nutrition, supplements, herbal medicine, hydrotherapy and counselling had successfully inhibited HIV and other viral activity in all patients in controlled trials lasting a year.

Patients chosen for the trial were HIV positive, not on anti-viral drugs and showing symptoms of a compromised immune system, but without frank AIDS (generally taken to be indicated by Karposi’s sarcoma and/or PCP-pneumocystitis carinii pneumonia).

Symptoms included: Lymphadenopathy in at least two sites, oral thrush, chronic diarrhoea, chronic sinusitis, leukoplakia, herpes, night sweats and fatigue.

Assessment was subjective and objective (including T-cell ratio tests). The patients did better than comparable groups in published trials using AZT.

Treatment was naturopathic and herbal. Patients receiving homoeopathy and acupuncture did not do as well as those receiving herbs.

Best results with herbs were: Liquorice (1g powder thrice daily); St John’s Wort (Yerba prima tablets, 3, on two days a week only). Patients reported a great increase in the sense of well-being on St John’s Wort. An equivalent dose of fresh plant tincture would be 10ml. The tincture should be of a good red colour. The College did not use Echinacea, which would stimulate the central immune system and which would therefore be contra-indicated.

Supplements given daily. Calcium ascorbate 3g+ (to bowel tolerance). Beta-carotene 300,000iu. Thymus gland extract tablets 6. Zinc 60mg (with some Copper). B-vitamins and EFAs.

To control specific symptoms: most useful herbs were: Tea Tree oil for fungal infections; Goldenseal and Gentian as bitters. Ephedra and Eyebright for sinusitis. Carob drinks for non-specific enteritis. Vitamin B12 and topical Liquorice for shingles.

Counselling and regular massage were used to maintain a positive spirit. Studies show all long term HIV positive survivors have a positive attitude and constantly work at empowering themselves.

Results showed significant improvements in symptoms suffered by HIV patients despite a slow deterioration in blood status. Methods used in the study had dramatically reduced mortality and morbidity. Intrauterine Contraceptive Device (iucd)

A mechanical device, commonly a coil, inserted into the UTERUS to prevent CONCEPTION, probably by interfering with the implantation of the EMBRYO. For many women, IUCDs are an e?ective and acceptable form of contraception, although only about 10 per cent of women in the UK use them. The devices are of various shapes and made of plastic or copper; most have a string that passes through the cervix and rests in the vagina.

Juglans Regia

Linn.

Family: Juglandaceae.

Habitat: Native to Iran; now cultivated in Kashmir, Himachal Pradesh., Khasi Hills and the hills of Uttar Pradesh.

Ayurvedic: Akshoda, Akshoda- ka, Akshota, Shailbhava, Pilu, Karparaal, Vrantphala.

Unani: Akhrot.

Siddha/Tamil: Akrottu.

Action: Leaves and bark— alterative, laxative, antiseptic, mild hypoglycaemic, anti-inflammatory, antiscrofula, detergent. An infusion of leaves and bark is used for herpes, eczema and other cutaneous affections; externally to skin eruptions and ulcers. Volatile oil— antifungal, antimicrobial.

Key application (leaf) ? In mild, superficial inflammation of the skin and excessive perspiration of hands and feet. (German Commission E.). When English Walnuts (Juglans regia) are added to low fat diet, total cholesterol may be decreased by 412% and LDL by 8-16%. (Natural Medicines Comprehensive Database, 2007.)

Walnut hull preparations are used for skin diseases and abscesses.

Walnut is eaten as a dry fruit. Because of its resemblance to the brain, it was thought, according to the "doctrine of signatures", to be a good brain tonic. Walnuts are also eaten to lower the cholesterol levels.

From the volatile oil of the leaves terpenoid substances (monoterpenes, sesquiterpenes, diterpene and triter- pene derivatives) and eugenol have been isolated. Fatty acids, including geranic acid; alpha-and beta-pinene, 1,8,cincole, limonene, beta-eudesmol and juglone are also important constituents of the volatile oil.

The leaves contain napthoquinones, mainly juglone. The root bark gave 3, 3',-bis-juglone and oligomeric ju- glones. Unripe fruit husk also gave napthoquinones.

The kernels of Indian walnuts contain 15.6% protein, 11% carbohydrates, 1.8% mineral matter (sodium, potassium, calcium, magnesium, iron, copper, phosphorus, sulphur and chorine). Iodine (2.8 mcg/100 g), arsenic, zinc, cobalt and manganese are also reported. Kernels are also rich in vitamins of the B group, vitamin A (30 IU/100 g), and ascorbic acid (3 mg/100 g).

The juice of unripe fruits showed significant thyroid hormone enhancing activity (prolonged use of such extract may cause serious side effect).

White Walnut, Lemon Walnut, Butternut, Oilnut of the USA is equated with Juglans cineraria L. The inner bark gave napthoquinones, including juglone, juglandin, juglandic acid, tannins and an essential oil.

Butternut is used as a dermatologi- cal and antihaemorrhoidal agent. Ju- glone exhibits antimicrobial, antipara- sitic and antineoplastic activities.

Liver

The liver is the largest gland in the body, serving numerous functions, chie?y involving various aspects of METABOLISM.

Form The liver is divided into four lobes, the greatest part being the right lobe, with a small left lobe, while the quadrate and caudate lobes are two small divisions on the back and undersurface. Around the middle of the undersurface, towards the back, a transverse ?ssure (the porta hepatis) is placed, by which the hepatic artery and portal vein carry blood into the liver, and the right and left hepatic ducts emerge, carrying o? the BILE formed in the liver to the GALL-BLADDER attached under the right lobe, where it is stored.

Position Occupying the right-hand upper part of the abdominal cavity, the liver is separated from the right lung by the DIAPHRAGM and the pleural membrane (see PLEURA). It rests on various abdominal organs, chie?y the right of the two KIDNEYS, the suprarenal gland (see ADRENAL GLANDS), the large INTESTINE, the DUODENUM and the STOMACH.

Vessels The blood supply di?ers from that of the rest of the body, in that the blood collected from the stomach and bowels into the PORTAL VEIN does not pass directly to the heart, but is ?rst distributed to the liver, where it breaks up into capillary vessels. As a result, some harmful substances are ?ltered from the bloodstream and destroyed, while various constituents of the food are stored in the liver for use in the body’s metabolic processes. The liver also receives the large hepatic artery from the coeliac axis. After circulating through capillaries, the blood from both sources is collected into the hepatic veins, which pass directly from the back surface of the liver into the inferior vena cava.

Minute structure The liver is enveloped in a capsule of ?brous tissue – Glisson’s capsule – from which strands run along the vessels and penetrate deep into the organ, binding it together. Subdivisions of the hepatic artery, portal vein, and bile duct lie alongside each other, ?nally forming the interlobular vessels,

which lie between the lobules of which the whole gland is built up. Each is about the size of a pin’s head and forms a complete secreting unit; the liver is built up of hundreds of thousands of such lobules. These contain small vessels, capillaries, or sinusoids, lined with stellate KUPFFER CELLS, which run into the centre of the lobule, where they empty into a small central vein. These lobular veins ultimately empty into the hepatic veins. Between these capillaries lie rows of large liver cells in which metabolic activity occurs. Fine bile capillaries collect the bile from the cells and discharge it into the bile ducts lying along the margins of the lobules. Liver cells are among the largest in the body, each containing one or two large round nuclei. The cells frequently contain droplets of fat or granules of GLYCOGEN – that is, animal starch.

