Chickenpox: From 3 Different Sources


A common, mild infectious disease (also called varicella) occurring in childhood and characterized by a rash and slight fever.

In adults, chickenpox is rare but usually more severe.

An attack gives lifelong immunity, but the virus remains dormant in nerves and may reappear later in life to cause herpes zoster (shingles).

The cause of chickenpox is the varicella-zoster virus, which is spread in airborne droplets.

A widespread rash develops 2–3 weeks after infection, consisting of clusters of small, red, itchy spots that become fluid-filled blisters within a few hours.

After several days the blisters dry out to form scabs.

Scratching the blisters can lead to secondary infection and scarring.

Paracetamol helps reduce fever and calamine lotion may be used to relieve itching.

In severe cases, aciclovir (an antiviral drug) may be prescribed.

Also known as varicella. An acute, contagious disease predominantly of children – although it may occur at any age – characterised by fever and an eruption on the skin. The name, chickenpox, is said to be derived from the resemblance of the eruption to boiled chickpeas.

Causes The disease occurs in epidemics affecting especially children under the age of ten years. It is due to the varicella zoster virus, and the condition is an extremely infectious one from child to child. Although an attack confers life-long immunity, the virus may lie dormant and manifest itself in adult life as HERPES ZOSTER or shingles.

Symptoms There is an incubation period of 14–21 days after infection, and then the child becomes feverish or has a slight shivering, or may feel more severely ill with vomiting and pains in the back and legs. Almost at the same time, an eruption consisting of red pimples which quickly change into vesicles ?lled with clear ?uid appears on the back and chest, sometimes about the forehead, and less frequently on the limbs. These vesicles appear over several days and during the second day may show a change of their contents to turbid, purulent ?uid. Within a day or two they burst, or, at all events, shrivel up and become covered with brownish crusts. The small crusts have all dried up and fallen o? in little more than a week and recovery is almost always complete.

Treatment The fever can be reduced with paracetamol and the itching soothed with CALAMINE lotion. If the child has an immune disorder, is suffering from a major complication such as pneumonia, or is very unwell, an antiviral drug (aciclovir) can be used. It is likely to be e?ective only at an early stage. A vaccine is available in many parts of the world but is not used in the UK; the argument against its use is that it may delay chickenpox until adult life when the disease tends to be much more severe.

n. a highly infectious disease caused by a *herpesvirus (the varicella-zoster virus) that is transmitted by airborne droplets. After an incubation period of 11–18 days a mild fever develops, followed after about 24 hours by an itchy rash of red pimples that soon change to vesicles. These usually start on the trunk or scalp and spread to the face and limbs; they crust over and resolve after about 12 days. Treatment is aimed at reducing the fever and controlling the itching (e.g. by the application of calamine lotion). Complications are rare but include secondary infection and occasionally *encephalitis. The patient is infectious from two days before the spots appear until they all scab over, usually five days after they first appeared. One attack usually confers life-long immunity, although the virus may reactivate at a later date and cause shingles (see herpes). In adult patients who are particularly vulnerable, e.g. those with AIDS or who are otherwise immunosuppressed, chickenpox can be a serious disease, which may be treated with *aciclovir. Herpes Zoster

Herpes zoster, or shingles, is a skin eruption of acute nature, closely related to CHICKENPOX and consisting in the appearance of small yellow vesicles, which spread over an area, dry up, and heal by scabbing. It receives its name from the Greek word for a ‘circingle’ or girdle, because it spreads in a zone-like manner along the intercostal nerves around half the chest. Herpes of the face also occurs, particularly on the brow and around the eye.

Causes Shingles is due to the same virus that causes chickenpox. This invades the ganglia of the nerves, particularly the spinal nerves of the chest and the ?fth cranial nerve which supplies the face. Despite being due to the same virus as chickenpox, it is rare for herpes zoster to occur as a result of contact with a case of chickenpox. On the other hand, it is not unusual for a patient with herpes zoster to infect a child with chickenpox. It is a disease of adults rather than children, and the older the person, the more likely he or she is to develop the disease. Thus in adults under 50, the incidence is around 2·5 per 1,000 people a year; between 50 and 60 it is around 5 per 1,000; whilst in octogenarians it is 10 per 1,000. Occasionally it may be associated with some serious underlying disease such as LEUKAEMIA, LYMPHADENOMA, or multiple myeloma (see MYELOMATOSIS).

Symptoms The ?rst symptoms are much like those of any feverish attack. The person feels unwell for some days, has a slight rise of temperature, and feels vague pain in the side or in various other parts. Often the area of skin to be affected feels hypersensitive (hyperaesthesia) as though something were rubbing on it. The pain ?nally settles at a point in the side, and, two or three days after the ?rst symptoms, the rash appears. Minute yellow blebs – or vesicles, as they are known – are seen on the skin of the back, of the side, or of the front of the chest, or simultaneously on all three, the points corresponding to the space between one pair of ribs right around. These blebs increase in number for some days, and spread until there is often a complete half-girdle around one side of the chest. The pain in this stage is severe, but it appears to vary a good deal with age, being slight in children and very severe in old people, in whom indeed herpes sometimes forms a serious illness. After one or two weeks, most of the vesicles have dried up and formed scabs. The pain may not pass o? when the eruption disappears, but may remain for weeks or even months – a condition known as post-herpetic NEURALGIA. Old people are prone to develop this condition.

Treatment ACICLOVIR or famciclovir can be given orally, and are e?ective if started in large doses early in the attack. Later, topical antibiotics may be required. Analgesics may be necessary if neuralgia is severe.... herpes zoster

Herpes

Any of a variety of conditions characterized by an eruption of small, usually painful, blisters on the skin. The term usually refers to an infection with the herpes simplex virus. Forms of this virus are responsible for cold sores and genital herpes (see herpes, genital), among other conditions.

A closely related virus, varicella–zoster, is responsible for 2 other conditions in which skin blisters are a feature: chickenpox and herpes zoster (shingles).... herpes

Virus

The term applied to a group of infective agents which are so small that they are able to pass through the pores of collodion ?lters. They are responsible for some of the most devastating diseases affecting humans: for example, INFLUENZA, POLIOMYELITIS, SMALLPOX and YELLOW FEVER. The virus of in?uenza measures 80 nanometres, whereas the STAPHYLOCOCCUS measures 1,000 nanometres (1 nanometre = one thousand-millionth of a metre).

