n. a genus of parasitic nematode worms. A. lumbricoides, widely distributed throughout the world, is the largest of the human intestinal nematodes – an adult female measures up to 35 cm in length. Eggs, passed out in the stools, may be transmitted to a new host in contaminated food or drink. Larvae hatch out in the intestine and then undergo a complicated migration, via the hepatic portal vein, liver, heart, lungs, windpipe, and pharynx, before returning to the intestine where they later develop into adult worms (see also ascariasis).

Ascariasis

Ascariasis is the disease produced by infestation with the roundworm Ascaris lumbricoides, also known as the maw-worm. Super?cially it resembles a large earthworm: the male measures about 17 cm (7 inches) and the female 23 cm (9 inches) in length. Ascariasis is a dirt disease, most prevalent where sanitation and cleanliness are lacking, particularly in the tropics and subtropics. Consumption of food contaminated by the ova (eggs), especially salad vegetables, is the commonest cause of infection. In children, infection is commonly acquired by crawling or playing on contaminated earth, and then sucking their ?ngers. After a complicated life-cycle in the body the adult worms end up in the intestines, whence they may be passed in the stools. A light infection may cause no symptoms. A heavy infection may lead to colic, or even obstruction of the gut. Occasionally a worm may wander into the stomach and be vomited up.

Treatment Mebendazole is the drug of choice in the UK, being given as a single dose. It should be combined with hygienic measures to break the cycle of autoinfection. All members of the family require treatment. Other ANTHELMINTICS include piperazine and pyrantel.... ascariasis

Artemisia Maritima

Linn.

Family: Compositae; Asteraceae.

Habitat: The western Himalayas from Kashmir to Kumaon.

English: Wormseed, Santonica.

Ayurvedic: Chauhaara, Kirmaani Yavaani, Chuhaari Ajawaayin; not related to Ajawaayin.

Unani: Dirmanah, Kirmaalaa, Afsanteen-ul-bahar. (Dirmanah Turki is equated with A. stechmani- ana Besser.)

Folk: Kirmaani Ajawaayin, Kirmaani-owaa, Kirmaani-ajmo.

Action: Deobstructant, stomachic, anthelmintic (effective against roundworms), antifungal.

A decoction of the fresh plant is given in cases of intermittent and remittent fever.

A. maritima var. thomsoniana C. B. Clarke is a santonin-yielding var.; A. maritima var. fragrans (Willd.) Ledeb. is a non-santonin var.

Immature flowerheads and leaves contain santonin. Roots, stems and twigs are devoid of santonin. Santonin, a sesquiterpene lactone, is used for the treatment of ascaris and oxyuris infections. Large doses (0.3 g is adults and 0.06 in children) are toxic.

Beta-santonin is less anthelmintic in action than santonin; pseudosantonin is devoid of anthelmintic property.

Studies is albino mice revealed that santonin had no androgenic, estro- genic, antiestrogenic, progestational and antiprogestational effects.

Santonin is toxic at 60 mg in children; 200 mg in adults. (Francis Brinker.)

Ascaricides

Drugs used to treat ASCARIASIS, a disease caused by an infestation with the parasitic worm Ascaris lumbricoides. LEVAMISOLE, MEBENDAZOLE and PIPERAZINE are all e?ective against this parasite.... ascaricides

Carica Papaya

Linn.

Family: Caricaceae.

Habitat: Native to West Indies and Central America; now cultivated in Uttar Pradesh, Punjab, Rajasthan, Gujarat, Maharashtra and South India.

English: Papaya, Papaw.

Ayurvedic: Erand-karkati, Papitaa.

Unani: Papitaa Desi.

Siddha/Tamil: Pappaali, Pappayi.

Action: Ripe fruit—stomachic, digestive, carminative, diuretic, galactagogue. Useful in bleeding piles, haemoptysis, dysentery and chronic diarrhoea. Seeds— emmengagogue, abortifacient, vermifuge. Juice of seeds is administered in enlarged liver and spleen, and in bleeding piles.

Key application: Papain, the enzyme mixture extracted from raw papain (latex of Carica papaya), has been included among unapproved herbs by German Commission E. Experiment-based as well as clinical research indicate that papain may be effective (in the treatment of inflammations) in high doses (daily dose 1500 mg corresponding to 2520 FIP units).

Unripe fruit—emmengagogue and abortifacient. Latex—applied topically on eczema, ringworm, psoriasis, corns, warts, sloughing wounds, carbuncles and eschar of burns.

Green parts of the plant and seed contain an alkaloid carpaine. Seeds also contain carpasemine.

Latex contain enzymes—papain and chymopapain and alkaloids carpaine and pseudocarpaine. A proteinaceous material from latex showed anticoagulant activity; in higher doses it is heart depressant and as a spasmogen on smooth muscle of guinea pig ileum. An alkaloid solution showed depressant action on heart, blood pressure and intestine.