Functions The liver is, in e?ect, a large chemical factory and the heat this produces contributes to the general warming of the body. The liver secretes bile, the chief constituents of which are the bile salts (sodium glycocholate and taurocholate), the bile pigments (BILIRUBIN and biliverdin), CHOLESTEROL, and LECITHIN. These bile salts are collected and formed in the liver and are eventually converted into the bile acids. The bile pigments are the iron-free and globin-free remnant of HAEMOGLOBIN, formed in the Kup?er cells of the liver. (They can also be formed in the spleen, lymph glands, bone marrow and connective tissues.) Bile therefore serves several purposes: it excretes pigment, the breakdown products of old red blood cells; the bile salts increase fat absorption and activate pancreatic lipase, thus aiding the digestion of fat; and bile is also necessary for the absorption of vitamins D and E.

The other important functions of the liver are as follows:

In the EMBRYO it forms red blood cells, while the adult liver stores vitamin B12, necessary for the proper functioning of the bone marrow in the manufacture of red cells.

It manufactures FIBRINOGEN, ALBUMINS and GLOBULIN from the blood.

It stores IRON and copper, necessary for the manufacture of red cells.

It produces HEPARIN, and – with the aid of vitamin K – PROTHROMBIN.

Its Kup?er cells form an important part of the RETICULO-ENDOTHELIAL SYSTEM, which breaks down red cells and probably manufactures ANTIBODIES.

Noxious products made in the intestine and absorbed into the blood are detoxicated in the liver.

It stores carbohydrate in the form of glycogen, maintaining a two-way process: glucose

glycogen.

CAROTENE, a plant pigment, is converted to vitamin A, and B vitamins are stored.

It splits up AMINO ACIDS and manufactures UREA and uric acids.

It plays an essential role in the storage and metabolism of FAT.... liver

Orotates

Orotic acid aids absorption of minerals by cells, and ensures against loss of these important nutrients through the action of free-radicals or infection. Actinomycosis

An infection caused by ACTINOMYCES ISRAELII or related actinomycete bacteria.

The most common form of actinomycosis affects the jaw area.

A painful swelling appears and pus discharges through small openings that develop in the skin.

Another form of actinomycosis affects the pelvis in women, causing lower abdominal pain and bleeding between periods.

This form was associated with a type of IUD, no longer in use, that did not contain copper.

Rarely, forms of the disorder affect the appendix or lung.

Liver Disease In The Tropics

ACUTE LIVER DISEASE The hepatitis viruses (A– F) are of paramount importance. Hepatitis E (HEV) often produces acute hepatic failure in pregnant women; extensive epidemics – transmitted by contaminated drinking-water supplies – have been documented. HBV, especially in association with HDV, also causes acute liver failure in infected patients in several tropical countries: however, the major importance of HBV is that the infection leads to chronic liver disease (see below). Other hepatotoxic viruses include the EPSTEIN BARR VIRUS, CYTOMEGALOVIRUS (CMV), the ?avivirus causing YELLOW FEVER, Marburg/Ebola viruses, etc. Acute liver disease also occurs in the presence of several acute bacterial infections, including Salmonella typhi, brucellosis, leptospirosis, syphilis, etc. The complex type of jaundice associated with acute systemic bacterial infection – especially pneumococcal PNEUMONIA and pyomiositis – assumes a major importance in many tropical countries, especially those in Africa and in Papua New Guinea. Of protozoan infections, plasmodium falciparum malaria, LEISHMANIASIS, and TOXOPLASMOSIS should be considered. Ascaris lumbricoides (the roundworm) can produce obstruction to the biliary system. CHRONIC LIVER DISEASE Long-term disease is dominated by sequelae of HBV and HCV infections (often acquired during the neonatal period), both of which can cause chronic active hepatitis, cirrhosis, and hepatocellular carcinoma (‘hepatoma’) – one of the world’s most common malignancies. Chronic liver disease is also caused by SCHISTOSOMIASIS (usually Schistosoma mansoni and S. japonicum), and acute and chronic alcohol ingestion. Furthermore, many local herbal remedies and also orthodox chemotherapeutic compounds (e.g. those used in tuberculosis and leprosy) can result in chronic liver disease. HAEMOSIDEROSIS is a major problem in southern Africa. Hepatocytes contain excessive iron – derived primarily from an excessive intake, often present in locally brewed beer; however, a genetic predisposition seems likely. Indian childhood cirrhosis – associated with an excess of copper – is a major problem in India and surrounding countries. Epidemiological evidence shows that much of the copper is derived from copper vessels used to store milk after weaning. Veno-occlusive disease was ?rst described in Jamaica and is caused by pyrrolyzidine alkaloids (present in bush-tea). Several HIV-associated ‘opportunistic’ infections can give rise to hepatic disease (see AIDS/HIV).

A localised (focal) form of liver disease in all tropical/subtropical countries results from invasive Entamoeba histolytica infection (amoebic liver ‘abscess’); serology and imaging techniques assist in diagnosis. Hydatidosis also causes localised liver disease; one or more cysts usually involve the right lobe of the liver. Serological tests and imaging techniques are of value in diagnosis. Whilst surgery formerly constituted the sole method of management, prolonged courses of albendazole and/or praziquantel have now been shown to be e?ective; however, surgical intervention is still required in some cases.

Hepato-biliary disease is also a problem in many tropical/subtropical countries. In southeast Asia, Clonorchis sinensis and Opisthorchis viverini infections cause chronic biliary-tract infection, complicated by adenocarcinoma of the biliary system. Praziquantel is e?ective chemotherapy before advanced disease ensues. Fasciola hepatica (the liver ?uke) is a further hepato-biliary helminthic infection; treatment is with bithionol or triclabendazole, praziquantel being relatively ine?ective.... liver disease in the tropics

Lobelia

Lobelia inflata. N.O. Campanulaceae.

Synonym: Indian Tobacco, Pukeweed, Emetic Weed.

Habitat: North America ; cultivated in Salt Lake City.

Features ? A biennial herb, in height from twelve to eighteen inches, the stem is angular and slightly hairy. One to three inches long, the leaves are alternate, sessile, and ovate-lanceolate, with small, whitish glands on the edge. The fruit is in the form of a flat, oval capsule, which contains ovate-oblong seeds about one eighth of an inch long, brown in colour, with a reticulated, pitted surface. The root is fibrous, and the plant bears a small blue, pointed flower. The taste is burning and acrid like tobacco, the odour slight. Part used ? Herb and seeds are used.

Action: Emetic, stimulant, antispasmodic, expectorant, and diaphoretic.

Lobelia inflata has for many years been one of the most widely discussed and hotly debated articles used in medicine. While many herbalists contend that it is the most valuable of all botanic remedies, official medicine in England classifies it as a poison. Herbalists who use Lobelia insist that it is most certainly not a poison, and that it can be administered by them in large doses with perfect safety. They use it chiefly as an emetic, and, as its administration brings about the prompt removal of accumulations of mucus, the action in bronchial complaints is speedy and beneficial. Coffin's comments in this connection are enthusiastic ? "Lobelia is decidedly the most certain and efficient emetic known, and is at the same time safe in its operations. Unlike most emetics from the mineral kingdom, it produces its specific effect without corroding the stomach or producing morbid irritation and inflammation of the mucous membrane of this organ, which are so common in the use of antimony, zinc, and the sulphate of copper. Lobelia may emphatically be said to 'operate in unison with the laws of life'."