A single virus particle, known as a virion, comprises an inner core of NUCLEIC ACID which is surrounded by one or two protective coverings (capsid) made of protein. Sometimes the capsid is enclosed by another layer called the viral envelope (also a protein structure). The envelope often disintegrates when the virus invades a cell. Viruses enter cells and then indulge in a complex and variable process of replication using some of the cells’ own structure. Viruses may stay in a host’s nucleus, being reactivated months or years later. There are more than a score of large families of viruses, from papoviruses, which cause WARTS, through HERPES viruses (cold sores, CHICKENPOX, SHINGLES) and orthomyxoviruses (in?uenza), to corona viruses (common cold) and retroviruses (AIDS/HIV). Viral diseases are more dif?cult to treat than those caused by bacteria: ANTIBIOTICS are ine?ective but INTERFERON, a group of natural substances, shows promise. IMMUNISATION is the most e?ective way of combating viral infections; smallpox, poliomyelitis, MUMPS, MEASLES and RUBELLA are examples of viral diseases which have been successfully combated. Rash

A group of spots or an area of red, inflamed skin. A rash is usually temporary and is only rarely a sign of a serious underlying problem. It may be accompanied by itching or fever.

Rashes are classified according to whether they are localized (affecting a small area of skin) or generalized (covering the entire body), and the type of spots. A bullous rash has large blisters, a vesicular rash has small blisters, and a pustular one has pus-filled blisters. A macular rash consists of spots level with the surrounding skin and discernible from it by a difference in colour or texture. Nodular and papular rashes are composed of small, raised bumps.

Rashes are the main sign of many infectious diseases (such as chickenpox), and are a feature of many skin disorders, such as eczema and psoriasis. They may also indicate an underlying medical problem, such as the rashes of scurvy or pellagra, which are caused by vitamin deficiency. The rashes of urticaria or contact dermatitis may be caused by an allergic reaction. Drug reactions, particularly to antibiotic drugs, are a common cause of rashes.

A diagnosis is based on the appearance and distribution of the rash, the presence of any accompanying symptoms, and the possibility of allergy (for example, to drugs). Any underlying cause is treated if possible. An itching rash may be relieved by a lotion, such as calamine, or an antihistamine drug.... rash

Aciclovir

Aciclovir is an antiviral drug that inhibits DNA synthesis in cells infected by HERPES VIRUSES, although it does not eradicate them. It is only e?ective if started at the onset of infection; uses include the systemic treatment of herpes simplex infections of the skin and mucous membranes (including genital herpes), as well as of varicella-zoster (chickenpox) pneumonia and encephalitis. It is also used topically in the eye. It is especially valuable for the treatment of herpes infections in those with IMMUNODEFICIENCY and may be required for the prevention of recurrence and for prophylaxis – indeed, it may be life-saving. Similar medications include famciclovir and valaciclovir.... aciclovir

Averrhoa Carambola

Linn.

Habitat: Native to Malaysia; cultivated throughout the warmer parts of India, especially in Kerala.

English: Carambola, Star Fruit, Chinese Gooseberry.

Ayurvedic: Karmaranga.

Unani: Khamraq, Karmal.

Siddha/Tamil: Tamarattai.

Folk: Kamarakh.

Action: Root—antidote in poisoning. Leaf and shoot—applied externally in ringworm, scabies, chickenpox. Flower—vermicidal. Fruit—laxative, antidysenteric, antiphlogistic, febrifuge, anti- inflammatory, antispasmodic (used in hepatic colic, bleeding piles). Seeds—galactogenic; in large doses act as an emmenagogue and cause abortion.

The fruits are a fairly good source of iron but deficient in calcium. They also contain oxalic acid and potassium oxalate. The presence of fluorine is also reported. A wide variation of vitamin-C content (0.3-23.0 mg/100 g) is recorded from different places in India. Sugar (3.19%) consists mainly of glucose (1.63%).... averrhoa carambola

Curcuma

The genus Curcuma belonging to the family Zingiberaceae comprises of a number of species which are medicinally very important. Among them, the most important species are described below.

1. C. amada Roxb.

Mango ginger is cultivated in Gujarat and found wild in parts of West Bengal, U. P, Karnataka and Tamil Nadu. It is a rhizomatous aromatic herb with a leafy tuft and 60-90cm in height. Leaves are long, petiolate, oblong-lanceolate, tapering at both ends, glabrous and green on both sides. Flowers are white or pale yellow, arranged in spikes in the centre of tuft of the leaves. Lip is semi -elliptic, yellow, 3-lobbed with the mid lobe emarginate. The rhizomes are useful in vitiated conditions of pitta, anorexia, dyspepsia, flatulence, colic, bruises, wounds, chronic ulcers, skin diseases, pruritus, fever, constipations, strangury, hiccough, cough, bronchitis, sprains, gout, halitosis, otalgia and inflammations (Warrier et al, 1994). The fresh root possesses the smell of green mango and hence the name mango ginger. The rhizomes are used externally in the form of paste as an application for bruises and skin diseases generally combined with other medicines. Tubers rubbed with the leaf- juice of Caesalpinia bonduc is given for worms (Nadkarni, 1982).

The essential oil contains -pinene, -and -curcumene, camphor, cuminyl alcohol, myristic acid and turmerone. Car-3-ene and cis-ocimene contribute the characteristic mango odour of the rhizome. Rhizome is CNS active, hypothermic and it shows potentiation of amphetamine toxicity. Tuber is trypsin inhibitor and is effective against Vibrio cholerae (Husain et al, 1992). The rhizomes are bitter, sweet sour, aromatic, cooling, appetiser, carminative, digestive, stomachic, demulcent, vulnerary, febrifuge, alexertic, aphrodisiac, laxative, diurectic, expectorant, antiinflammatory and antipyretic (Warrier et al, 1994).