The anthelmintic action of seeds against Ascaris lumbricoides is due to carpasemine.

Papain, an enzyme mixture prepared from the fruit, seeds and leaf, hydrolyses polypeptides, amides and esters, particularly when used in an alkaline environment, and is used in digestive disorders.

Papain inhibits platelet aggregation, which may further increase the risk of bleeding in patients also taking anticoagulants. Concurrent administration of cyclophosphamide with papain caused sever damage to lung tissues in rats. (Sharon M. Herr.)

Chymopapin C is an immunosup- pressive enzyme from plant extract. Carpaine, extracted from the plant, exhibited anti-tubercular activity, also antitumour in vitro, and hypotensive.

Toxocara

Liver Disease In The Tropics

ACUTE LIVER DISEASE The hepatitis viruses (A– F) are of paramount importance. Hepatitis E (HEV) often produces acute hepatic failure in pregnant women; extensive epidemics – transmitted by contaminated drinking-water supplies – have been documented. HBV, especially in association with HDV, also causes acute liver failure in infected patients in several tropical countries: however, the major importance of HBV is that the infection leads to chronic liver disease (see below). Other hepatotoxic viruses include the EPSTEIN BARR VIRUS, CYTOMEGALOVIRUS (CMV), the ?avivirus causing YELLOW FEVER, Marburg/Ebola viruses, etc. Acute liver disease also occurs in the presence of several acute bacterial infections, including Salmonella typhi, brucellosis, leptospirosis, syphilis, etc. The complex type of jaundice associated with acute systemic bacterial infection – especially pneumococcal PNEUMONIA and pyomiositis – assumes a major importance in many tropical countries, especially those in Africa and in Papua New Guinea. Of protozoan infections, plasmodium falciparum malaria, LEISHMANIASIS, and TOXOPLASMOSIS should be considered. Ascaris lumbricoides (the roundworm) can produce obstruction to the biliary system. CHRONIC LIVER DISEASE Long-term disease is dominated by sequelae of HBV and HCV infections (often acquired during the neonatal period), both of which can cause chronic active hepatitis, cirrhosis, and hepatocellular carcinoma (‘hepatoma’) – one of the world’s most common malignancies. Chronic liver disease is also caused by SCHISTOSOMIASIS (usually Schistosoma mansoni and S. japonicum), and acute and chronic alcohol ingestion. Furthermore, many local herbal remedies and also orthodox chemotherapeutic compounds (e.g. those used in tuberculosis and leprosy) can result in chronic liver disease. HAEMOSIDEROSIS is a major problem in southern Africa. Hepatocytes contain excessive iron – derived primarily from an excessive intake, often present in locally brewed beer; however, a genetic predisposition seems likely. Indian childhood cirrhosis – associated with an excess of copper – is a major problem in India and surrounding countries. Epidemiological evidence shows that much of the copper is derived from copper vessels used to store milk after weaning. Veno-occlusive disease was ?rst described in Jamaica and is caused by pyrrolyzidine alkaloids (present in bush-tea). Several HIV-associated ‘opportunistic’ infections can give rise to hepatic disease (see AIDS/HIV).

A localised (focal) form of liver disease in all tropical/subtropical countries results from invasive Entamoeba histolytica infection (amoebic liver ‘abscess’); serology and imaging techniques assist in diagnosis. Hydatidosis also causes localised liver disease; one or more cysts usually involve the right lobe of the liver. Serological tests and imaging techniques are of value in diagnosis. Whilst surgery formerly constituted the sole method of management, prolonged courses of albendazole and/or praziquantel have now been shown to be e?ective; however, surgical intervention is still required in some cases.

Hepato-biliary disease is also a problem in many tropical/subtropical countries. In southeast Asia, Clonorchis sinensis and Opisthorchis viverini infections cause chronic biliary-tract infection, complicated by adenocarcinoma of the biliary system. Praziquantel is e?ective chemotherapy before advanced disease ensues. Fasciola hepatica (the liver ?uke) is a further hepato-biliary helminthic infection; treatment is with bithionol or triclabendazole, praziquantel being relatively ine?ective.... liver disease in the tropics

Ziziphus Oenoplia

Habitat: North India and both the Peninsulas.

English: Jackal Jujube.

Ayurvedic: Laghu-badara, Shrgaala- badari.

Siddha/Tamil: Soorai.

Action: Fruits—stomachic. Root— given for hyperacidity and ascaris infection.

Stem bark and root bark contain cy- clopeptide alkaloids—zizyphines A, B, C, D, E, F and G, and zizyphinine. The bark contains 12% tannin.

Aerial parts exhibit diuretic and hy- potensive activity.... ziziphus oenoplia