In view of the controversy surrounding its use, the history of Lobelia is interesting. North American Indians had apparently long been acquainted

with its properties, but its first introduction to general use was due to the efforts of the famous American, Samuel Thomson. His disciple, Dr. Coffin, brought the herb to this country and used it extensively in his practice for over forty years, apparently with great success "in almost every form of disease, and from the tender infant to the aged," to quote Coffin himself. In both America and Britain herbalists have been tried on charges of causing death by administering Lobelia, but in no instance has a verdict been obtained against them.... lobelia

Liver, Diseases Of

The LIVER may be extensively diseased without any obviously serious symptoms, unless the circulation through it is impeded, the out?ow of BILE checked, or neighbouring organs implicated. JAUNDICE is a symptom of several liver disorders, and is discussed under its separate heading. ASCITES, which may be caused by interference with the circulation through the portal vein of the liver, as well as by other reasons, is also considered separately. The presence of gallstones is a complication of some diseases connected with the liver, and is treated under GALLBLADDER, DISEASES OF. For hydatid cyst of the liver, see TAENIASIS. Liver diseases in a tropical environment are dealt with later in this section.

In?ammation of the liver, or HEPATITIS, may occur as part of a generalised infection or may be a localised condition. Infectious hepatitis, which is the result of infection with a virus, is one of the most common forms. Many di?erent viruses can cause hepatitis, including that responsible for glandular fever (see MONONUCLEOSIS). Certain spirochaetes may also be the cause, particularly that responsible for LEPTOSPIROSIS, as can many drugs. Hepatitis may also occur if there is obstruction of the BILE DUCT, as by a gall-stone.

Cirrhosis of the liver A disorder caused by chronic damage to liver cells. The liver develops areas of ?brosis or scarring; in response, the remaining normal liver cells increase and form regeneration nodules. Those islands of normality, however, suffer from inadequate blood supply, thus adversely affecting liver function. Alcohol is the most common cause of cirrhosis in the United Kingdom and the USA, and the incidence of the disorder among women in the UK has recently risen sharply as a consequence of greater consumption of alcohol by young women in the latter decades of the 20th century. In Africa and many parts of Asia, infection with hepatitis B virus is a common cause. Certain drugs – for example, PARACETAMOL – may damage the liver if taken in excess. Unusual causes of cirrhosis include defects of the bile ducts, HAEMOCHROMATOSIS (raised iron absorption from the gut), CYSTIC FIBROSIS, cardiac cirrhosis (the result of heart failure causing circulatory congestion in the liver), and WILSON’S DISEASE (raised copper absorption).

Symptoms Some people with cirrhosis have no signs or symptoms and the disease may be diagnosed at a routine medical examination. Others may develop jaundice, OEDEMA (including ascites – ?uid in the abdomen), fever, confusion, HAEMATEMESIS (vomiting blood), loss of appetite and lethargy. On examination, cirrhotic patients often have an enlarged liver and/ or SPLEEN, and HYPERTENSION. Liver function tests, cholangiography (X-ray examination of the bile ducts) and biopsy of liver tissue will help to reach a diagnosis.

Treatment Nothing can be done to repair a cirrhosed organ, but the cause, if known, must be removed and further advance of the process thus prevented. In the case of the liver, a high-protein, high-carbohydrate, low-fat diet is given, supplemented by liver extract and vitamins B and K. The consumption of alcohol should be banned. In patients with liver failure and a poor prognosis, liver TRANSPLANTATION is worthwhile but only after careful consideration.

Abscess of the liver When an ABSCESS develops in the liver, it is usually a result of amoebic DYSENTERY, appearing sometimes late in the disease – even after the diarrhoea is cured (see below). It may also follow upon in?ammation of the liver due to other causes. In the case of an amoebic abscess, treatment consists of oral metronidazole.

Acute hepatic necrosis is a destructive and often fatal disease of the liver which is very rare. It may be due to chemical poisons, such as carbontetrachloride, chloroform, phosphorus and industrial solvents derived from benzene. It may also be the cause of death in cases of poisoning with fungi. Very occasionally, it may be a complication of acute infectious hepatitis.

Cancer of the liver is not uncommon, although it is rare for the disease to begin in the liver – the involvement of this organ being usually secondary to disease situated somewhere in the stomach or bowels. Cancer originating in the liver is more common in Asia and Africa. It usually arises in a ?brotic (or cirrhotic) liver and in carriers of the hepatitis B virus. There is great emaciation, which increases as the disease progresses. The liver is much enlarged, and its margin and surface are rough, being studded with hard cancer masses of varying size, which can often be felt through the abdominal wall. Pain may be present. Habitat: A large tree, cultivated mainly in Uttar Pradesh, Bihar.

English: Mahua tree, Moha.

Ayurvedic: Madhuuka, Madhu- pushpa, Madhusrav, Gudapushpa.

Unani: Mahuaa.

Siddha/Tamil: Ieluppai.

Action: Flowers—stimulant, demulcent, laxative, anthelmintic, bechic. Seed oil—galactogenic, anticephalgic, emetic. Used in pneumonia, skin diseases, piles. Bark—astringent, emollient. Used for tonsilitis, gum troubles, diabetes, ulcers. Bark, seed oil and gum— antirheumatic.

The Ayurvedic Pharmacopoeia of India recommends the flower without stalk or calyx in asthma and pthisis.

The fruit pulp yielded a number of triterpenoids (including alpha- and beta-amyrin acetate); also n-hexaco- sanol, beta-D-glucoside of beta-sitos- terol and free sitosterol.

Nut shell gave beta-sitosterol gluco- side, quercetin and dihydroquercetin.

The carollas are rich source of sugars, vitamins, phosphorus, calcium and iron; magnesium and copper are also present. The sugars identified are sucrose, maltose, glucose, fructose, ara- binose and rhamnose.

The seeds yielded saponins—2,3- di-O-glucopyranoside of bassic acid (saponin A and saponin B). Mixture of saponins from seeds exhibits spermi- cidal activity.

Momordica Charantia

Linn.

Family: Cucurbitaceae.

Habitat: Cultivated all over India for its fruits.

English: Bitter Gourd, Blsam Pear, Carilla.

Ayurvedic: Kaaravellaka, Kaaravella, Kaathilla, Sushaavi.

Unani: Karelaa.

Siddha/Tamil: Paakal, Paharkai.

Action: Seed/fruit—improves diabetic condition. Fruit—stomachic, laxative, antibilious, emetic, anthelmintic. Used in cough, respiratory diseases, intestinal worms, skin diseases, also for gout and rheumatism. Powdered fruit—applied to wounds and ulcers. Leaf— emetic, antibilious, purgative. Fruit, leaf and root—abortifacient. Leaf and seed—anthelmintic. Root— astringent; appled to haemorrhoids.

Immature fruits gave several nonbitter and bitter momordicosides. Fruits, seeds and tissue culture gave a polypeptide containing amino acids. Fruits also gave 5-hydroxytryptamine, charantin (a steroidal glucoside), dios- genin, cholesterol, lanosterol and beta- sitosterol. Bitter principles are cucur- bitacin glycosides.