Wild turmeric or Cochin turmeric or Yellow zeodoary is found wild throughout India and cultivated in Bengal and Kerala. It is a perennial tuberous herb with annulate, aromatic yellow rhizome which is internally orange-red in colour. Leaves are elliptic or lanceolate- oblong, caudate-acuminate, 30-60cm long, petioles as long or even longer, bracts ovate, recurved, more or less tinged with red or pink. Flowers are pink, lip yellow, obovate, deflexed, sub-entire or obscurely three lobed. Fruits are dehiscent, globose, 3-valved capsules. Rhizomes are used in combination with astringents and aromatics for bruises, sprains, hiccough, bronchitis, cough, leucoderma and skin eruptions (Warrier et al, 1994). The rhizomes have an agreeable fragrant smell and yield a yellow colouring matter like turmeric, and the fresh root has a camphoraceous odour. The dried rhizome is used as a carminative and aromatic adjunctant to other medicines (Nadkarni, 1998).

Essential oil contains -and - -curcumene, d-camphene and p-methoxy cinnamic acid. The colouring matter is curcumin. Numerous sesquiterpenoids of germacrone and guaiane skeletons have been identified recently. Rhizome has effect on respiration. It is spasmolytic and shows antagonism of amphetamine hyperactivity. Rhizome is an anti-dote for snakebite and carminative (Husain et al, 1992).

Turmeric is cultivated all over India, particularly in W. Bengal, T. N and Maharashtra. It is a perennial herb, 60-90cm in height, with a short stem and tufts of erect leaves. Rhizome is cylindric, ovoid, orange coloured and branched. Leaves are simple, very large, petiole as long as the blade, oblong-lanceolate, tapering to the base upto 45cm long. Flowers are pale yellow, arranged in spikes concealed by the sheathing petioles and flowering bracts are pale green (Warrier et al, 1994). Turmeric occupies an important position in the life of Indian people as it forms an integral part of the rituals, ceremonies and cuisine. Due to the strong antiseptic properties, turmeric has been used as a remedy for all kinds of poisonous affections, ulcers and wounds. It gives good complexion to the skin and so it is applied to face as a depilatory and facial tonic. The drug cures diseases due to morbid vata, pitta and kapha, diabetes, eye diseases, ulcers, oedema, anaemia, anorexia, leprosy and scrofula. It purifies blood by destroying the pathogenic organisms. A paste of turmeric alone, or combined with a paste of neem (Azadirachta indica) leaves, is used to cure ringworm, obstinate itching, eczema and other parasitic skin diseases and in chicken pox and small pox. The drug is also useful in cold, cough, bronchitis, conjunctivitis and liver affections (Nadkarni, 1954; Kurup et al,1979; Kolammal, 1979). The rhizome is the officinal part and is an important ingredient of formulations like Nalpamaradi taila, Jatyadi taila, Narayana gula, etc. (Sivarajan et al, 1994).

Turmeric paste mixed with a little limejuice and saltpetre and applied hot is a popular application to sprains and bruises. In smallpox and chickenpox, a coating of turmeric is applied to facilitate the process of scabbing. The smoke produced by sprinkling powdered turmeric over burnt charcoal will relieve scorpion sting when the part affected is exposed to the smoke for a few minutes. Turmeric and alum powder in the proportion of 1:20 is blown into the ear in chronic otorrhoea (Nadkarni, 1998). “Haridra Khand”, a compound containing powdered turmeric, sugar and many other ingredients is a well-known preparation for cold, cough and flu, and for skin diseases. In Unani system, roasted turmeric is an ingredient of “Hab Narkachur”, used as antidysenteric for children (Thakur et al, 1989).

Essential oil contains ar-turmerone, and ar-curcumene as ma jor constituents. Some of the other compounds are -and -pinene, sabinene, myrcene, -terpinene, limonene, p- cymene, perillyl alcohol, turmerone, eugenol, iso-eugenol, eugenol methyl ether and iso- eugenol methyl ether. Curcumin and related compounds have also been reported as major constituents of the rhizomes. Recently a number of sesquiterpenes have been reported from C. longa, viz., the sesquiterpenoids of germacrane, bisabolane and guainane skeletons (Husain et al, 1992). The study of sesquiterpenes has revealed a new compound curlone (Kisoy et al, 1983). The crystalline colouring matter curcumin (0. 6%) is diferuloyl methane (Mathews et al, 1980). Stigmasterol, cholestrol, -sitosterol and fatty acids, mainly straight chain dienoic acids are reported (Moon et al, 1977). Curcumin, the colouring agent and major constituent of C. longa, is said to possess local as well as systemic antiinflammatory property which has been found to compare favourably with phenylbutazone (Srimal and Dhawan, 1973). An extract of the crude drug ‘akon’ containing the rhizomes exhibited intensive preventive activity against carbon tetrachloride induced liver injury invivo and invitro. The liver protecting effects of some analogs of ferulic acid and p-coumaric acid, probable metabolites of the curcuminoids have been also evaluated (Kiso et al, 1983). Curcumin is antiinflammatory. Rhizome is antiprotozoal, spasmolytic, CNS active, antiparasitic, antispasmodic, antibacterial, antiarthritic, anthelmintic, carminative, antiperiodic, emo llient, anodyne, laxative, diruretic, expectorant, alterative, alexertive, febrifuge, opthalmic and tonic.

4. C. zedoaria (Berg.) Rosc. syn. C. zerumbet Roxb; Amomum zedoaria

The round zedoary or Zerumbet is mostly found in India and S. E. Asia. The plant has 4-6 leaves with 20-60cm long lamina. The leaf lami na is oblong-lanceolate, finely acuminate and glabrous on both the surfaces. Flower stalk is 20-25cm long, emerging before the leaves. Flowers are yellow, while the flowering bract is green tinged with red. Calyx is 8mm long, corolla tube is twice as long as the calyx. Capsule is ovoid, trigonous, thin smooth and bursting irregularly. Tubers are palmately branched and camphoraceous (Thakur et al, 1989). The identity of the plant sources of the drug Karcura is a matter of debate. There is difference of opinion among men of Ayurveda, as to whether Sati and Karcura are the same drug or different. Many authors consider them different and equate Sati with Hedychium spicatum Smith. and Karcura with C. zedoaria, both belonging to Zingiberaceae (Kurup et al,1979; Chunekar 1982; Sharma, 1983). Some others treat them to be the same and equate it with C. zedoaria (Kirtikar and Basu, 1918; Vaidya, 1936; Nadkarni, 1954; Kapoor and Mitra, 1979). However, the source of Karcura in Kerala in the recent times has been Kaempferia galanga of the same family. The rhizome of C. zedoaria is used as appetiser and tonic, particularly prescribed to ladies after childbirth. In case of cold, a decoction of long pepper (Piper longum), cinnamon (Cinnamomum verum), zedoary and honey is given. In Ayurveda it is an ingredient of “Braticityadi kwatha”, used in high fever (Thakur et al, 1989). Root is useful in flatulence and dyspepsia, and as a corrector of purgatives. Fresh root checks leucorrhoeal and gonorrhoeal discharges. Root powder is a good substitute for many foreign foods for infants. For worms, the juice from the tubers is given to children. Juice of the leaves is given in dropsy (Nadkarni, 1982). It is an odoriferous ingredient of the cosmetics used for the cure of chronic skin diseases caused by impure or deranged blood (Nadkarni, 1998).

Essential oil from rhizomes contains -pinene, d-camphene, cineole, d-camphor, sesquiterpenes and sesquiterpene alcohols (Husain et al, 1992). The novel sesquiterpenoids which have been isolated and characterised are cuzerenone, epi-cuzerenone, iso- furanogermerene, curcumadiol, curcumol, curcumenol, iso-curcumenol, procurcumenol, dehydrocurdione (Hikino et al, 1968, 1971, 1972), germacrone-4, 5-epoxide, germacrone, germacrone furanodienone, curcumenol, iso-curcumenol, curcumanolides A and B and curcumenone (Shiobara et al, 1985). The starch left after the extraction is purified and sold as a commodity of cottage industry in West-Bengal under the name ‘Shoti’ (Rao et al, 1928). Ethyl-p methoxy-cinnamate has been isolated from the alcoholic extract of the plant (Gupta et al, 1976). Rhizome is stomachic, diuretic, and carminative and gastrointestinal stimulant.

Agrotechnology: Curcuma species are tropical herbs and can be grown on different types of soils both under irrigated and rainfed conditions. Rich loamy soils having good drainage are ideal for the crop. The plant is propagated by whole or split mother rhizomes. Well developed, healthy and disease free rhizomes are to be selected. Rhizomes are to be treated with copper oxychloride fungicides and stored in cool, dry place or earthen pits plastered with mud and cowdung. The best season of planting is during April with the receipt of pre-monsoon showers. The land is to be prepared to a fine tilth during February-March. On receipt of pre- monsoon showers in April, beds of size 3x1.2m with a spacing of 40cm between beds are to be prepared. Small pits are to be taken in the beds in rows with a spacing of 25-40cm.

Finger rhizomes are to be planted flat with buds facing upwards and covered with soil or dry powdered cattle ma nure. The crop is to be mulched immediately after planting and 50 days after first mulching. Cattle manure or compost is to be applied as basal dose at 20-40t/ha at the time of land preparation or by spreading over the beds after planting. Application of NPK fertilizers is beneficial and found to increase the yield considerably. Weeding is to be done twice at 60 and 120 days after planting, depending upon weed intensity. Earthing up is to be done after 60 days. No major incidence of pest or disease is noticed in this crop. Leaf blotch and leaf spot can be controlled by spraying Bordeaux mixture or 0.2% Mancozeb. Shoot borers can be controlled by spraying 0.05% Dimethoate or 0.025% Quinalphos. Time of harvest usually extends from January-March. Harvesting is generally done at about 7-10 months after planting depending upon the species and variety. Harvested rhizomes are to be cleaned of mud and other materials adhering to them. Good fingers separated are to be used for curing (KAU, 1996).... curcuma

Encephalitis

Encephalitis means in?ammation or infection of the brain, usually caused by a virus; it may also be the result of bacterial infection. It occurs throughout the world and affects all racial groups and ages. Rarely it occurs as a complication of common viral disease such as measles, mumps, glandular fever, or chickenpox. It may occur with no evidence of infection elsewhere, such as in HERPES SIMPLEX encephalitis, the most common form seen in Europe and America. RABIES is another form of viral encephalitis, and the HIV virus which causes AIDS invades the brain to cause another form of encephalitis (see AIDS/HIV). In some countries – North and South America, Japan and east Asia and Russia

– there may be epidemics spread by the bite of mosquitoes or ticks.

The clinical features begin with in?uenza-like symptoms – aches, temperature and wretchedness; then the patient develops a headache with drowsiness, confusion and neck sti?ness. Severely ill patients develop changes in behaviour, abnormalities of speech, and deterioration, sometimes with epileptic seizures. Some develop paralysis and memory loss. CT (see COMPUTED TOMOGRAPHY) and MRI brain scans show brain swelling, and damage to the temporal lobes if the herpes virus is involved. ELECTROENCEPHALOGRAPHY (EEG), which records the brainwaves, is abnormal. Diagnosis is possible by an examination of the blood or other body ?uids for antibody reaction to the virus, and modern laboratory techniques are very speci?c.

In general, drugs are not e?ective against viruses – antibiotics are of no use. Herpes encephalitis does respond to treatment with the antiviral agent, aciclovir. Treatment is supportive: patients should be given painkillers, and ?uid replacement drugs to reduce brain swelling and counter epilepsy if it occurs. Fortunately, most sufferers from encephalitis make a complete recovery, but some are left severely disabled with physical defects, personality and memory disturbance, and epileptic ?ts. Rabies is always fatal and the changes found in patients with AIDS are almost always progressive. Except in very speci?c circumstances, it is not possible to be immunised against encephalitis.