Hypoglycaemic effects of the fruit have been demonstrated by blood tests in both humans and animal studies.

Researchers have warned that the fruit extract leads to a false negative test for sugar in the urine (due to its ability to maintain the indicator dye in the glucose oxidase strips and the alkaline copper salts in a reduced state).

Chronic administration of the fruit extract (1.75 g/day for 60 days) to dogs led to testicular lesions with mass atrophy of the spermatogenic elements. The extract reduced the testicular content of RNA, protein and sialic acid as also the acid-phosphatase activity. The fruits and seeds yielded a poly- peptide, p-insulin, which was considered similar to bovine insulin. (Fitoter- apia, 60,1989; Chem Abstr 112,1990.)

The seed and fruit contain an inhibitor of HIV, MAP-30 (Momordi- ca anti-HIV-protein) which exhibited antiviral and antitumour activity in vitro. (Chem Abstr, 113, 1990; ibid, 117, 1992.) Another protein, MRK-29, found in the seed and fruit of a smaller var. of Bitter Gourd found in Thailand, was found to inhibit HIV reverse transcriptase and to increase tumour necrosis factor (TNF). (Planta Med, 67, 2001; Natural Medicines Comprehensive Database, 2007.)

The seeds yield alpha-and beta- momorcharins (glycoproteins). When these glycoproteins were co-cultured with isolated hepatocytes, morphological changes in hepatocytes were observed, indicating hepatotoxicity. Another principle with antilipolytic and lipogenic activities, found along with the alpha-and beta-momorcharin in the seed extract, did not show toxic effect.

Vicine is the hypoglycaemic constituent in the seed. Pure vicine has been found to possess 32.6% hypogly- caemic activity as against 22.2% shown by fresh juice, when tested on albino rats. The vicine is non-haemolytic.

Dosage: Fresh fruit—10-15 ml juice (API, Vol. II); 10-20 ml juice (CCRAS.)... momordica charantia

Nasturtium Officinale

R. Br.

Synonym: Rorippa nasturtium- aquaticum (Linn.) Hayek.

Family: Cruciferae; Brassicaceae.

Habitat: Cultivated in Bengal, Orissa and Punjab.

English: Watercress.

Folk: Piriyaa-Haalim (Punjab), Latputiyaa (Maharashtra).

Action: Leaves—antiscorbutic, expectorant (used in catarrh of the respiratory organs), diuretic (used in kidney and bladder disorders), detoxifying. A lotion of leaves is applied to blotches, spots and blemishes. Fresh herb is used as a blood purifier.

Key application: For catarrh of respiratory tract. (German Commission E.)

Watercress contains vitamin A 4720 IU, ascorbic acid 77 mg/100 g, also thiamine, riboflavin, niacin and biotin; mineral matter 2.2%—calcium 290, phosphorus 140, iron 4.6 mg/100 g, also sulphur, iodine, manganese, zinc, arsenic and copper; proteins 2.9%, amino acid composition includes leucine, phenylalanine, valine, lysine, tyrosine, alanine, threonine, glutamic acid, serine, aspartic acid, cystine, methionine sulphoxide and proline.

The glucosinolate phenethyl isothio- cyanate, which is released upon chewing the leaf, is a chemopreventive agent against lung cancer. (cited in Expanded Commission E Monographs.)

Amalgam

n. an alloy of mercury, especially one used for dental fillings. Dental amalgam is a mixture of mercury with silver, tin, copper, and other trace metals. The components are mixed together in a machine known as an amalgamator.... amalgam

Benedict’s Test

a test for the presence of sugar in urine or other liquids. A few drops of the test solution are added to Benedict’s solution, prepared from sodium or potassium citrate, sodium carbonate, and copper sulphate. The mixture is boiled and shaken for about two minutes, then left to cool. The presence of up to 2% glucose is indicated by the formation of a reddish, yellowish, or greenish precipitate, the highest levels corresponding to the red coloration, the lowest (about 0.05%) to the green. Chalcosis

n. the deposition of copper in the tissues of the eye, usually resulting from the presence of a copper foreign body within the eye.... chalcosis

Globulin

n. one of a group of simple proteins that are soluble in dilute salt solutions and can be coagulated by heat. A range of different globulins is present in the blood (the serum globulins, including alpha (?), beta (?), and *gamma (?) globulins). Some globulins have important functions as antibodies (see immunoglobulin); others are responsible for the transport of lipids, iron, or copper in the blood. Peltophorum Pterocarpum

Family: Caesalpiniaceae.

Habitat: Coastal forests of the Andaman Islands.

English: Copper Pod, Rusty Shield-Bearer.

Siddha/Tamil: Ivalvagai, Perun- gondrai.

Action: Bark—used for dysentery; also used as a constituent of gargles, tooth-powders and lotions for sores and muscular pains.

The bark contains 20.8% of a catechol type of tannin and 9.5% non-tans. The wood and leaves also contain tannin.

The pods contain bergenin which exhibits anti-inflammatory activity in carrageenan-induced rat paw oedema and is found to be equipotent to phenylbutazone.

Aqueous extract of leaves and etha- nolic extract of flower exhibit antifun- gal activity.

Family: Asclepiadaceae.

Habitat: West Bengal, Gujarat and Peninsular India.

Ayurvedic: Kaakanaasaa, Kaakanaasikaa, Kaakaangi, Kaak- tundphalaa, Shringiritti.

Siddha/Tamil: Uppilankodi.

Folk: Singarauti (Gujarat). Amarvel.

Action: Plant—antifungal, antiseptic, keratolytic; used in various skin conditions.

The plant gave n-octacosanol, alpha- amyrin, friedelin, beta-sitosterol. An appreciable amount of salicyclic acid has been isolated from the plant. The plant also yields a cardiac glycoside.

Physalis Peruviana

Linn.

Habitat: Native to tropical America; grown in the hills and plains throughout India.

English: Cape Gooseberry.

Ayurvedic: Parpoti (var.).

Siddha/Tamil: Perungunni, Potti- pallam.

Folk: Rasbhari, Mako.

Action: Plant—diuretic. Leaf— anthelmintic, an infusion is used in abdominal disorders. Fruits— a good source of carotene and ascorbic acid; eaten as a table fruit.

The fruit contain carotene (as vitamin A) 2,380 IU, thiamine 0.05, ri- boflavin 0.02, nicotinic acid 0.3 and ascorbic acid 49 mg/100 g; mineral matter 0.8%; phytin phosphorus 18, iron 2.0, ionizable iron 0.9, sodium 0.9, potassium 320, copper 0.19, and sulphur 43 mg/100 g. The juice from the ripe fruits contain considerable quantity of pectin. The chief acid is citric acid, but malic and tartaric acids are also present.

The plant is a source of highly oxygenated ergostane-type of steroids— withanolides and related compounds.... physalis peruviana

Post-coital Contraception

Action taken to prevent CONCEPTION after sexual intercourse. The type of contraception may be hormonal, or it may be an intrauterine device (see below, and under CONTRACEPTION). Pregnancy after intercourse without contraception – or where contraception has failed as a result, for example, of a leaking condom – may be avoided with a course of ‘morning-after’ contraceptive pills. Such preparations usually contain an oestrogen (see OESTROGENS) and a PROGESTOGEN. Two doses should be taken within 72 hours of ‘unprotected’ intercourse. An alternative for the woman is to take a high dose of oestrogen on its own. The aim is to postpone OVULATION and to affect the lining of the UTERUS so that the egg is unable to implant itself.