Encephalitis lethargica is one, now rare, variety that reached epidemic levels after World War I. It was characterised by drowsiness and headache leading on to COMA. The disease occasionally occurs as a complication after mumps and sometimes affected individuals subsequently develop postencephalitic PARKINSONISM.... encephalitis

Herpes Viruses

One member of a group of viruses containing DNA which cause latent infections in animals and humans. Viruses from this group cause HERPES SIMPLEX, HERPES ZOSTER (shingles) and CHICKENPOX, and include CYTOMEGALOVIRUS (CCMV) and EPSTEIN-BARR VIRUS.... herpes viruses

Pimples

Technically known as papules, these are small, raised and in?amed areas on the SKIN. On the face, the most common cause is ACNE. BOILS (FURUNCULOSIS) start as hard pimples. The eruption of SMALLPOX and that of CHICKENPOX begin also with pimples. (See also SKIN, DISEASES OF.)... pimples

Shingles

Also called Herpes zoster. It is caused by the chickenpox virus, and usually occurs in middle-age, beginning as inflammation, sharp pain and finally vesicles, erupting at the edges of posterior ganglia of the trunk or face. Usually lasting two or three weeks, it is often triggered by stress or a concurrent viral infection, and can return again in some individuals.... shingles

Blister

A collection of fluid beneath the outer layer of the skin that forms a raised area. A blister contains fluid that has leaked from blood vessels in underlying skin layers after minor damage and protects the damaged tissue. Common causes are burns and friction. Blisters may also occur with pemphigus, pemphigoid, dermatitis herpetiformis, some types of porphyria, and some skin diseases. These include eczema, epidermolysis bullosa, impetigo, and erythema multiforme. Small blisters develop in the viral infections chickenpox, herpes zoster (shingles), and herpes simplex. Generally, blisters are best left intact, but large or unexplained blisters need medical attention.... blister

Eruption

Eruption, or rash, means an outbreak, in a scattered form, upon the surface of the skin. The skin is usually raised and red, or it may be covered with scales, or crusts, or vesicles containing ?uid. Eruptions di?er in appearance: for example, the eruption of MEASLES is always distinguishable from that of CHICKENPOX. But the same disease may also produce di?erent eruptions in di?erent people; or in the same person in di?erent states of health; or even on di?erent parts of the body at one time.

Eruptions may be acute or chronic. Most of the acute eruptions belong to the exanthemata (see EXANTHEM): that is, they are bright in col-our and burst out suddenly like a ?ower. These are the eruptions of SCARLET FEVER, measles, German measles (see RUBELLA), SMALLPOX and chickenpox. In general, the severity of these diseases can be measured by the amount of eruption. Some eruptions are very transitory, like nettle-rash, appearing and vanishing again in the course of a few hours. (See also SKIN, DISEASES OF.)... eruption

Reye’s Syndrome

A condition, now rare, which occurs predominantly in young children following a viral infection of the upper respiratory tract or a viral infection such as CHICKENPOX or INFLUENZA.

The cause is not known, but there is evidence that ASPIRIN may also play a part in its causation. Doctors recommend that children should be given PARACETAMOL in place of aspirin. The initial feature is severe, persistent vomiting and fever. This is followed by outbursts of wild behaviour, DELIRIUM and CONVULSIONS terminating in COMA and death, often from liver failure. The MORTALITY rate is around 23 per cent, and 50 per cent of the survivors may have persistent mental or neurological disturbances. The younger the patient, the higher the death rate and the more common the permanent residual effects. Since aspirin has no longer been licensed for use in children and young people the incidence of the condition has fallen dramatcally. Some cases, previously thought to be Reye’s syndrome, have subsequently turned out to have been due to certain inherited metabolic diseases and to be unconnected with aspirin.... reye’s syndrome

Contagious

A term used to describe a disease that can be transferred from person to person by ordinary social contact. All contagious diseases, such as the common cold or chickenpox, are infectious. The term contagious does not apply to the many infectious diseases, such as typhoid, syphilis, or , which are spread by other means.... contagious

Immunoglobulin Injection

Administration of immunoglobulin preparations

(antibodies) to prevent or treat infectious diseases. Such preparations, also known as immune globulin or gammaglobulin, work by passing on antibodies obtained from the blood of people who have previously been exposed to these diseases. The main use of these injections is to prevent infectious diseases, such as chickenpox, in people exposed to infection who are not already immune or are at special risk (during cancer treatment, for example). They are also given regularly for immunodeficiency disorders. Side effects include rash, fever, and pain and tenderness at the injection site.... immunoglobulin injection

Incubation Period

The time during which an infectious disease develops, from the point when the infecting organism enters the body until symptoms appear.

Different infections have characteristic incubation periods; for example, 14–21 days for chickenpox and 7–14 days for measles.

Salmalia Malabarica

(DC) Schott & Endl.

Synonym: Bombax ceiba Linn. Bombax malabaricum DC. Gossampinus malabarica (DC.) Merr.

Family: Bombacaceae.

Habitat: The hotter parts of India, up to 1,350 m.

Ayurvedic: Shaalmali, Mochaa, Mochaahva, Pichhila, Raktapushpa, Sthiraayu, Kankataadhya, Tuulini. Shaalmali-veshtaka (gum).

Unani: Semal. Mochras (gum).

Siddha: Mul Ilavam. Ielavampisin (gum).

Folk: Semar.

Action: Young roots (Semul- musali)—astringent, (used for dysentery) stimulant, demulcent. Fruits—stimulant, diuretic, expectorant. Used for chronic inflammation of bladder, kidney also for calculus affections. Flowers— astringent and cooling, applied to cutaneous affections. Leaves— anti-inflammatory. Stem bark— demulcent, styptic. Aqueous extract with curd is given for blood- dysentery. Bark—paste is applied to skin eruptions, boils, acne, pimples. Seeds used for chickenpox, smallpox, catarrhal affections, chronic cystitis and genitourinary diseases. Gum—astringent, demulcent, styptic. Used for diarrhoea, dysentery, haemoptysis, bleeding piles, menor- rhagia, spermatorrhoea. Root and pod—used for the treatment of low vitality and debility.

The Ayurvedic Pharmacopoeia ofIn- dia recommends the stem bark in bleeding disorders and in acne vul- garis.

All parts of the plant gave beta- sitosterol and its glucosides; seeds, bark and root bark, lupeol; flowers, hentri- acontane, hentriacontanol; root bark, in addition, gave 7-hydroxycadalene. The seed oil yields arachidic, linoleic, myristic, oleic and palmitic acids; seeds contain carotenes, n-hexacosanol, et- hylgallate and tocopherols; the gum contains gallic and tannic acids, yields L-arbinose, D-galactose, D-galacturo- nic acid and D-galactopyranose.

Younger roots contain more sugars (arabinose and galactose 8.2%) and peptic substances (6.0%) than the older ones. They contain mucilage, starch (71.2%), mineral matter (2.1%), tannins 0.4 and non-tannins 0.1%, along with other constituents.

The Musali compares favourably with the nutritive value of Pueraria tuberosa, Dioscorea bulbifera, Ipomoea digitata and Butea monosperma (all used in sexual debility).

A related species, Salmalia insig- nis (Wall.) Schott & Endl., synonym Bombax insigne Wall. (Assam, Western Ghats and the Andamans); is known as Semul; Dumboil (Assam), Didu (Andamans) and Kal-ilavu (Tamil Nadu).

Yellow Fever Vaccine Is Prepared From

chick embryos injected with the living, attenuated strain (17D) of pantropic virus. Only one injection is required, and immunity persists for many years. Re-inoculation, however, is desirable every ten years. (See YELLOW FEVER.)

Haemophilus vaccine (HiB) This vaccine was introduced in the UK in 1994 to deal with the annual incidence of about 1,500 cases and 100 deaths from haemophilus MENINGITIS, SEPTICAEMIA and EPIGLOTTITIS, mostly in pre-school children. It has been remarkably successful when given as part of the primary vaccination programme at two, three and four months of age – reducing the incidence by over 95 per cent. A few cases still occur, either due to other subgroups of the organism for which the vaccine is not designed, or because of inadequate response by the child, possibly related to interference from the newer forms of pertussis vaccine (see above) given at the same time.

Meningococcal C vaccine Used in the UK from 1998, this has dramatically reduced the incidence of meningitis and septicaemia due to this organism. Used as part of the primary programme in early infancy, it does not protect against other types of meningococci.

Varicella vaccine This vaccine, used to protect against varicella (CHICKENPOX) is used in a number of countries including the United States and Japan. It has not been introduced into the UK, largely because of concerns that use in infancy would result in an upsurge in cases in adult life, when the disease may be more severe.

Pneumococcal vaccine The pneumococcus is responsible for severe and sometimes fatal childhood diseases including meningitis and septicaemia, as well as PNEUMONIA and other respiratory infections. Vaccines are available but do not protect against all strains and are reserved for special situations – such as for patients without a SPLEEN or those who are immunode?cient.... yellow fever vaccine is prepared from

Sore Throat

A rough or raw feeling in the back of the throat that causes discomfort, especially when swallowing.

Sore throat is a common symptom, usually caused by pharyngitis and occasionally by tonsillitis. It is often the first symptom of the common cold, influenza, laryngitis, infectious mononucleosis, and many childhood viral illnesses, such as chickenpox, measles, and mumps.

A sore throat may be relieved by gargling with salt water. Sore throats due to bacterial infection are treated with antibiotic drugs. (See also strep throat.)... sore throat

Encephalomyelitis

Inflammation of the brain and spinal cord, causing damage to the nervous system. Encephalomyelitis develops as a rare complication of measles or, less commonly, of other viral infections such as chickenpox, rubella, or infectious mononucleosis. Symptoms include fever, headache, drowsiness, seizures, partial paralysis or loss of sensation, and, in some cases, coma.

Diagnosis is as for encephalitis. There is no cure, but corticosteroid drugs are given to reduce inflammation and anticonvulsant drugs to control seizures. The disease is often fatal; those who survive may have permanent damage to the nervous system.

Myalgic encephalomyelitis is another term for chronic fatigue syndrome.... encephalomyelitis

Itching

An intense irritation or tickling sensation in the skin. Generalized itching may result from excessive bathing, which removes the skin’s natural oils and may leave the skin excessively dry. Some people experience general itching after taking certain drugs. Many elderly people suffer from dry, itchy skin, especially on their backs. Itching commonly occurs during pregnancy.

Many skin conditions, including chickenpox, urticaria (nettle rash), and eczema, produce an itchy rash. Generalized skin itchiness can be a result of diabetes mellitus, kidney failure, jaundice, and thyroid disorders.

Pruritus ani (itching around the anal region) occurs with haemorrhoids and anal fissure. Threadworm infestation is the most likely cause of anal itching in children. Pruritus vulvae (itching of the external genitalia in women) may be due to candidiasis, hormonal changes, or to use of spermicides or vaginal ointments and deodorants. Insect bites, lice, and scabies infestations cause intense itching.

Specific treatment for itching depends on the underlying cause. Cooling lotions, such as calamine, relieve irritation; emollients reduce dryness.... itching

Viruses

The smallest known types of infectious agent. It is debatable whether viruses are truly living organisms or just collections of molecules capable of selfreplication under specific conditions. Their sole activity is to invade the cells of other organisms, which they then take over to make copies of themselves. Outside living cells, viruses are inert.

A single virus particle (virion) consists of an inner core of nucleic acid, which may be either DNA or RNA, surrounded

by 1 or 2 protective protein shells (capsids). Surrounding the outer capsid may be another layer, the viral envelope, which consists mainly of protein. The nucleic acid consists of a string of genes that contain coded instructions for making copies of the virus.

Common viral diseases include the common cold, influenza, and chickenpox (caused by the varicella–zoster virus). AIDS is caused by the human immunodeficiency virus (HIV).... viruses

Herpesvirus

n. one of a group of DNA-containing viruses causing latent infections in animals (including humans). The herpesviruses are the causative agents of *herpes and chickenpox. The group also includes the *cytomegalovirus and *Epstein–Barr virus. Herpesvirus simiae (virus B) causes an infection in monkeys similar to herpes simplex, but when transmitted to humans it can produce fatal encephalitis.... herpesvirus

Pock

n. a small pus-filled eruption on the skin characteristic of *chickenpox and *smallpox rashes. See also pustule.... pock

Rickettsial Pox

a disease of mice caused by the bacterium Rickettsia akari and transmitted to humans by mites: it produces chills, fever, muscular pain, and a rash similar to that of *chickenpox. The disease is mild and runs its course in 2–3 weeks. See also typhus.... rickettsial pox

Eucalyptus, Blue Gum

Eucalyptus globulus var. globulus

FAMILY: Myrtaceae

SYNONYMS: Gum tree, southern blue gum, Tasmanian blue gum, fever tree, stringy bark.

GENERAL DESCRIPTION: A beautiful, tall, evergreen tree, up to 90 metres high. The young trees have bluish-green oval leaves while the mature trees develop long, narrow, yellowish leaves, creamy-white flowers and a smooth, pale grey bark often covered in a white powder.