Intrauterine contraceptive device (IUCD) This, in e?ect, is a form of post-coital contraception. The IUCD is a plastic shape up to 3 cm long around which copper wire is wound, carrying plastic thread from its tail. Colloquially known as a coil, it acts by inhibiting implantation and may also impair migration of sperm. Devices need changing every 3–5 years. Coils have generally replaced the larger, non-copper-bearing ‘inert’ types of IUCD, which caused more complications but did not need changing (so are sometimes still found in situ). They tend to be chosen as a method of contraception (6 per cent) by older, parous women in stable relationships, with a generally low problem rate.

Nevertheless, certain problems do occur with IUCDs, the following being the most common:

They tend to be expelled by the uterus in women who have never conceived, or by a uterus distorted by, say, ?broids.

ECTOPIC PREGNANCY is more likely.

They are associated with pelvic infection and INFERTILITY, following SEXUALLY TRANSMITTED DISEASES (STDS) – or possibly introduced during insertion.

They often produce heavy, painful periods (see MENSTRUATION), and women at high risk of these problems (e.g. women who are HIV positive [see AIDS/HIV], or with WILSON’S DISEASE or cardiac lesions) should generally be excluded – unless the IUCD is inserted under antibiotic cover.... post-coital contraception

Kayser–fleischer Ring

a brownish-yellow ring in the outer rim of the cornea of the eye. It is a deposit of copper granules and is diagnostic of *Wilson’s disease. When well developed it can be seen by unaided observation, but faint Kayser-Fleischer rings may only be detected by specialized *slit-lamp ophthalmological examination. [B. Porphyrin

n. one of a number of pigments derived from *porphin, which are widely distributed in living things. All porphyrins form chelates with iron, magnesium, zinc, nickel, copper, and cobalt. These chelates are constituents of *haemoglobin, *myoglobin, the *cytochromes, and chlorophyll, and are thus important in many oxidation/reduction reactions in all living organisms. Trace Element

an element that is required in minute concentrations for normal growth and development; the body contains a total of <5 g of the element. Trace elements include fluorine (see fluoride), manganese, *zinc, copper, *iodine, cobalt, *selenium, molybdenum, *chromium, and silicon. They may serve as *cofactors or as constituents of complex molecules (e.g. cobalt in vitamin B12).... trace element

Rumex Scutatus

Habitat: Western Himalayas up to an altitude of 2,400 m.

English: French Sorrel.

Action: Plant—refrigerant, astringent; given in dysentery. Juice of leaves—antiscorbutic.

Action: Plant—astringent antiscorbutic, stomachic, diuretic, used for disorders of lymphatic and glandular system; for bronchitis, asthma; constipation, dyspepsia, diseases of liver and spleen; urinary and renal disorders; alcoholism. Seeds—antidysenteric.

Anthraquinone glucosides, emodin and chrysophanol, have been reported from leaves, root and seeds. The leaves contain large amounts of oxalate (21.8% on dry basis); vitamin C content is 12 mg and vitamin A 6,100 IU/100 g.

The leaves of Rumex species are eaten in salad or cooked like spinach. They contain protein, carbohydrates, potassium, magnesium, phosphorus, calcium, manganese, copper, zinc, (iodine, in some samples), ascorbic acid, beta-carotene and thiamine; also oxalic acid, potassium binoxalate and some tartaric acid.... rumex scutatus

Secale Cereale

Linn.

Family: Poaceae.

Habitat: Ladakh, Lahul and other north-western Himalayan areas; and as host for cultivation of medicinal ergot (fungus) in Kashmir.

English: Rye Grass.

Action: Grass—used for benign prostatic hyperplasia (BPH), chronic prostatis and prostatodynia.

Rye bread, biscuits, porridge and alcoholic products are available in European countries and the US. Rye grain contains 12.1% protein; made up of 42% gliadin (a prolamine), 42% glutelin, 8% globulin and 8% albumin. The biological value of Rye protein at 5% level of intake is 80.4% and the coefficient of true digestibility 91.0%.

The mineral contents in the grain are: calcium 61, potassium 453, magnesium 155, phosphorus 376, sulphur 146 and iron 4.8 mg/100 g; and small amounts of zinc, copper, manganese and aluminium. The carbohydrates (73.4%) include surcose, pentosans, starch and raffinose.

Medicinally applicable part of Rye Grass is the polan extract. The extract contains beta-sitosterol; relaxes urethral smooth muscle tone and increases bladder muscle contraction. Some evidence suggests that it might affect alpha-adrenergic receptors and relax the internal and external bladder sphincter muscle. The extract does not affect LH, FSH, testosterone or dihydrotestosterone. A specific Rye Grass pollen extract 126 mg three times daily has been used for BHP. (It is not known if Rye Grass pollen is comparable to finasteride or hytrin. However, it is comparable to Pygeum and Paraprost, a Japanese prostate remedy containing L-glutamic acid, L-alanine and aminoacetic acid.)... secale cereale

Stereospermum Personatum

English: Trumpet Flower, Yellow Snake tree.

Ayurvedic: Paatalaa, Paatali, Paata- lai, Krishna-vrantaa, Madhu-duuti,

Kaama-duuti, Ativallabhaa, Taam- rapushpi, Kuberaakshi. Amoghaa, Kumbhipushpi, Ambuvaasini. Copper-red-flowered var., known as Taamrapushpi, is equated with S. suaveolens (Paatalaa) and the white- flowered one with S. chelenoides. (Paatalai).

Siddha/Tamil: Paadiri.

Action: The Ayurvedic Pharmacopoeia of India recommends the root of Paatalaa in lipid disorders; the stem bark in oedema and retention of urine.

The white-flowered var. purifies blood, increases appetite and is prescribed for vomiting, hiccough, thirst, oedema and inflammatory chest diseases.

The copper-red-flowered var. is prescribed in difficult breathing, vomiting, oedema, flatus and high fever.

Ethanolic extract of the plant showed hypoglycaemic and anticancer activity experimentally.

A decoction of S. personatum root is prescribed for asthma and cough; of the leaves in chronic dyspepsia. A decoction of the root and leaves is credited with antipyretic properties. The bark exhibited antibacterial and antitubercular properties.

A decoction of S. suaveolens roots is prescribed for intermittent and puerperal fevers, inflammatory affections of the chest. Extracts of the plant contain lapachol.

The leaves of S. chelonoides contain a flavone, stereolensin. The bark gave an iridoid glycoside; the root bark gave n-triacontanol and beta- sitosterol; the root heartwood gave la- pachol, dehydro-alpha-lapachone and dehydrotectol. Ceryl alcohol, palmitic, stearic and oleic acids were isolated from the root. Lapachol exhibited cy- totoxic activity.

Tragopogon Porrifolius

Family: Compositae; Asteraceae.

Action: Root—specific in obstructions of the gall in jaundice; antibilious. Also used for treating arteriosclerosis and high blood pressure.