DISTRIBUTION: Native to Tasmania and Australia. Mainly cultivated in Spain and Portugal, also Brazil, California, Russia and China. Very little of this oil now comes from its native countries.

OTHER SPECIES: There are over 700 different species of eucalyptus, of which at least 500 produce a type of essential oil. Many have been extracted simply for experimental purposes, and research is still being carried out with regard to the different constituents of each oil. In general, they can be divided into three categories. 1. The medicinal oils containing large amounts of cineol (or eucalyptol), such as the blue gum, but increasingly the blue malee (E. polybractea), the narrow-leaved peppermint (E. radiata var. australiana) and the gully gum (E. smithii). 2. The industrial oils containing mainly piperitone and phellandrene, such as the peppermint eucalyptus (E. piperita), grey peppermint (E. radiata var. phellandra) and increasingly the broad-leaved peppermint (E. dives var. Type). 3. The perfumery oils containing mainly citronellal, such as the lemon-scented eucalyptus (E. citriodora). See also Botanical Classification section.

HERBAL/FOLK TRADITION: A traditional household remedy in Australia, the leaves and oil are especially used for respiratory ailments such as bronchitis and croup, and the dried leaves are smoked like tobacco for asthma. It is also used for feverish conditions (malaria, typhoid, cholera, etc.) and skin problems like burns, ulcers and wounds. Aqueous extracts are used for aching joints, bacterial dysentery, ringworms, tuberculosis, etc. and employed for similar reasons in western and eastern medicine. The wood is also used for timber production in Spain.

ACTIONS: Analgesic, antineuralgic, antirheumatic, antiseptic, antispasmodic, antiviral, balsamic, cicatrisant, decongestant, deodorant, depurative, diuretic, expectorant, febrifuge, hypoglycaemic, parasiticide, prophylactic, rubefacient, stimulant, vermifuge, vulnerary.

EXTRACTION: Essential oil by steam distillation from the fresh or partially dried leaves and young twigs.

CHARACTERISTICS: A colourless mobile liquid (yellows on ageing), with a somewhat harsh camphoraceous odour and woody-sweet undertone. It blends well with thyme, rosemary, lavender, marjoram, pine, cedarwood and lemon. (The narrow-leaved eucalyptus (E. radiata var. australiana) is often used in preference to the blue gum in aromatherapy work, being rich in cineol but with a sweeter and less harsh odour.)

PRINCIPAL CONSTITUENTS: Cineol (70–85 per cent), pinene, limonene, cymene, phellandrene, terpinene, aromadendrene, among others.

SAFETY DATA: Externally non-toxic, non-irritant (in dilution), non-sensitizing. ‘When taken internally eucalyptus oil is toxic and as little as 3.5ml has been reported as fatal’..

AROMATHERAPY/HOME: USE

Skin Care: Burns, blisters, cuts, herpes, insect bites, insect repellent, lice, skin infections, wounds.

OTHER USES: The oil and cineol are largely employed in the preparation of liniments, inhalants, cough syrups, ointments, toothpaste and as pharmaceutical flavourings also used in veterinary practise and dentistry. Used as a fragrance component in soaps, detergents and toiletries – little used in perfumes. Lemon-scented

Eucalyptus citriodora

FAMILY: Myrtaceae

SYNONYMS: Lemon-scented gum, citron-scented gum, scented gum tree, spotted gum, ‘boabo’.

GENERAL DESCRIPTION: An attractive, tall, evergreen tree with a smooth dimpled bark, blotched in grey, cream and pink, cultivated as an ornamental. The trunk grows fast, straight and to considerable height, and is used for timber. The young leaves are oval, the mature leaves narrow and tapering.

DISTRIBUTION: Native to Australia; cultivated mainly in Brazil and China.

OTHER SPECIES: There are numerous other species of eucalyptus – see entry on eucalyptus blue gum. See also Botanical Classification section.

HERBAL/FOLK TRADITION: Used traditionally for perfuming the linen cupboard by enclosing the dried leaves in a small cloth sachet. During the last century it was regarded as a good insect repellent, especially for cockroaches and silverfish.

ACTIONS: Antiseptic, antiviral, bactericidal, deodorant, expectorant, fungicidal, insecticide.

EXTRACTION: Essential oil by steam distillation from the leaves and twigs.

CHARACTERISTICS: A colourless or pale yellow mobile liquid with a strong, fresh, citronella-like odour and sweet balsamic undertone.

PRINCIPAL CONSTITUENTS: Citronellal (80–95 per cent), citronellol, geraniol and pinene, among others. (The gum or ‘kino’ contains the antibiotic substance ‘citriodorol’.)

SAFETY DATA: Non-toxic, non-irritant, possible sensitization in some individuals. Eucalyptus oil is toxic when taken internally, see Eucalyptus blue gum entry.

AROMATHERAPY/HOME: USE

Skin Care: Athlete’s foot and other fungal infections (e.g. candida), cuts, dandruff, herpes, insect repellent, scabs, sores, wounds.

Respiratory System: Asthma, laryngitis, sore throat.

Immune System: Colds, fevers, infectious skin conditions such as chickenpox, infectious disease. ‘The essential oil contained in the leaves appears to have bacteriostatic activity towards Staphylococcus aureus; this is due to synergism between the citronellol and citronellal present in the oil’..

OTHER USES: Used as a fragrance component (in place of E. globulus) in soaps, detergents and perfumes; also used in room sprays and insect repellents Employed for the isolation of natural citronellal.... lemon-scented

Ravensara

Ravensara aromatica

FAMILY: Lauraceae

SYNONYMS: Madagascar clove-nutmeg, aromatic ravensare, Ravensara anisata, Agatophyllum aromaticum.

GENERAL DESCRIPTION: The genus Ravensara includes many species of evergreen trees or shrubs native to Madagascar and the Comoro Islands. These evergreen trees can grow up to 30 metres tall in rainforests, usually on all type of soils. The best-known species of this genus is R. aromatica used for its essential oil. It grows to a height of about 20 metres with several buttress roots at the base, with reddish bark, dark green shiny leaves and small yellow-green flowers. The fruit, a fleshy berry, are an important food source for birds. All parts of the tree are aromatic.