T. porrifolius contain several flavo- noids, including orientin and iso- orientin.

The seeds yield an oil which contains a saturated epoxy acid viz. cis-9, 10-epoxysteric acid and several conjugated dienoic acids.

Trianthema Portulacastrum

Linn.

Family: Aizoaceae.

Habitat: South India, Gujarat, Rajasthan, Uttar Pradesh and Haryana.

Ayurvedic: Varshaabhu (related species).

Siddha/Tamil: Vellai Sharunnai.

Folk: Bisakhaparaa.

Action: Root—deobstruent; used for asthma, hepatitis and amenorrhoea. The root, leaf and stem gave positive test for alkaloid. The plant is a good source of zinc (1.077 ± 0.188) and copper (0.416 ± 0.057) mg/100 g.

Synonym: T. monogyna Linn.

Family: Aizoaceae.

Habitat: Cultivated fields and wastelands.

English: Horse Purslane.

Ayurvedic: Varshaabhu, Vrshchira, Vishakharparikaa, Shilaatikaa, Shothaghni, Kshdra. Wrongly equated with Shveta-punarnavaa or Rakta-punarnavaa. Varshaabhu and Punarnavaa are two different herbs. T. portulacastrum is a rainy season annual. Rakta-punarnavaa is equated with Boerhavia diffusa, Shveta-punarnavaa with white- flowered species, B. erecta L. (B. punarnava).

Unani: Biskhaparaa.

Siddha: Sharunai.

Folk: Pathari, Bisakhaparaa.

Action: Root—antipyretic, analgesic, spasmolytic, deobstruent, cathartic, anti-inflammatory.

Leaves—diuretic; used in oedema and dropsy. A decoction of the herb is used as an antidote to alcoholic poison.

The Ayurvedic Pharmacopoeia ofIn- dia recommends the dried root in diseases of the liver and spleen, anaemia and oedema.

Ethanolic extract of the aerial parts exhibited hepatoprotective activity in CCl4-induced intoxication in rats. The acetone-insoluble fraction of the extract is responsible for the activity.

The red and white flowers contain an alkaloid trianthemine, also punaranavine. The plant also gave ecdysterone (0.01 g/kg), a potential chemosterilant; nicotinic acid and ascorbic acid. The plant is rich in phosphorus and iron but poor in calcium.

The high content of oxalate affects the assimilation of calcium. Carotene (2.3 mg/100 g) has also been reported.

Trianthema sp. are used as adulterant of the roots of Boerhavia diffusa.

Agranulocytosis

A condition in which there is a marked decrease or absence of polynuclear or granular white cells in the blood following prolonged septic conditions or from taking such drugs as phenothiazines, thiouracil and chloramphenicol. Other causes: radiation therapy, leukaemia, aplastic anaemia.

Symptoms: sore mouth, throat and lungs with bacterial infection.

Treatment: Should be supportive to specific medical treatment. Stop drugs that may be causative. Select from:–

Anaemia: Iron Deficiency

An estimated 15 per cent of the female population suffers from this form of anaemia. A deficiency of nutritional iron is responsible for oxygen starvation of the blood due to insufficient haemoglobin. Number of red cells is reduced.

Aetiology: heavy menstrual loss, feeble constitution from hereditary weakness, poor diet, hidden or known blood loss from gastric ulcer, pregnancy, bleeding piles or insufficient food minerals: iron, copper, calcium, etc, chronic liver or kidney disease, worms, anorexia nervosa, rheumatoid arthritis, tuberculosis. Symptoms. Tiredness, dizziness, breathlessness, palpitations, pale face and mucous membranes. White of eyes may be blue. Enlarged flabby tongue often bears impression of teeth marks. Hair lifeless, fingernails brittle and ridged. There may be angina, tinnitus and general reduced efficiency.

Treatment. The object is to achieve absorption of iron to raise normal haemoglobin levels and increase red cells. Echinacea has a reputation for regeneration of red cells. Herbs used with success: Echinacea, Gentian, Motherwort, Mugwort, Barberry, Hops, Nettles, Saw Palmetto, Chaparral, Red Clover, Dandelion.

Bitter herbs stimulate absorption of vital nutrients from the stomach, toning liver and pancreas, increasing the appetite; usually given half hour before meals. See: BITTERS.

Gentian. 1 teaspoon fine-cut chips to 2 cups cold water steeped 8 hours (overnight). Dose: Half-1 cup thrice daily before meals.

Coronary Heart Disease

The cause of: coronary occlusion, coronary blockage, coronary thrombosis. A heart attack occurs when a coronary artery becomes blocked by swellings composed, among other things, of cholesterol. Such swellings may obstruct the flow of blood leading to a blood clot (thrombus). Cholesterol is a major cause of CHD.

Coronary thrombosis is more common in the West because of its preference for animal fats; whereas in the East fats usually take the form of vegetable oils – corn, sunflower seed, sesame, etc. Fatty deposits (atheroma) form in the wall of the coronary artery, obstructing blood-flow. Vessels narrowed by atheroma and by contact with calcium and other salts become hard and brittle (arterio-sclerosis) and are easily blocked. Robbed of oxygen and nutrients heart muscle dies and is replaced by inelastic fibrous (scar) tissue which robs the heart of its maximum performance.

Severe pain and collapse follow a blockage. Where only a small branch of the coronary arterial tree is affected recovery is possible. Cause of the pain is lack of oxygen (Vitamin E). Incidence is highest among women over 40 who smoke excessively and who take The Pill.

Down’s Syndrome

Mongolism. Trisomy 21. Not a disease but a defect in mental and physical development. In the normal human being there are 46 chromosomes; in Down’s there are 47 – one extra No 21 chromosome. The syndrome increases with the age of the mother after the age of 35. Over the age of 40 the chances of a mother having such a child are 1-2 per cent. Children with the defect have low levels of zinc.

Cases of Down’s have followed use of nonoxynol-9 (vaginal contraceptive device) such as the polyurethane sponge. The sponge. when left in situ for a long time, may cause Down’s to follow.

Certain physical characteristics are present. The most important feature is impaired mental development. Almost all are coeliacs.

Symptoms. Low IQ, short fingers, small flat head, flattened nose, low-set ears. May be subject to umbilical hernia, and heart disease. No treatment can cure, but certain herb teas rich in minerals (Alfalfa, Red Clover) together with Kelp (either in tablet or powder form) may help children, with possible improvements in IQ. Gotu Kola

Hydrocotyle asiatica, Indian Pennywort, European Water-marvel. Centella asiatica L. French: Hydrocotyle. German: Wassernabel. Italian: Idrocotile. Indian: Brahami. Chinese: Chi-hsueh- ts’ao.

Constituents. flavonoids, terpenoids, volatile oil.

Action. Adaptogen, alterative, de-toxifier, bitter, diuretic, digestant, powerful blood tonic, central nervous system relaxant, laxative, emmenagogue, Ginseng-like effect, antibiotic (ointment and dusting powder).

Uses: Mentioned in most Eastern religions and medical systems. Has a reputation for longevity. Under the name Fo-ti-tieng it was prescribed and taken by Professor Li-Ching-Yun, Chinese herbalist who died 1933 at the reputed age of 256. (Guinness Book of Records) The herb is active in Ayurvedic Medicine, having a long history for leprosy and tumour. Prominent as a mild analgesic to alleviate pains of the female generative organs, for mental illness. Some success has been reported for cancer of the cervix. In Chinese medicine it covers a wide range including infertility, insomnia, crumbling nails, impaired vision, chronic sinusitis, sexual debility and some venereal diseases (juice of the fresh leaves).