DISTRIBUTION: R. aromatica is native to Madagascar and once thrived in the rainforests, but is now an endangered species due to de-forestation and over-exploitation. By the early part of this century, this species required protection as the production of the essential oil from the bark, used by the pharmaceutical industry, was particularly destructive as it required felling the trees. Only the leaves can now be used for oil production, to prevent cutting down the whole tree. There is now a conservation programme in place to protect these trees: it is therefore important to obtain the essential oil of the leaf from sustainable sources.

OTHER SPECIES: There are over 30 species of Ravensara that grow in Madagascar. The common name of the genus Ravensara is actually a latinization of the Malagasy term ravintsara which means ‘good leaves’, a term originally used by the natives to describe another species of tree. European botanists in the nineteenth century made several attempts to classify the various indigenous aromatic trees of Madagascar, but due to the similarity of the two names ‘ravintsara’ and ‘ravensara’ they came to be used almost interchangeably. The botanical confusion between these two species has continued to cause a great deal of uncertainty as to their precise origins. In fact, the essential oils derived from these two species are very different aromatically as well as in their chemical composition. However, most of so-called ‘ravensara’ oil from Madagascar, is actually produced from the Cinnamomum camphora species (even if it is said to derive from R. aromatica) so great care needs to be taken in identifying the correct source of each oil. See also entry for Ravintsara.

HERBAL/FOLK TRADITION: In Madagascar, this genus of trees is commonly called hazomanitra, meaning ‘tree that smells’, as the whole part of the plant is aromatic. Traditionally, the Malagasy people used the bark and stem as a tonic and as an antibacterial medicine. The leaves were also burnt in homes after a death to prevent the spread of disease while the anise-flavoured bark was used in the production of local rum. The oil from the Ravensara genus is referred to locally as ‘the oil that heals’ because it is used for so many different conditions. The essential oil from the bark, R. anisata (a synonym for R. aromatica) is known as ‘havozo’ and has a strong anise-like odour.

ACTIONS: Antiviral, antibacterial, antifungal, anti-infectious, antiseptic, antispasmodic, expectorant, immune-stimulant.

EXTRACTION: 1. Ravensara oil is steam distilled from the leaves of the plant. 2. The essential oil distilled from the bark is called havozo. 3. An essential oil is also extracted occasionally from the fleshy fruits or berries.

CHARACTERISTICS: 1. Ravensara oil (from the leaf) has a sweet, fresh, slightly lemony, licorice-like scent with an earthy undertone. 2. Havozo (from the bark) has a fresh, strong aniseed-like odour.

PRINCIPAL CONSTITUENTS: 1. The oil from the leaf contains relatively small amounts of methyl chavicol (estragole), myrcene, a-pinene and linalool with the main constituents being limonene (approx. 13–22 per cent) and sabinene (approx. 10–16 per cent) with very little 1,8-cineole (1.8–3.3 per cent). 2. Havozo (bark oil) has a high methyl chavicol content (up to 90 per cent).

SAFETY DATA: 1. Ravensara (R. aromatica) is generally considered a safe oil (non-toxic and non-irritant) although it is best avoided during pregnancy. 2. Havozo oil is not recommended for use in aromatherapy due to its high methyl chavicol content, as it is a suspected carcinogen. Research has shown, nevertheless this oil has potent anti-microbial properties, being active against E. coli.

AROMATHERAPY/HOME: USE

Skin care: Chickenpox, cold sores, herpes (all types), shingles.

Circulation muscles and joints: Aching limbs, arthritis, cramp, gout, muscle fatigue, rheumatism.

Respiratory system: Bronchitis, colds, congestion, influenza, sinusitis, throat and lung infections, whooping cough.

Immune system: Viral infections especially herpes, hepatitis, chicken pox.

Nervous system: Anxiety, insomnia, nervous tension, stress.

OTHER USES: Ravensara species have been used for a long time to produce essential oils for the pharmaceuticals industry. In perfumery usage, it is employed as a middle note.... ravensara

Tea Tree

Melaleuca alternifolia

FAMILY: Myrtaceae

SYNONYMS: Narrow-leaved paperbark tea tree, ti-tree, ti-trol, melasol.

GENERAL DESCRIPTION: A small tree or shrub (smallest of the tea tree family), with needle-like leaves similar to cypress, with heads of sessile yellow or purplish flowers.

DISTRIBUTION: Native to Australia. Other varieties have been cultivated elsewhere, but M. alternifolia is not produced outside Australia, mainly in New South Wales.

OTHER SPECIES: Tea tree is a general name for members of the Melaleuca family which exists in many physiological forms including cajeput (M. cajeputi) and niaouli (M. viridiflora), and many others such as M. bracteata and M. linariifolia – see Botanical Classification section.

HERBAL/FOLK TRADITION: The name derives from its local usage as a type of herbal tea, prepared from the leaves. Our present knowledge of the properties and uses of tea tree is based on a very long history of use by the aboriginal people of Australia. It has been extensively researched recently by scientific methods with the following results: ‘1. This oil is unusual in that it is active against all three varieties of infectious organisms: bacteria, fungi and viruses. 2. It is a very powerful immuno-stimulant, so when the body is threatened by any of these organisms ti-tree increases its ability to respond.’ .

ACTIONS: Anti-infectious, anti-inflammatory, antiseptic, antiviral, bactericidal, balsamic, cicatrisant, diaphoretic, expectorant, fungicidal, immuno-stimulant, parasiticide, vulnerary.

EXTRACTION: Essential oil by steam or water distillation from the leaves and twigs.

CHARACTERISTICS: A pale yellowy-green or water-white mobile liquid with a warm, fresh, spicy-camphoraceous odour. It blends well with lavandin, lavender, clary sage, rosemary, oakmoss, pine, cananga, geranium, marjoram, and spice oils, especially clove and nutmeg.

PRINCIPAL CONSTITUENTS: Terpinene-4-ol (up to 30 per cent), cineol, pinene, terpinenes, cymene, sesquiterpenes, sesquiterpene alcohols, among others.

SAFETY DATA: Non-toxic, non-irritant, possible sensitization in some individuals.

Skin care: Abscess, acne, athlete’s foot, blisters, burns, cold sores, dandruff, herpes, insect bites, oily skin, rashes (nappy rash), spots, verrucae, warts, wounds (infected).