It is a medicine of some versatility. In the West it has been used for recovery from surgical operation, drug withdrawal. Addison’s disease (copper-coloured complexion), rheumatism. For skin disorders: discharging ulcers, acne, pemphigus and lupus (where not ulcerative). It is said to heal without a scar. Of value for tiredness, depression, loss of memory, and to improve the nervous system generally in Parkinson’s disease.

Recent research reports improved memory and the overcoming of stress, fatigue and mental confusion. Preparations. Average dose: half-1g. Thrice daily. Tea. Quarter to half a teaspoon to each cup boiling water; infuse 10 minutes; dose – 1 cup.

Liquid extract: 1:1. Dose – 2-4ml (half-1 teaspoon).

Bengal tincture. 1 part coarsely powdered dried plant in 5 parts by weight of strong alcohol. Macerate 8 days in well-corked bottle in a dark place; shake daily; strain; filter. Dose – 1-2 teaspoons in water.

Hair Care

Hair, like nails and skin, is a protein material built up on amino acids. It is rich in minerals, especially sulphur. A sebaceous gland at the base of the hair follicle secretes sebum, an oily substance, which acts as a lubricant. When vital minerals and vitamins are lacking in the blood the quality of fibre and sebum deteriorates resulting in lustreless hair and change of texture. Healthy hair depends upon good personal hygiene, brushing, and washing with gentle-acting materials instead of harsh detergent shampoos which remove natural oils from the scalp and spoils its condition.

An adequate daily intake of essential fatty acids is assured by the golden oils (Sunflower, Corn, etc) which can be well supported by Evening Primrose oil capsules.

Internal: Bamboo gum. Nettle tea, Alfalfa, Horsetail, Soya.

Topical. Shampoo. Soapwort or Yucca. Chop 2 tablespoons (dry) or 1 tablespoon (fresh) leaves or root. Place in cup of warm water. Stir until a froth is produced. Decant and massage liquor into scalp.

Aloe Vera gel is noted for its moisturising effect and to provide nutrients. It may be used as a shampoo, hair set and conditioner. Jojoba oil has been used for centuries by the Mexican Indians for a healthy scalp; today, it is combined with Evening Primrose and Vitamin E with good effect. Olive oil stimulates strong growth.

One of several herbs may be used as a rinse, including Nettles, Rosemary, Southernwood, Fennel, Chamomile, Yellow Dock and Quassia. Hair should be washed not more than once weekly with warm water and simple vegetable soap; rinse four times with warm rinse, finishing off with cold. Brunettes should add a little vinegar; blondes, lemon juice. Selenium once had a reputation as a hair conditioner; recent research confirms. Selenium shampoos are available.

Heavy Metal Toxicity

Pollution of the blood and tissues by environmental poisons and traces of chemicals is a source of chronic disease. The most common toxic metals are lead, aluminium, cadmium, mercury and arsenic in that order. Copper is also toxic but is essential in small amounts.

Lead disrupts neurotransmitters in the brain and disposes to nervous excitability, aggression and hyperactivity. Aluminium is associated with senile dementia and Alzheimer’s disease, accumulating in the brain. Cadmium induces changes in behaviour with reduced mental ability. Mercury is present in the amalgam used in dental surgery as part-filling for teeth. Arsenical poisoning may occur in food contamination or paints.

An internal chelating or cleansing of tissues of the lungs, urinary system, blood and lymph may be assisted by a combination of relative expectorants, diuretics, hepatics and adaptogens among which are: Barberry, Blue Flag root, Chaparral, Burdock, Echinacea, Red Clover, Yellow Dock. Kwashiorkor

A disease of poor nutrition following a diet lacking adequate protein. Children 1-5 years vulnerable when fed too much starch, sugar and milk. Growth is retarded, hair scanty and skin unhealthy.

Symptoms. Feeble appetite, irritable bowel, oedema, nervous irritability.

Leukaemia

Greek word ‘white blood’. (Leukosis) Acute myeloid and lymphoblastic. Cancer of the white blood cells of two main types; myeloid, involving the polymorph type and lymphatic involving lymphocytes. Each type may take acute or chronic form, the acute being more serious. The disease is not an infection.

Causes: exposure to chemicals, X-rays or radioactive material. Genetic factors are believed to predispose. The condition may be acute or chronic and may follow chemotherapy.

Remissions are known to have been induced by a preparation from the Periwinkle plant (Vinca rosea) now re-classified as Catharanthus roseus.

Acute Leukaemia. Rapid onset with fatality within weeks or months. Fever. Proliferation of white cells in the bone marrow which are released and blood-borne to the liver, spleen and lymphatics. There may be bleeding from kidneys, mouth, bowel and beneath the skin. (Shepherd’s Purse, Yarrow) The acute form is known also as acute lymphoblastic or acute myeloblastic leukaemia. May be mis-diagnosed as tuberculosis.

Chronic Leukaemia. Gradual onset. Breathlessness from enlargement of the spleen. Swelling of glands under arms, in neck and groin. Loss of weight, appetite, strength, facial colour and body heat. Anaemia, spontaneous bleeding and a variety of skin conditions. Diarrhoea. Low grade fever.

No cure is known, but encouraging results in orthodox medicine promise the disease may be controlled, after the manner of diabetes by insulin. Successful results in such control are reported by Dr Hartwell, National Cancer Institute, Maryland, USA, with an alkaloid related to Autumn Primrose (Colchicum officinale). Vinchristine, a preparation from Periwinkle is now well-established in routine treatment. Red Clover, also, is cytotoxic to many mammalian cells. Vitamin C (present in many herbs and fruits) inhibits growth of non-lymphoblastic leukaemia cells. Good responses have been observed by Dr Ferenczi, Hungary, by the use of raw beet root juice.

Also treated with success by Dr Hartland (above) has been lymphocytic leukaemia in children which he treated with a preparation from Periwinkle.

Choice of agents depends largely upon the clinical experience of the practitioner and ease of administration. Addition of a nerve restorative (Oats, Kola, Black Cohosh or Helonias) may improve sense of well-being. To support the heart and circulatory system with cardiotonics (Hawthorn, Motherwort, Lily of the Valley) suggests sound therapy.

Herbal treatment may favourably influence haemoglobin levels and possibly arrest proliferation of leukaemic cells and reduce size of the spleen. It would be directed towards the (a) lymphatic system (Poke root), (b) spleen (Tamarinds), (c) bone marrow (Yellow Dock), and (d) liver (Blue Flag root).

An older generation of herbalists prescribed Blue Flag root, Yellow Dock, Poke root, Thuja and Echinacea, adding other agents according to indications of the particular case.

Tea. Formula. Equal parts: Red Clover, Gotu Kola, Plantain. 1-2 teaspoons to each cup boiling water; infuse 10-15 minutes. 1 cup thrice daily.

New Jersey tea (ceanothus). 1 teaspoon to each cup boiling water. Half-1 cup thrice daily.

Periwinkle tea (Vinca rosea). 2 teaspoons to each cup boiling water; infuse 15 minutes. 1 cup thrice daily.

Decoction. Formula. Equal parts: Echinacea, Yellow Dock, Blue Flag root. 1 teaspoon to each cup water gently simmered 20 minutes. 1 cup before meals thrice daily.

Vinchristine. Dosage as prescribed. In combination with other medicines.

Wheatgrass. Juice of fresh Wheatgrass grown as sprouts and passed through a juicer. Rich in minerals. One or more glasses daily.

Beetroot juice. Rich in minerals. Contains traces of rare rabidium and caesium, believed to contribute to anti-malignancy effect. (Studies by Dr A. Ferenczi, Nobel Prize-winner, published 1961)

Childhood Leukaemia. Research has linked the disease with fluorescent lighting. “Fluorescent tubes emit blue light (400mm wavelength). Light penetrates the skin and produces free radicals. Free radicals damage a child’s DNA. Damaged DNA causes leukaemia to develop. The type and intensity of lighting in maternity wards should be changed. Malabsorption Syndrome

Arising from poor assimilation of nutrients, minerals, fat soluble vitamins by the intestines. Patient not getting maximum nourishment from food.

Multiple causes: diseases of the gut; strictures, fistulas, Crohn’s disease, obstructions, parasites, infections, drugs, X-rays, endocrine disease, gastric surgery. A common cause is gluten sensitivity due to ingestion of gluten foods (wheat, oats, rye, barley).

Symptoms: Wasting of muscles, weight loss, flatulence, loss of appetite, distension, fat in the faeces, large pale frothy stools, vitamin and mineral deficiencies.

Nails

Nail loss. Paronychia. Clubbing. Brittle nails. Deformity (koilonychia). Spoon-shaped, as from iron-deficiency anaemia. A wide range of diseases affect the nails which, themselves, provide useful clues to underlying constitutional disturbance. Brittle and malformed nails are usually due to mineral deficiency. Ridging and grooving show altered nutrition and damage to the nail bed. Psoriasis nails are pitted.

Infections include candidiasis (monilia), ringworm, staphylococcal or streptococcal bacteria. Biting of fingernails reveals anxiety. Colour change and atrophy of nails may be caused by antibiotics, antimalarials, betablockers, gold and arsenic medicines, steroids, “The Pill”; requiring Eliminatives, liver, kidney and possibly Lymphatic agents.

Iud

An abbreviation for intrauterine contraceptive device. An IUD, which is also known as an IUCD or coil, is a mechanical device that is inserted into the uterus for purposes of contraception. Most IUDs are plastic devices with either copper or silver incorporated to improve their effectiveness. One type of IUD releases small amounts of the progestogen hormone levonorgestrel and is sometimes known as an intrauterine system (IUS). IUDs are believed to inhibit the implantation of a fertilized egg in the wall of the uterus (see implantation, egg).

An IUD is inserted through the vagina and cervix into the uterine cavity. Once in position, an IUD provides immediate protection. Most IUDs have a plastic string attached to make removal easier and also to indicate its presence when in place. IUDs usually need to be replaced every 3–8 years.

Women who have been pregnant are less likely to have complications than women who have never been pregnant. For example, they may experience less pain on insertion and have lighter menstrual flow and lower expulsion rates. IUDs are usually not recommended for women with fibroids or an irregular uterine cavity. If menstrual flow is heavy, or there is a history or increased risk of pelvic inflammatory disease (PID), a progestogen IUD may be recommended.

Rarely, pregnancy can occur, although IUDs seldom cause problems and can be removed. Nonprogestogen IUDs increase the risk of PID, which can lead to permanent infertility. A rare complication of IUD use is perforation of the uterus, which most commonly occurs at the time of insertion.... iud

Zinc

A trace element that is essential for normal growth, the development of the reproductive organs, normal functioning of the prostate gland, healing of wounds, and the manufacture of proteins and nucleic acids in the body. Zinc also controls the activities of more than 100 enzymes and is involved in the functioning of the hormone insulin.

Particularly rich sources of zinc include lean meat, wholemeal breads, whole grain cereals, dried beans, and seafood.

Zinc deficiency is rare. Most cases occur in people who are generally malnourished. Deficiency may also be caused by any disorder that causes malabsorption; acrodermatitis enteropathica; or by increased zinc requirements due to cell damage (for example, as a result of a burn or in sickle cell anaemia). Symptoms of deficiency include impairment of taste and loss of appetite; there may also be hair loss and inflammation of the skin, mouth, tongue, and eyelids. In children, zinc deficiency impairs growth and delays sexual development.

Prolonged excessive intake of zinc may interfere with the intestinal absorption of iron and copper, leading to a deficiency of these minerals.

Iucd

(intrauterine contraceptive device) a plastic or metal coil, spiral, or other shape, about 25 mm long, that is inserted into the cavity of the uterus to prevent conception. Its exact mode of action is unknown but it is thought to interfere with implantation of the embryo. Early IUCDs (such as the Lippes loop) were made of plastic; later variants (such as the Gravigard) are covered with copper, which slowly dissolves and augments the contraceptive action. About one-third of women fitted with an IUCD find the side-effects (heavy menstrual bleeding or back pain) unacceptable, but most have no complaints. The unwanted pregnancy rate is about 2 per 100 woman-years. If pregnancy should occur there is normally no need to remove the device (it may, however, be shed spontaneously). Melaleuca cajeputi

FAMILY: Myrtaceae

SYNONYMS: M. minor, cajuput, white tea tree, white wood, swamp tea tree, punk tree, paperbark tree.

GENERAL DESCRIPTION: A tall evergreen tree up to 30 metres high, with thick pointed leaves and white flowers. The flexible trunk has a whitish spongy bark which flakes off easily. In Malaysia it is called ‘caju-puti’, meaning ‘white wood’, due to the colour of the timber.

DISTRIBUTION: It grows wild in Malaysia, Indonesia, the Philippines, Vietnam, Java, Australia and south eastern Asia.

OTHER SPECIES: Several other varieties of Melaleuca are used to produce cajeput oil, such as M. quinquenervia – see Botanical Classification section. Closely related to other members of the Melaleuca group, notably eucalyptus, clove, niaouli and tea tree.

HERBAL/FOLK TRADITION: Held in high regard in the East, it is used locally for colds, headaches, throat infections, toothache, sore and aching muscles, fever (cholera), rheumatism and various skin diseases. Only the oil is used in the Western herbal tradition, known for producing a sensation of warmth and quickening the pulse. It is used for chronic laryngitis and bronchitis, cystitis, rheumatism and to expel roundworm.

ACTIONS: Mildly analgesic, antimicrobial, antineuralgic, antispasmodic, antiseptic (pulmonary, urinary, intestinal), anthelmintic, diaphoretic, carminative, expectorant, febrifuge, insecticide, sudorific, tonic.

EXTRACTION: Essential oil by steam distillation from the fresh leaves and twigs.

CHARACTERISTICS: A pale yellowy-green, mobile liquid (the green tinge derives from traces of copper found in the tree), with a penetrating, camphoraceous-medicinal odour. Compared with eucalyptus oil, it has a slightly milder fruity body note.

PRINCIPAL CONSTITUENTS: Cineol (14–65 per cent depending on source), terpineol, terpinyl acetate, pinene, nerolidol and other traces.

SAFETY DATA: Non-toxic, non-sensitizing, may irritate the skin in high concentration.
